Can still use it even though the commission expires date is expired?
I just go to my bank and use them...there are online notaries too that you could try...maybe call your county clerks office if you find nothing else. :)

Can it be a friend of a family that would take the child or it have to someone actually applying for passport also? Since box 4 only said name of an adult. Not name one of the parents. Thank You

This is paired with an application for a passport, so all that stuff that the child will need for that. Have you looked into that already? I have a video about getting a passport for kids too if you need more info on what you need. The notary will also want proof of ID when you sign the document in front of them.

In the form DS-3053 (Consent Form) the part 5 , my wife did not list the phone number and left it blank since she has issues with phone or is disconnected. But she listed her address and email and signed it too with notary. Is it OK if she leave the phone number part blank or should write N/A ? Or they can reject it for no phone listed/left blank? They both need to sign the form in front of a notary, provide copies of their IDs, and they need to provide a notarized letter signed by both of them indicating that you have permission to represent them.
in this form DS-3053 im now separated with my wife and she is a non-citizen but took custody of the kids. do still put her name section 5 as the person who can't come during interview since we're divorce.
She need to get the passport on emergency as she has serious emergency matter to deal with back home for 25th of June 2023. She is only held back by the baby and she just got the notarized form 3 days and ago and sighted the error

A copy probably wouldn't be acceptable unless you also provided the original notary signature.

I would. You should always include as much information as possible.

No, you don't need to fill up. this form is only applicable who are unable to attend the interview with child.
My husband signed just little bit up on the oath line is it acceptable and which address i will write where he use to live or where he is right now?
You'll want to include his current address, and the signature should be okay as long as it's legibly in the correct spot

They’re pretty strict on having the forms written correctly. I would re-do it and send her a new form.
can you fax the notarized form to the parent abroad?
who makes the copy of the ID presented to the notary? the courthouse in PA told me the notary should have made the copy and signed it.
I like redundancy and would take an original and a copy to a notary. The notary will make a copy for you though, use theirs.
