Had a transperineal biopsy last summer following a psa test of 5 (picked up during investigations for another health issue). Had local anaesthetic injections first then 30 biopsies- didn’t really feel anything, worst part was when the doctor sprayed the area at the end with a disinfectant type spray - it really stung ! I was able to go home about an hour later when I had passed urine. Turned out all areas of my prostate had cancer (I had no symptoms of this at all) so I chose to have it removed robotically a few months ago, and am now waiting for the first psa test result which I had done yesterday.

I had 32 biopsies and my prostate was so swollen I had to have a catheter left in for 3 weeks.
Had my prostate removed last December by robotic surgery.
That was less painful than the biopsy.
Just passed my second blood test at 71 years.

Thanx for this video. I'm looking at a biopsy in the next few weeks and am very apprehensive about it. My dad had one done around twenty years ago, and he said it was the most painful thing he'd ever experienced, so watching your video gives me hope that there are now significant steps taken to lessen the pain involved with the procedure. I wish you a complete recovery and a very long and happy life. ☮✌

PSA 18. Started ADT. Biopsy time. Had mine last week in November '23. Was given antibiotic for 4 days before. Drove to the office. Dressed in lower part of gown and rested on my side on the bed. The Dr. and assistant on the ultrasound machine. Lubricated the ultrasound probe and inserted. He then sprayed "the tender spot" with a local. 12 cores taken. Tiny shock, like a static spark from walking on a rug. The Probe was the most uncomfortable because of hemorrhoids. Less than 30 minutes after I arrived I was driving home. Continued Antibiotics for 4 days. No aftereffects. No pain, no blood, no lingering soreness. Gleason 9. Looks encapsulated. I start radiation in 3 days. 79 Year old.

Good on you for driving yourself there and back! If they hadn't given me Xanax before the biopsy I may have been okay driving myself- but for biopsies and colonoscopies I try to arrange a ride. 😁 Good luck with the radiation and other treatment. Hang in there! 💪💪

I've had 3 prostate biopsies in the past. The last one was the most painful. At no time was I given anesthesia at any of them. I was diagnosed 8 years ago with an aggressive cancer of the prostate. I had brachytherapy plus external radiation and hormone therapy. It was a year to remember for sure. I'm fine now with a PSA of 0.008.

Wow that is a blessing to hear your story, I'm having brachytherapy next month, been on ADT hormone therapy for 3 mos now. I managed to get my psa down from 40 to 6.36 taking soursop tea and apricot seeds. Doctors also want to followup with external beam radiation. So my story is similar to yours. No recommendations on removing the prostate.

My biopsy went well. Uncomfortable is the best word that fits. Actual pain is minimal. The whole procedure took 10-15 minutes. I'm a little sore, but that's it. Prostate is enlarged but not abnormally for my age. No tumors or lesions. We'll see what the tissue says though. Pain wise breaking my foot and diverticulitis was worse. Look forward to healing up though. 😊 I was wide awake and drove myself home right after.

Yup, same for me, not really a big deal at all.
Just a doctor and nurse, ultrasound went in, local anaesthesia injection went in (most painful part, but not that bad really), then 12 core samples (each feels like a baby electric shock, but not painful). Done.
No sedation should be needed for this.
Afterward just a dull ache in the prostate for a while, kinda feels like you need to pee constantly.
And be aware, ejaculating looks like a murder scene for a few days lol

I had a prostate biopsy last week, after PSA test. Hey, I wouldn't put a biopsy on my bucket list, but it wasn't as bad as the stories I heard. The click went about 12 times. I felt no pain at all from the actual needle that takes the biopsy. Just an uncomfortable sensation of obviously getting that up your backside. Not unbearable pain, just uncomfortable sensation, but to be honest, not anywhere near as bad as I thought. They put me in a wheel chair, took me in a room to rest for 30 mins with some tea, and I walked out of the hospital fine about 40 mins later. Told me I might urinate blood and from my backside when go to the toilet. Urine was fine, a little blood from the other side, but on a whole, fine. Worst part is waiting for my results. So don't worry about the biopsy. Not that bad.

I just went for a LATP biopsy (local anesthetic transperineal prostate biopsy) last month. I asked for half of the typical dose of local anesthesia and was granted, no relaxant either. I took one single Tylenol 500 mg right before the procedure instead of two as prescribed. I took one single dose of Keflex (cephalexin) 500 mg two hours before the procedure to reduce the chance of infection.
The experience: No pain at all even when my urethral was accidentally pierced through (fresh blood coming out from the tip of my thing). Throughout the procedure, I chatted and laughed with the doctor and the nurse.
At home, I touched the area where the needles went in, there was no pain either. To compare: a dental root canal is much worse, I tell you. I was back in my regular activities the next day, sans exercising.

I had the same procedure last week (very very painful...wish i took something like you) and just got the results today. Came back normal, no cancer, no BPH and no evidence of prostatitis, although my MRI from a few months ago noted mild BPH, current or past prostatitis, with a 1.3 cm focus in the transition zone. It was suspected I had prostatitis causing the appearance or other conditions on the MRI that since cleared up and also caused the mildly high PSA and low free PSA. I'm relieved. ;) Also, my MRI prostate was almost 30 cc in size but by the biopsy, it was down to almost 25 cc, normal size. Sorry to hear about your situation and will watch to see how you're doing now. Thanks for sharing

It sounds like experiences vary greatly. My doc had me use a Fleet enema first. The first needle stick into my prostate was extremely and shockingly painful and I kicked a wheeled tray across the room and yelled out in pain. The Lidocaine took effect between the first and second samples and then all was numb. Afterwards, the nurse handed me a bag of pads to put in my underwear since I was bleeding. It stopped after a few days. My next several ejaculations were blood red, then increasingly light pink. There are worse medical experiences, I'm sure, but this one was worse than I anticipated and I hope to never have to repeat it.

Biopsy under general anaesthetic is definitely the best way to go, you're out of it for about 20 mins, Yes you ache afterwards, but that's what painkillers can deal with if you need some. Best decision I made, luckily advised by a mate who went for a local Anaesthetic before me and almost passed out before his biopsy so they advised him to have done under general anaesthetic. Just waiting for result, fingers crossed as mine was found at very early stage 😊

It really isn't or shouldn't be full general anesthesia. No reason to breath for you when dulling pain is the only goal. I watched as a team of medical personnel used a very long, thick, hollow-core needle to remove three core samples from my liver. A biopsy. No pain at all. I was Very talkative (from the meds) and curious. Surprised I did not faint as I actually watched the needle jammed in bit by bit into my chest. All eight inches. Three times. It was like watching yourself get shot but, thankfully, no blood. And, I knew and clearly understood that they were putting deep holes in me but I just did not care. Surprising reaction. Alas, all cores came back normal so they sadly missed the targets three times. But, I'm still here to talk about it. By the way, for the curious, they do seal the holes in you after ALL is done. The plasticizing sealant used lasts longer than than actual healing process time. You can bathe and wash as normal; just no hard scrubbing. So you go home a few hours later (meds wear off fast) and no more needs to be done. No need for three individual stitches or anything like that or removing such things. Interesting experience. Recommend it? Not unless you have to.

I'm overweight so that didn't help. They did about 3 MRIs trying to get it in right and I finally told them, "Just shove it in it won't hurt me" which they did and it was not uncomfortable at all. I had no tranquilizer. I felt no pain or discomfort except for some groin pain a few days later for a few days. Unfortunately, they did find cancer rated Gleason 7. I had a PSMA PET CT scan to see if it spread. I'm finding out tomorrow.

I had a transrectal biopsy in Luxembourg last week after having a psa of 5.2 and a 1cm lump on the left side shown by the MRI. Although nervous it couldnt have gone better.I was given antibiotics to take 1 hour before arriving at the hospital. I arrived at the hospital at 7, was taken to a ward where I changed into special pants, gown, socks and hat. Then a short walk to the nurses station where I was fitted with heart and blood pressure monitors and a short wait. Then into an operating room onto a bed with movable leg supports. Here I was fitted with a saline drip and the anaesthetic which didnt quite knock me out but all I could feel was a slight pressure. "20 minutes later I was back in my bed in the recovery room for two hours (my blood pressure crashed) and then home. There was quite a lot of blood and lumps in my urine at first but that cleared up within two days and apart from that no pain, infection or side effects to mention. I will get the result in 3 or 4 days. Apart from a certain amount of indignity this is not something to be feared at all.

@@richardc6932 My results showed that I had cancerous cells in most of the Prostate and the Seminal Vesicle as well so a gleason score of 3/4 and stage 3 cancer. I followed this with a full body tissue scan and a full body bone scan to see if there was cells anywhere else but that was negative. I opted for a robotic prostatectomy which was a 5 hour OP and 4 days in hospital. The 5 days with a catheter were uncomfortable and the three weeks since its removal have resulted in a certain amount of leakage and the loss of an erection but i am having physio and medics to cure this and should all be cleared up within 3 months.. What is also often not mentioned is that the OP causes your penis to shrink 3 or 4 cm. Also I had to have 20 daily injections and wear really tight stockings for 20 days to prevent any thrombosis which were very unconfortable. The 6 holes in my abdomen have healed up and never gave me any trouble. After 3 weeks I was able to restart quite a vigorous workout regime (I teach spinning classes in a gym as a hobby as I am now retired) and in two months I will have more blood tests to see how my PSA is looking. If there is further signs of cancer I will then be having radiation treatment. I had a in office prostrate biopsy done about 10 years ago. It was the WORST Experience of my life. I still have nightmares about the procedure.
I am totally impotent as a result. No sex life at all!!! Because there is no climax, no ejaculation, no feelings. Dr said it was one of the side effects. I almost returned to the alcoholic lifestyle. If you are waiting for a happy ending forget!!! MY wife and I sleep in different rooms, sometimes we watch TV, go to the movies, or go out to eat. I'm still sober. I call the building where the procedure took place, the house of pain. In my mind, the sign above the reads," Abandoned all hope he who enters here." Dante Inferno
I asked my regular DR. If I could have a prostrate exam. after the exam my DR. said, there is nothing wrong with your prostrate, there are no lumps, it felt normal to me.

I'm 56 and had no clue about what I was in for when I had it done. Out of the 12 needle core samples only 4 of them were tolerable to me. I hated the 8 remaining but you get through it, not fun at all nor is the ride home sitting.
I had a gleason score of 4+4 (8) with a diagnose of a contained malignant mass. I had the robotic surgery done 4 weeks ago ( July 27th ) to remove the prostate, that's a fun time too. Catheter for a week made sleeping difficult. Look, none of it's fun but what's the alternative? I will say the whole cancer thing taught me many thing's about myself, my faith and others around me.
The drug/hormone prescribed before surgery to stop cancer growth Bicalutamide made me emotional, needy, no appetite and unable to make simple decisions. I lost 20 lbs and I was a fit person, so I didn't look well losing that much weight. The doctor took me off the drug about a week before the surgery. To put weight back on I drank a 350 cal Ensure plus shake with each meal I ate. That's 1,050 added calories to my diet, so far I gained 13 pounds back.
My wife was a tremendous help at home both physically and mentally. I hope others can be as lucky to have the support I've had from family, friends and work associates. I'd also like to mention I feel blessed because this is one of the many times the lord has answered my prayers.

@@Delani11 Well brother it's been a heck of a ride the last year and a half. After the robotic surgery removing the prostate my psa was at .08 then began a slow climb .12, .14, .19, .27, .34 and .44. Apparently I still had some cancer in the pelvic bed but nothing in my lymph nodes Doctors felt it was time for radiation (after 2 pet scans and a MRI) 38 sessions total and hormone therapy for 6 months (lupron shots). Radiation wasn't bad but you have to have a full bladder every time when getting treated, not fun trying to hold it without a prostate.
Hormone shots have a host of side effects, for me it was some muscle mass loss, sleepy, no sexual desire, because it literally knocks out your testosterone. Legs are achy, swelling in my lower right leg/ankle, night sweats and hot flashes during the day. The good news is that for now I'm looking in the rear view mirror. I had my final shot about 3 weeks ago an my last 2 PSA results were < 0.02 ng/mL AWESOME !! That's where we want to be. I feel blessed and thank god, I do know there's always the chance of recurrence but for now it's good times! My message to all is when it gets tough take it day by day, hour by hour, foot by foot or inch by inch. Your mental health has great effect on your family and well being. Cancer doesn't define who we are it's a reminder to be ready for a fight.
Hopefully you're doing well, I assume if you're poking around here you've got your own battle going on. Stay in the fight and god bless you and your family.

I've had three prostate biopsy's. No shots, no drugs. It only took about 10 minutes. I felt a little pressure when the needles were fired in to the prostate, not what I would call pain. The first one, was a diagnosis of Cancer. I had a second one some time later which had the same results. We decided to monitor it and it has been 8 years since the diagnosis with only one change. The last biopsy's was without any findings so they did a MRI and that could not find it anymore. My PSA has been rising slowly but I've had no changes in the physical or medical findings that would normally be associated with cancer. I am still monitoring it every six months in the form of another PSA.
I didn't find it painful or nearly as melodramatic as what was just presented. The only unpleasant thing was that I did have bloody semen for a week or so and after that it was fine. I didn't find any of it to be painful or problematic and it wasn't done for some macho matter at all. So now like I said, it has been 8 years and the only thing that has changed is the fact that they can no longer find the tumor. I turned 75 yesterday and now it becomes doubtful if it would be wise to do anything if it flares up again. I wouldn't be risking that much time really.

Some pressure when the ultrasound is initially guided in, but other than that nothing I would categorize as painful. The "clicking" sound of the scope is the only indication to me that a tissue sample is being taken. Actual procedure over in 10-15 minutes. I have had two transrectal biopsies I had one a day ago and one 6 years ago. In both cases, I had the procedures at a surgical center and while under twilight anesthesia (same intravenous anesthesia as given during a colonoscopy). In both cases, I had no pain during or after the procedure. I did have a small amount of blood in the first few seconds of my first post-biopsy urination and a bit more blood on the first stool that I passed. For a few hours after the biopsy, I felt as though I had to poop--but only a mild sensation, at that. For about three days before the procedure, I had a soft diet, not too much fiber, which kept me regular and my stool firm. A few hours after my regular morning poop, I performed my enema procedure. I did so about two hours before arriving at the surgical center. I pooped easily 10 minutes after the enema--a normal and easily passed stool. The diet also meant that I did not need to strain to poop after the procedure, which you really want to avoid. I continue my antibiotics as prescribed and await the results. If all is well, in a few months, I will undergo a PAE procedure at the University of Miami Medical Facilities. My PSA has hovered around 4.25 for many years--with a low of 3.80 and a high of 5.20. All DREs have been negative. I have a large prostate of about 130 grams. The PAE is intended to improve urination and avoid having to use a catheter (which I have never needed) as the prostate continues to grow. I am nearly 65, very good health. Let's see what happens. Don't be afraid. I do recommend twilight anesthesia, though. Oh, by the way, I did have a Multi-parametric MRI of the prostate about 4-1/2 years ago. That, too, was negative. Good luck and improve your health while you are at it--you want to see your kids and grandkids grow up. Bye!

That's my picture. 65 yr old Man. It was 9 weeks (I was told "probably a week" to heal") ago I got my 10 sample locations. I was told by my Urologists "I am certain you have cancer" Maybe, but this did not show that. It was the most painful experience besides herniated disc pain wracking you w/spams I have experienced. I have had daily pain with hours of aching and spikes in pain like knives. Yesterday I finally called to seek help (VA Clinic) I was told I had to get referral from current Dr. for different Urologist. This man was unkind and unprofessional in my opinion and I did not want him to examine me as they don't ever admit mistakes. He said he was using topical anesthetic but I never felt that numbness or even close. I will have a journey ahead I think.

Have been diagnosed with BHP for many years 69 now. My big fear is infection, can't find anyone in my area that does the transperineal (N.W. Ohio). Had a CT scan a week ago to check for a kidney stone, it also showed an enlarged prostate. I really don't want to go down the medical rabbit hole, seems many have a tough time getting out one problem leads to another. Seems to me at the very least they would do an MRI first instead of blindly poking holes through my rectum into my prostate. How do I find a Dr. that offers the transperineal. Family jewels were taped up against my stomach, probe inserted in the rectum and 28 biopsies taken. Not pleasant, just uncomfortable, but not painful as I was injected with Novocain locally. Doctor, technician and nurse were present. Once completed, I was given the paper towels to clean all the lubricant and blood up. It obviously bleeds as a 12" needle shoved in your undercarriage 28 times is going to get a reaction. Went home on the bus and was sore for a few days. Be prepared for blood in your urine. Four out of 28 biopsies were cancerous. Gleason score of 7. PSA blood tests every 3 months, and "active surveillance" ( watch & wait).

I’m having one on October 16. Worst part is going to the urology office the day before and after to get high doses of powerful antibiotics. I’ve had multiple spine injections so the needle part isn’t a big deal. I had a biopsy back in 2018 but it was with a testicular surgical procedure I was having done. I have no fear of needles or injections. Sometimes it’s a bit uncomfortable positioning but the docs and their nurses have seen it all before so no need to panic or get embarrassed. The lumbar spine epidural injections are very similar to what the urologist does, so I’m
    Used to being poked..😊

It was one of the most painful experiences of my lifetime. I cannot believe that 12 gun shots with a small caliber weapon would have been less painful. Indeed, my only greater experience with pain was when I passed a stone some 30 yrs ago. I didn't expect nearly so much pain. I struggled walk after the process was completed. I am certain that I did not receive any form of medication to reduce pain prior to or during the procedure. There was an alarming amount of blood on table when they finished. I was never told about that. All in all a necessary but extremely unpleasant procedure.

I blame doctors for not considering pain when doing this procedure. One problem is that the doctor only knows what the patient tells them and many patients don't want to complain directly to the doctor about how painful any procedure (this one included) was for them. Patients have an obligation to medicine to make sure doctors know what their experience is like. Sometimes a patient has to demand attenton to be heard and that is not easy. It's the doctor's exam room, the doctor is very much at home and the patient feels like they've been kidnapped by aliens. Nevertheless, if only because the patient owes it to the next person going to have the procedure, a patient must explain exactly what they felt. I have a rule and will interupt a doctor mid sentence if they use the "D" or "T" word. Discomfort is when the drycleaner puts heavy startch instead of light. Tolerate is a difficult proces and cannot by definition be done "well". I will cock my head sideways if a doctor says something to the nature of, "it's a simple procedure that is generally well tolerated and we do our best to manage any discomfort." I then say, "what I heard was it doesn't hurt the doctor while the patient is in agony. Do you want to explain that again?" As patient who really believes I have an obligation to tell a doctor when I've had a bad experience, last year I had a very bad experience at the dermotoogist. I phoned twice, and then confirmed when they called to reconfirm my appointment that I'd have 10 mintues with the doctor and the entire staff to explain how they'd failed basic customer service at my last appointment. Of course, when I got there, nobody had any idea that I'd been promised the 10 minutes. I was offered the office manager instead and told her exactly what went wrong and why I'd not been treated with respect. After she heard me out, she asked if we were all good now. I said that we would be as soon as the doctor and everyone who would be treating me that day was in the room and I could repeat what I just said. Reluctantly, she got th doctor who at first attempted to blow me off. I seriously felt like I was doing them a favor by telling them of my bad experience. (side note it was the dermotologist and I was butt naked by the time the doctor got in the room which didn't make giving a presentation to him and three women easier). But, I did get my 10 minutes and when I was done, the doctor thanked me for what I'd said. He said that if patients don't speak up when something goes wrong, they have no way to know. I'd had my time and did not point out how hard it was to get his attention. All that to say, speak up if it hurts. Tell them to stop.

I had a prostate biopsy yesterday. At Penn Medicine here in Philadelphia. I have 3 lesions on my prostate. The whole procedure went well. Home about 3 hours later. A little blood in my urine, and my poop but that’s to be expected. Now I wait until next week to find out the results. Getting old sucks. Diet, exercise and good sleep are all important factors in helping your prostate. Not vitamins or supplements or anything else.

Glad things went smoothly for you Mark. I very much agree about the diet, exercise and sleep. But some studies suggest that supplements like lycopene "may decrease the growth of prostate cancer"- pubmed.ncbi.nlm.nih.gov/11489752/ They aren't conclusive but I am of the mind that if it is not going to hurt me, and it may help me, then here- take my dollars and I will give it a try. 😁

I definitely do not! I'll be honest, I'm not liking what i saw in a couple of videos. Imma get fucked up before I go in. To hell with it. I have chronic pain so I have pain meds and they don't offer something better that Xanax, I'm cancelling this shit. I loathe Xanax. Needles through my peroneum? Not without big valium. BUG SHIT! My urologist offered to do my procedure at an outpatient surgery clinic so they could put me completely under- if that was what it was going to take to get me to agree to the biopsy. Insurance may not cover the extra cost but it might be worth the expense. And if you discuss your concerns with the doctor maybe they can offer you something other than Xanax. Good luck with everything!

Seriously for me the worst parts was the insertion of the probe/ultrasound, just very uncomfortable. I had no issues, but everyone is different. A coworker had his about week after mine, he freaked out , panicked and passed out. We had the same Doc. I had blood in urine and semen for about a week or two, best thing you can do to clear it up is keep the pipes clean. Fortunately no cancer for me, my coworker was not so lucky. Currently doing the foot work for a third biopsy.

The latest method is a transperineal biopsy with a general anaesthetic lasting about 1 hour. You obviously don't feel anything and post the procedure there is just some general soreness in the area and blood in the urine for a few days. I had a fairly low PSA of 3.4 but there was a lesion on the MRI. On biopsy it is a Gleason 3+4 which needs treatment....this information could only be obtained via the 16 core biopsy (12 random cores and 4 targeted cores). I had an excellent surgeon and I didn't even need painkillers post the biopsy.

    • @dr.roberthandy1869
      @dr.roberthandy1869 ปีที่แล้ว +3

Transperineal biopsy is the method to consider after a MRI since it does not require needles to go through your rectal cavity.
Do not let a urologist stick needles into your prostate through the rectal cavity without first getting a MRI. You may not even need a needle biopsy. My urologist stated that a transrectal biopsy was not a "big deal" and I walked out of his office and found another doctor. I received an MRI that showed no cancer or disease but an enlarged prostate. And no side effects from this middle ages technique of a transrectal biopsy.
END TRANSRECTAL PROSTATE BIOPSY. Norway has recently (2023) has banned this procedure (transrectal) due to complications and death. Investigate your own heath and new advances in prostate treatment and cancer (disease) before submitting to any invasive procedure.

I’m scared as hell about the testosterone injection and the horrible side effects. Can you tell me how bad the testosterone injection is and how long is makes you have zero energy and when will I feel normal again?? My Gleason is 8,9,8 and I’m a high risk for surgery. I've got to say that my biopsy was nearly painless. They inserted the probe, not entirely unpleasant, then the actual biopsies, where the needle shoots through your rectum and into the prostate. Each of the dozen needle inserts were kind of painful but not any big deal. Nothing i would try to avoid if i needed it again. Afterwards i didn't have any bleeding or anything untoward. It was all done in very little time and not bad at all.
