Why didn't the crew immediately notify ATC that they there was a suspected tail strike? Isn't there a risk that some piece of the aircraft could have been left behind on the runway, potentially posing an issue for other aircraft?

Not likely to have left anything on the runway other than pride.

If pieces were left behind after a tail strike they wouldn't have had to suspect anything they would immediately know.

It looks like the aircraft resumed its flight to SFO after 3:10 on the ground so presumably there was no significant damage.

Really did you listen to all? They returned to Honolulu. But only when they reached the ALANA holding pattern they told the tower of the tail strike, a long time after it happened

I wonder what the consequences of a tail strike are for the pilot. Probably some paperwork, a couple of meetings with the chief pilot, a reprimand... and possible a couple of sim hours if needed?

May 4th 2023: A TUI Airways Boeing 737-800, registration G-FDZZ performing flight BY-1424 from Newcastle,EN (UK) to Las Palmas,CI (Spain), was enroute at FL360 over the Celtic Sea about 150nm south of Cork (Ireland) when the crew decided to return to Newcastle reporting one of the pilots became ill, descended to FL330 for the return, later FL250, burned off fuel at FL110 and landed safely back in Newcastle about 2 hours after leaving FL360.

Is a tail scrape a pilot error or aircraft problem? Does the pilot catch heat for tail scrapes?

It's actually known as a tailstrike - pilot error.

Pilot error. You’d think, by the time they get to fly these sorts of aircraft, they’d have mastered the art of becoming airborne.

most of the time the flight attendants in the rear hear and feel it then notify the pilots.

The 757-300 is easy to tail strike as it’s so long. Probably pilot error though unless the weight and balance was screwed up.
