You should always hold the pick so there's just a very small piece of the tip sticking out. You have more control that way. Then when you want to strum fast you also angle the pick a little bit more down towards the floor. That makes it slide over the string easier. Little bit hard to explain that part, but there's plenty of youtube videos that shows how to do it.
I never really used alternate picking until I started wanting to play fast songs like this. Downpicking isn't too hard for me to be honest, Master of Puppets is quite easy (not trying to brag).

I don't play fingerstyle with my thumb like Geddy Lee for example. Playing Hardwired finger style without using three fingers is cramp city.

I was really struggling to play this because I kept trying to follow the screen to much. When I just sorta mesmorized the pattern in became easier!

Yeah, playing stuff slowed down is the best way to get better. Knowing how to play something slow makes it so much easier to play it fast.
I love the feeling of practising shit like this at real speed. My hand just goes "Wut? up or down, make up your mind..."
