I do feel like dementia should be a trait, maybe a hidden late onset congenital one like melencholic or lunatic can be. I imagine it should actually effect dialogue event options. I think that infirm should be changed so that you can't do any event that requires physical exertion like hunting the prowess penalty should be more catastrophic maybe instead of -4 it should give you a 90% penalty to prowess and you obviously shouldn't be able to command armies or participate in combat except for self defense from hostile schemes. But on the other hand I think that they should either reduce the massive health penalty, or allow you to counter it more intentionally by hiring a court aid position like you said. All the current strategies for stacking health buffs to prolong your life are not unique to being old and I feel like you should be able to take some unique decisions to prolong your life specifically once you are old. Finally I wanted to say since you didn't bring it up the lifestyle perk know thyself makes you completely immune to death from disease during for it's duration, it also makes any efforts to improve your health completely futile as they will be ignored even if you get your health back up to "fine". As you said the death roll has already occurred and know thyself will give you exactly 1 extra year of immunity from natural causes even if you have every single disease in the game simultaneously, it really is the cherry on top for just how overpowered old people are in ck3.
Frankly being old is so overpowered that I wouldn't be against more interesting consequences, maybe dementia could cause you to forget about hooks, maybe events could occur that would cause you to lose legitimacy, maybe you could be given the option to abdicate without having to induce a mental breakdown from stress. The game could do more to encourage passing the torch now that there are so many more options to extend your lifespan with seemingly very little consequences.
Well written and well said, I agree with everything here. They could play with the dialogue a lot when it comes to dementia. Dementia patients can repeat phrases which offer insight to their past, although not always in the correct context. Maybe at a certain point your character just starts cycling through the same phrases which could be good or bad depending on your history. Ex: "You know? My son fought in a battle once! ," which might give like 25 prestige each time or, "Wow you're fat! Your husband must be a great hunter!" Which might give like -20 opinion. I'm tall and each time I see one of my residents he asks me if I play basketball no matter the context.

I hate how your character is strongest right before death. Even with infirm trait. How are 80 year olds smarter than people during their actual prime? Aging is way too forgiving now. No dementia, illnesses, etc as you mentioned

Good video. I also think the bonuses from medicine learning focus tree just need to be nerfed. I feel like doing that just makes you guaranteed to live ridiculously long for the time period everytime.
Yeah, but I do think a lot of the mental stuff is due to ppl now dying like 20 years later, like during the middle ages people (ignoring child/infant mortality) would usually die in their 60s or 70s of some kind of disease because the immune system weakens as you age. Also there was no understanding of strokes or heart disease or cancer or similar. So I do think people should die a lot more "aware" in this game.

do you think that maybe your impression of older people is colored by where you work? My grandparents all had very different reactions to being old, one was totally forgetful (dementia) while another was riding bikes until he was 85~.
I could definitely see that. I might have a bias because I see a lot of old people who have to come here, as opposed to old people who are doing fine on their own
