I was a bit obsessed with House during medical school. Then, when I qualified, I realised just how bad a doctor he is. He'd be struck off the medical register at least once per episode. On this Doctor REACTS video I'm watching episode 1 of season 4, called Alone, which brings up some interesting discussions on antidepressants, alcohol dependence and serotonin syndrome. Hope you like it!

I treat House less like a medical drama but more as a crime drama. A "who done it?" If you will... the "who" being the thing making the patient sick once the other "suspects" have been weeded out. House is largely inspired by Sherlock Homes whom was inspired by a real life doctor Sir Arthur Conan Doyle had known who could claim to simply look a person up and down and tell them exactly whats wrong. Once you get in the mindset that House is a Detective show, you can start to ignore the medicine
Yes, this is how I’ve always thought about House and why I love it so much. It’s a mystery and you get to try to solve it along side the characters on the show.
Yeah, the connection couldn't be more clearer
Holmes (home) = House
Watson = Wilson
They both use drugs, both have great deduction skills, both got shot by a guy named marathi

I never watched House for medical accuracy. I watched it for Hugh Laurie who is a really good actor. He's one of the nicest people ever and can still play an absolute asshole to perfection.
Medical accuracy isn't horrible from the diagnostic point if view. Accuracy of practicing medicine on the other hand is bullshit.
and he faked an American accent for 8 years!! I was shocked when I saw him in an interview and heard his real British voice.
The way I have always seen it is that the reason he needs a team is because they give him views and opinions that differ to his own, by being able to see it from a different view than your own helps to pick up on things that he may have outright dismissed. If you add the fact he is normally on drugs as well...
also so he doesnt have to directly deal with the patient and family
And he likes being in control. It's a bit of an ego thing.
Request- Could you do House MD s6e21 "Baggage"? It uses House's therapy session as a framing device for the the episode's events and has a lot of Dr. Nolan in it. I'd love to get your perspective on their conversation!
Seconded!! One of the best episodes imo, and it also shows a great deal of psychotherapy. Would love to see you talking about that episode!
From what I remember of the episode, it wasn’t the hospitals fault they had a misidentified patient. The guys actual girlfriend and the girl that was being treated looked alike after the building collapsed, and misidentified the body to the emergency workers, which led to a cascade of “why are these adverse reactions occurring.” If anything, I don’t think the hospital is at fault. It was a fun episode though.
yeah, the misidentification was from the families mixing the two up. They would still likely try and sue, however the lawsuit wouldn't hold up in a court of law due to the hospital treating the patients as they were informed, just informed ass backwards. lawsuit would be dropped and a judge would just give their condolences
House needs his minions (and Wilson) the same way Sherlock needs Watson (and the constabulary): to witness his genius. After all, House is Sherlock.
One of the things often overlooked with this series is, it's a Sherlock Holmes adaptation, which while subtle, once you know it's one of those facepalm moments of realization, because all though it's subtle, it's also glaringly obvious!!! lol House is Holmes, Wilson is Watson and Cutty is a bit of a blending of characters, but at least in the early seasons, very much Inspector Lestrade and Mrs. Hudson.

Had serotonin syndrome and yet had to figure it out myself and bring evidence to the doctors because no one would listen to me. Went to A&E three times in 2 weeks, each time with worsening symptoms but sent home each time and told to follow up with my GP who didn't give a shit. So much trauma from that time

But according to the product inserts and drug-interaction checkers for the medications I'm on, yes, yes there's evidence of it, lol. Oh, and per my own psychiatrist.
@CV MUSE He talks about it in this video without questioning it's existence or saying there's little proof for it so I think that suggests there's some proof for it. People have died from it (there's on somewhat well known case in America). Also if you look at the leaflet that comes with any SSRI medication it's listed in the side effects as a serious condition to be aware of.
On a separate note it may not have been your intention but leaving this comment after reading mine is somewhat offensive. I talked about an experience that was traumatic for me and your response was to question if it were real or I essentially was imagining things/making things up.
Ah the No Hope Service. Murdered my gran. Murdered my gf by negligence on her 22nd birthday. Misdiagnosed me since 1981 including cancer for a decade (now on #5) Fibromyalgia, Autism and misophonia. That's why they're about to be sued for £7.5m.
@Baconburguer20 serotonin syndrome is very unpleasant way to die and Harri whom I'm assuming is of the feminine persuasion is very very lucky she's not dead. It happens because cretin doctors don't check what meds/supplements are being used. If you have two drugs that raise serotonin you get the brain effect of running a car engine without coolant, it literally burns out the neurons. It very much exists and she's very very lucky not to be a corpse.
Another one I had was histamine cascade. My lungs blew out, because my doctor was dumb.. Mirtazapine stopped cold turkey.
Another brilliant video, I really appreciate you showing the role of Psychiatrists in managing psychiatric presentations in other physical heath issues, and in managing physical health issues in patients with mental illness. The drug interactions with tamoxifen and antidepressants is a great example.
I actually found it very upsetting to watch that doctor say that she was okay with a person suffering because their injury is related to drug use. Unfortunately those attitudes exist, if people don't usually come out and say it so blatantly.

I had surgery in 2019 and I was shaky when coming off the anesthesia (I'm a lightweight.) When I was rolled back to where my mom was, still shaky, the nurse said they could give me something to help it. It was Demerol. My mom knew I shouldn't be given that because of my sensitivity to medicine in general, plus it was an opioid, and the lady injected it in my IV anyway. My blood pressure nearly bottomed out (I tend to run low normally,) and I passed out before they gave me something to raise my blood pressure again. Needless to say, I got very sick from that experience 😖

Your comment about the sad reality of medicine and how some may « rank » their priority of who deserves treatment hit close to home. I suffer from chronic pancreatitis due long term alcohol abuse and on the 2 occasions I had a flare up and had to go hospital (in an ambulance, mind you…trust me, the pain is intolerable) , they made sure I was not treated as a priority in order to « teach me a lesson ».

I'm so sorry you had to go through that... It makes absolutely no sense to me to treat people like that. Do they really expect that people will go "oh, they treated me like shit in the hospital, I'd better get my shit together and steer clear of substances in the future because I want to be treated well in the hospital"? That's not how humans work. In fact, ostracizing and mistreating people to "teach them a lesson" has the exact opposite effect. Multiple studies show that ex-addicts (and ex-convicts!) who are welcomed by their community and supported in building a new life and finding a home and a job etc. have MUCH greater chances of staying sober, than those who are cast out and treated like garbage.
For most people, drugs and alcohol etc. fill a hole that their life can't fill. It's not something you can just choose to give up without filling that hole with something else. And that hole can be filled with human connection, love, a sense of purpose and belonging, etc. By mistreating (ex-)addicts and (ex-)convicts etc, people are just making that hole larger and increasing the need for those people to self-soothe. Punishment has never gotten us anywhere. What we need is love, empathy, connection, and understanding.

I've had a puo for 7 yrs . The rheumatologist ran out of tests to run , tropical disease department had no clue so I've been left with a dx of "unspecified autoinflammatory disease " code for no freaking clue 😂

Immediate family members are burdened with decisions of care for these situations in the US. I can’t speak on whether the show portrays it accurately, as I’ve never been in that position, but I know that my mom had to sign off on all of the medical decisions surrounding my dad when he was sick and unable to communicate. It’s interesting to hear how the UK handles these situations differently.
It’s good drama, that’s it… the medical setting is just a way to deliver that. Thanks for the video btw, as enjoyable as always.
I just thought about the medical bills people end up with after being referred to House. I am not sure how worth it it would be to live through some of these episodes because I'd imagine a good 95% of the people that go through all these extreme treatments would have to apply for medical bankruptcy because this show is based on the US system and medical costs are astronomical here. I think in many of these cases if I knew that treating me would involve even half of the amount of what they do in most of these eps, I would choose to die instead just so my family wouldn't drown in medical debt to try to keep me alive.
This is a horrible choice and one that shouldn't exist.

@@roselover411 I always found it terrifying when people were reluctant to call an ambulance when they needed one or visit the emergency room as often if you leave things they tend to get worse.
@@nat3007 It's because we can't afford it. Especially if you don't have insurance but don't qualify for government assistance. You're facing with thousands of dollars in bills you simply can't pay. Ambulances alone cost a ton of money just to ride in one because they're privately owned and so can essentially charge however much they want and there's nothing we can do to negate the charge.
@@nat3007 And there are also people, MANY people in America that ration their medication like insulin and leads to many needless deaths all because the government doesn't force pharma companies to negotiate prices with government like most every other country in the world. My brother's insulin here in Canada has always been 80% covered by the government and he can easily afford the difference, thank god. America needs to get their priorities right and stop being so obsessed with whether something is "socialist".
I once had a case of Serotonin Syndrome as a result of drug interactions. I suppose I got lucky, since I didn't get into dangerous territory, but I felt like I was on a huge dose of MDMA or some other serotonergic drug. Seriously people, be careful with these interactions
Paracetamol was vital in getting my degree - I needed to give an example in my toxicology viva and this has an interesting cause of toxicity
I watched House from day one and expecting medical accuracy on a TV show is a tall order. In any case, it always bugs me is the pattern of wrong, wrong, wrong, then finally right, and most the time patient has a good outcome. Also, the time concept, obviously they have to cream into a 40 min episode, but a lot of developments wouldn't be overnight, but rather measured in weeks.
I can't get over how mean House is to many of his patients.
His patients come to him for a chance to live, not a chance for politeness.

The procedure in Britain is different than the States. Family does make the call here (something I’m grateful for)
I like House, but when I saw this episode and the female doctor mentioned leaving the drug user in pain it really irked me too, definitely a product of older times but it still happens. I've been in the hospital for drug use related problems and it's definitely a noticeable attitude in some doctors still. Luckily I've never had it go beyond noticeable disapproval, I didn't ever feel like treatment was withheld from me. The worst thing that happened is I asked to be referred to VNN (our national Drug Addiction treatment organisation) and the doctor just said "yeah that's what they all say, but next week you're back at it" (roughly translated) and left.
I have chronic pain and go to the ER when I can no longer manage on my own during a flare up and the pain is intolerable. They will sometimes ignore me for 4+ hours while I'm crying in pain because in our medical system I'm viewed as a "drug seeker".
@@yourlittleinsomniac5369 I'm so sorry to hear that. I can't believe they'd do that, it's so unethical and how can you be a doctor and leave someone in pain for 4 hours? I am studying to be a doctor and I just can't imagine leaving someone to sit and wait in extreme pain. If they come to the ER, obviously it's serious. ESPECIALLY if they have chronic pain! People with chronic pain know they're pain, if they're in the ER, it's serious.
I'm sure they deal with drug seekers but man. I'd rather give 1 drug seeker a dose of morphine once than deny 10s of genuine people in pain because "they're probably pretending". Just awful. Sorry to hear that :(

Regarding your comment about the mental capacity act, by family input does that refer to any kin unit or only legally recognized kin (parents, sposes, etc). And if it's only the legalised kin, have you encountered situations where legalised kin is valued differently than queer "choosen" families/kin?
(Complete aside, I sympathize with professions ruining TV fun - I was very disponted in undergrad to find out that "Bones" bioarchaeology and real bioarchaeology are nothing alike!)
Have you considered getting real subtitles for those who are death, hard if hearing or aren’t able to listen to your videos due to other circumstances?

He's a genius diagnostician with his own specialized team that focuses on rare cases no one else can diagnose. He's forced to work the clinic for all the ridiculous things he does to get his diagnosis on those cases. In other words he deals with zebras exclusively.
But he is knowledgeable in almost all medicine (and countless other things) due to the fact that his father was stationed all over the world in the military and all he had to do was read for fun.
And also aren't benzo withdrawals more deadly than alcohol withdrawal?

Whilst I have never had DTs I have had Alcohol Withdrawal and Benzos withdrawal and I would take the alcohol withdrawal ANY day of the week.
Fentanyl withdrawal was, without a doubt, the worst thing ever. I’m still pissed a doc thought that it was appropriate for what he thought was “only” fibro. Turns out it’s CRPS/RSD, but it’s still not indicated and I didn’t get diagnosed properly til years later. Now I have an SCS (spinal cord stimulator) implanted, and being a cyborg has helped a lot, but oh my god that withdrawal.
@@sarmajere2866 I had never heard of a spinal cord stimulator, could you perhaps explain to me what it does for you and how it's helped you?
I'm sure it won't be something useful for me but I'd like to learn nonetheless. I have chronic back pain, all the way from the base of my spine to the base of my skull, and due to the pain and my chronic fatigue and ADHD it's virtually impossible to get me to do exercises for it. I've lost count of the number of therapists I've seen for it. But the pain hinders and impedes me every day, in almost everything that I do, and I can hardly be taking painkillers 24/7 without screwing up my liver. I just want the pain to stop so I can actually live.

I had a psychiatrist suggest MAOIs to me a few years back. We'd tried a whole bunch of different meds already. The problem wasn't that they hadn't worked though, it was that I'm just so overly sensitive to side effects to the point that they completely cancelled out any of the benefits of each drug. We were running out of options, but when he told me about all the stuff that goes with MAOIs and food you can't eat etc, I refused to try them. It sounds hardcore and has way more side effects even before you consider my sensitivities, AND you have to watch all these foods when I was already a picky eater. It just didn't make any sense to give them a go. If mild antidepressants mess me up so bad, why expect MAOIs to be any better!? 😨
In the States, Demerol is called Meperidine. Why the difference? We also call paracetamol, acetaminophen.
From a diagnostic standpoint, wouldn't using ethanol be a better solution. If it worked, then it would be a clearer sign as to the cause? A benzo would be great for dealing with potential alcohol withdrawal, but using such would narrow the possibilities as greatly.

I remember this episode, so I know how it ends. However, if you're going to bother discussing the case, see it through to the end so that people who've never seen it can have closure.
Is there a reason SSRIs are prescribed more commonly than SNRIs. I've had both and for various reasons much prefer SNRIs (I won't list the reasons because every body is different so I don't think it's helpful)
According to the almighty internet it's because SSRIs tend to cause fewer side effects on average than SNRIs. Sounds reasonable to go with SSRIs as a first choice then, though obviously it won't work as well in everyone. It feels kind of similar to my experience with ADHD meds. Psychiatrists generally prescribe methylphenidate (Ritalin, Concerta) first, which is a dopamine and norepinephrine reuptake inhibitor, but it had no effect on me so I got dexamphetamine instead and that works better. Dexamphetamine is mostly a dopamine and norepinephrine stimulant/agonist. But there's another reason in this case, because (if I remember correctly) methylphenidate is cheaper, so they kind of like to push you in that direction anyway. Here in the Netherlands ADHD meds generally aren't covered by insurance, but if they're covered they usually only cover methylphenidate and not dexamphetamine, which is just plain financial discrimination if you ask me. Because some people with ADHD can get their meds covered, while others can't, simply because their bodies happen to respond better to one or the other, it's just a matter of luck. And that doesn't seem fair to me.

One question (hypothetical): Benzodiazepines can be also very addictive, isn't there a risk to the patient (let's say someone more susceptible to addiction).?

Well I mean they dug her out from under a buildering with her face crushed, she had the other girl's ID, and the family identified her. I don't see the hospital's liability here. It's not like they swapped two patients' rooms around, this is who they were told she was. They never even had the girl they thought she was because she was already dead.

Is there any reason you'd prescribe Citalopram over Escitalopram?
In the UK, escitalopram is difficult to get hold of and people have a false perception that its more expensive than citalopram, so its more a matter of convenience and availability than anything scientific

- I keep hearing that a lot when medicine is described. that we don't know why it works. Its kinda freaky to think about what the percent of meds we use daily that might fall into that category. Like is it just more of an expression and we do know but its just so technical its hard to put into words or do we really have like 25% or whatever of meds still not understood?

This episode is one where House's judgment is at its worst, becayse of the whole no team thing. Great review of the pure medicine part of it.

My Dad hates the incorrectness of House too. 😂 Also, my Mom had Lupus, so any time they loved to say, “It’s not Lupus. It’s never Lupus,” we rolled our eyes so so hard. Yeah, we get it’s non-specific to a degree, and back then maybe the bloodwork was iffier and needed higher levels to get caught, but it CAN BE LUPUS!
Working in a pain management clinic now, I’ve heard of people denying their results in the past because they were like, “it can’t be Lupus,” ~scoff.~ I thought, “Ah, House fans.”

@@tahraethestoryteller6079 Won't get you high, you'll get serotonin syndrome like he talks about here which can cause altered mental state, fevers, rapid heart rate, high blood pressure, seizures and, if left untreated, death.

House might be awful at bedsides manners but he is a great doctor in that he's able to solve some very rare medical diagnoses.
At the end when you joked about there being a lawsuit because they got the wrong patient, that's incorrect because the boyfriend was the one who identified that woman as his girlfriend in the first place.
I totes told my specialist that I did my 8 years (by watching House) he did not prescribe Vicodin or or anything else good!
Taking out booze with benzo makes me think about putting out a bush fire with hydrogen bomb

House got annoying after you realize that, he wouldn't realistically get to keep his job or his medical license. I personally think that mental hospital story arc, was an attempt to address that elephant in the room.

Can you imagine someone wanting to be a doctor because of this show then they become a doctor and realize well I'm glad I'm an actual doctor now and not like House who is actually horrible 🤣

With House it’s not necessarily an ego thing. He needs the people as a sounding board. He is actually a lot less confident than he lets on and you see that as he lets the wall down a little bit in later seasons and you get glimpses of him second guessing himself and seeking approval. He doesn’t need the team to show off to, but to call him out on his crap and keep him in check as well as the validation that he is doing the right thing. He annoys people because he psychologically needs the attention and to interact with others, but he doesn’t quite know how to go about it. His general affect paired with his high intelligence and attitude paired with his erratic behavior and fascination with how the minds of the mentally ill work and how they adjust to society I would think he shows signs of high functioning autism which in his late 40s or early 50s in the 00s when the show took place it was likely fairly unheard of when he was a child to be diagnosed and just never bothered to get tested as an adult and autism in adulthood can be easily overlooked unless you’re specifically looking for it and his only experience with psychology as an adult was through drug treatment where they were more focused on his sobriety and after he drove the car into his boss’s living room and that was more so about how he was adjusting to being back.

@@tahraethestoryteller6079 Yeah, thanks for asking. Physio told me I can stop coming at the end of my current prescription and my neurologist is full "yeah everything is fine just keep taking your meds".
10:17, I 100% agree. But for scars, not drug addiction. I went in for surgery last year for severe chronic abdo pain (laparoscopy & IUD insertion). Pre-op, I was to be given take home meds. Post-op, after a dozen or so people had seen me naked, seen my scars & frankly, after I'd allowed them to sexually assault me (the IUD), there was suddenly no take home meds. I went home as the morphine was wearing off & was in so much pain (9/10), I couldn't breathe properly. I had to take 9 buprenorphine tablets just to breathe (normally would take 2-3). If it wasn't for me accurately predicting they'd discriminate against me in the OR (due to my scars, chronic pain & sexism: women recieve 20% less pain meds in ER settings), id probably have died that night (id gotten an extra script from my GP the day before). It happens all the time too, only my GP gives a damn about my pain & prescribes meds to cover it - ive had pain specialists scoff & shame me for taking so many opiates, even though I can't help how much pain im in (intolerant to other meds). So many of us are forced into euthanasia just because we can't obtain scripts & drs just want us off everything. It causes anxiety & depression, knowing id have to end my life if my GP stopped prescribing. And i cannot imagine how much harder it would be if you're an addict with chronic pain (50% heroin users are chronic pain sufferers). Why can't we be more compassionate?
IUD isn't sexual assault you're undermining real sexual assault and demonizing bc simultaneously by saying dumb shit like that. Great job giving ppl more reasons to not listen to women.
Not sure if this holds for other places, but I’ve been told in Florida that ER docs aren’t really able to do much for take home pain control these days, or to do more than asses and control. This means that when I know what a problem is, I usually don’t bother with the hospital since it won’t end well for anyone involved.
I’ve also had a pain specialist scoff at me and give me an exercise program that made my fucked ankle and leg swell and turn vivid purple so badly that my primary doc sent me to ER for a CT scan to be sure it wasn’t a blood clot. Turns out it was CRPS/RSD, which the pain doc didn’t catch at the time. I did transfer after that and eventually got diagnosed by a specialist but I know that feeling so, so we’ll!

@@FS-qk5uq @F S This is the kind of intolerance I'm sick of. Ive also been sexually assaulted (attempted rape) & the loss of control felt similar, except that I could remember one & not the other. Don't delegitimise what I went through just because other women endure worse & aren't believed. Rampant sexism is not my fault. My pain is not my fault. Its def not as bad as rape but I have refused to let anyone near me since my early 20s because Im sick of being sexualised, for 10 years no man been able to hurt me yet i had to let doctors do it, while unconscious & let people insert objects just to hope it would reduce my pain (it hasn't & i was judged when unconscious, couldnt even defend myself). Im guessing you've never experienced eitherkind of violation. Don't be so horribly judgemental. I literally asked for more compassion & i get this. Im not even surprised. Its only ever *your* fault if you don't listen to sexual assault survivors testimony, how dare you assign blame to me.
@@sarmajere2866 Me too, the 'excersise til you drop' stuff doctors push is often ill-advised & OTT (I actually go on the bike for 1
& 1/2 hrs/day and yet im still in agonising pain, it doesn't improve my pain, i just do it for fitness & leg strength so I can squat instead of bend over to pick up stuff, but most people with pain aren't this mobile). Yep, you're also right about the meds in ER rooms - except I was expressly told by my gyne to go to the ER (Western Australia) when I had severe pain, only to be told (after 7 hrs) to take what I had at home, b/c they dont give out meds...i reported this & apparently they'll 'be reminded' the ER isn't for fobbing off pain patients to! Conversely, the post-op meds were supposed to be given to me (both the lovely nurse who was in post op & my GP were shocked they refused, the nurse even queried the surgeon in front of me, he was like "she's 'had enough'"!). But, like I said, prejudice abounds. Its easier for their livlihood to fob us off onto 'deep breathing' & exersise than take some degree of liability & prescribe. I'd foolishly assumed chronic pain patients would be treated the best by drs, now I know we're the lepers no-one wants around. Makes you vulnerable, which scares me.

i think u kinda missed the whole point of the show, if someone gets to house for medical reasons, it means that they went through regular medics already and exhausted all regular possibilities. this wasnt just a medical show haha ♥
