That was a wheel from a rendition of the wheel of fortune, but had some anomalous properties. Each spin leads to a misfortune to befall the spinner. Except in very rare cases, where they could get rewarded with fabulous prizes, including the rewards of various anomalous gifts. Some even being duplicate instances of various SCP objects.

It was the side effects from being resurrected in the way used for him, which he then assumed to be what simply happens to everyone, and do did the rest of the 05s. So perhaps the cognitohazard part is real, and the Foundation ironically created it because of what they did. But most likely that's all it is, a cognitohazard. The aftermath was probably unique to just the one guy.

About SCP 2030, I'm pretty sure it isn't confirmed the "guests" are the souls of the recently deceased. Just that they have been recorded as having died of natural causes. SCP 1155, the one doctor Bright took out with a bomb vest. SCP foundation should start using suicidal bombers more for all anomalies that kidnap their victims. Its atleast worth a shot if the blast kills the creature or atleast destroys its home and the explosion could be tracked.
SCP 4205, for me has always been one of the scariest ways to go. The idea of thinking you're still alive although you have died and are basically just very slowly going through the process of dying is just kinda terrifying to me
