Breakage of four rotary files followed by removal of four broken files

  • I intentionally broke four NiTi files in a plastic block with a 60-degree curve. Then I removed those four broken files one by one by using Hyflex CM files and an XP-endo Finisher in combination with ultrasonics

    This show us how easy is to remove separated file if you can see it through transparent plastic block

    When Yoshi Terauchi start retrieving, it doesn't matter how many separation take place.

    Too much of the outer part of the canal is thinned out due to the over enlargement of the rotary file. Check out the thickness of the block before and after the files where removed

      was about to write the same. the difference of remaining dentin between 1:06 and 8:57 is supremely significant.

      It is , but what are the options after breaking an instrument ? Another thing to keep in mind here are 4 of them and the block is plastic not dentin.

    The files are removed. As a dentist, in my opinion, unless you can restore it with a cemented post( which you have to do in the blind) you have lost too much root structure for predicatble long term crown. And with soy bean oil your chances of a good bond are non existent. Excellent technique but I dont know how successful the prosthetic will be.

    Do we rotate the hyflex clockwise or counterclockwise?

    Dr. Yoshi, can you give us information during the retrieval process what rotation you are doing? For exampled I am assuming it's counter clockwise or the opposite direction of the initial filing correct ?

    Am I the only dentist in the world who made the experience that stuck root canal instruments can be left in place without no negative results for the outcome? I even remember that at the University of Mainz they had an older booklet (in the sevienties) where they suggested breaking the file at the end of the procedure to get a tight solid filling for the canal.

      Da hast du vollkommen recht. Es gibt Studien darüber, dass abgebrochene Feilen kaum Einfluss auf die Prognose haben. Allerdings sollte vorher schon etwas aufbereitet und gespült worden sein. Bei so einem Feilenrettungsversuch kann auch viel schiefgehen (Perforation etc).

    Hi Dr. Youshi... I had the broken tool stuck in my root canal. I live an Georgia State, US. Looking for a contact who can remove the tool. The Dentist who was doing the root canal suggesting me to extract the tooth and have implant. Appreciate your help.
