The script changing the faction according to what you are wearing is only applied to the player, if i remember correctly. You would need to change the clothing script to change caesar's faction

The sad part with stealth kills in FNV is that even if you are perfectly hidden, everyone knows you did it and your faction rep takes a hit. Killing Caesar for example drops your legion rep to rock bottom even if you are hidden and everyone magically knows you did it. Only exception is if you kill him with the surgery and manage to pass the skill check to convince them it was due to his advanced age. Wipe out a legion camp in the middle of the wasteland with no survivors? The rest of the legion magically knows you did it. That part of the rep system really annoyed me.
I don't know if this works for main characters, but I recently killed an NCR ranger by dropping TNT at his feet and shooting at it while being hidden, I didn't lose NCR rep even though I killed him, maybe this approach can be applied to other npcs like Cesar
Its not really like that. You can kill nearly all npc stealthy and not get any infamy. Its just the most important npc's that always give you infamy and theyre heavily guarded, so you can kinda imply guards have noticed your entry even with stealth boy. Also Im pretty sure radio comms exist in fnv universe.

I think the devs really wanted to railroad you into being hated by the faction if you killed their leaders. But the way the faction rep works is that the moment you start shooting, and you are not hidden, the entire faction magically knows you did it. Instantly. Like via telepathy. I think all bethsoft games have that problem.
sneak and crit run go together like peanut butter and jelly. a crit run by itself is really good, but when you combine with stealth you also get the extra x6 sneak attack damage which is insanely powerful, imagine a gun, just with 18 crit damage, now times it by 6. not to mention the perks that increase critical damage and weapon damage

If you're using the same build every playthrough, you're the one who's missing out. Switch it up and try new things. It doesn't have to always be about optimization.

Higher ranking NPCs (like those praetorians) tend to see through the disguise.

You can kill him and Lucius will give you quests instead. But that means Lanius is in charge and he just doesn't do it like Caesar.
