      I use the links of chain theory for accidents. By cutting a chain link you can avoid the accident. In this case it all started with and ended with the pilot.

    As a passenger on some of these charter services, how in the hell are you supposed to know whether or not these carriers are up to par? Scary. People don’t write reviews unless they are angry.

      As a former GA charter pilot, you're climbing aboard the lowest bidder. All GA charter companies are dodgy, they have to be to be competitive and there is no quality assurance oversight beyond regulators. Easy to get a third party to take care of that and pilots get told to do it or someone else will. The only good charter companies are ones with long term resource industry or government contracts.

      @@goodshipkaraboudjan I don't know how the FAA treats that issue, but as for EASA operators, I can only state that your claim is false.
      I work for one of those GA operators, and I have been for a number of years now, both in a leading position on the ground as well as in the cockpit.
      Yes there are shady companies, but regulators, the civil aviation authority and internal safety and compliance measures make sure far more than 90% of us GA operators fly within specs and to a commonly accepted safety level.

    I had no idea Aaliyah were carried unconscious aboard the plane, and most likely were killed without ever waking. That is so messed up. :(

      The people she trusted most killed her. That includes the bodyguard who died soon after the crash. He was asking about her before he passed. I hope they told him the truth.

      I blame the A-Team. It was very clear that show promoted drugging people if they were nervous flyers. I can't remember how many times Mr T was drugged before being flown by Mudoch. So sad!

      @@Zzz2x I have a good CFI, but I really connect to how Hoover shares information.
      His explanations on things made elements of flight school a little bit easier to digest. Not to mention, when I first found the channel, I played his videos religiously to/from training, listening as I drove.
      I very quickly ran through them all and so I now wait for the next ones 😂😅

      ​​@@Zzz2xfact is Pilots do learn all these basic facts in their training and most of what Hoover does in most of his reviews is reveal how standard procedure was not followed. But you can never stop learning or get enough examples of what can go wrong when you don't follow procedures. We don't learn respect for all these procedures just through learning them as facts. It helps to hear stories of what can go wrong and put the fear in you to make darn sure you don't get sloppy.

    I recall the crash so many years ago and that the pilot seemed to have been bullied into flying an overweight aircraft, but I didn't realize there was so much more to the story, thanks.

      Her entourage bullied everyone from the pilot to her into flying that day. As much as the pilot is to blame for attempting this. He also argued the plane was overweight, but the entourage bullied him into flying. It was no "setting" that made them crash, they were horribly over weight.

      It’s not dangerous but fire and trauma are. She burned alive. One guy survived but died in Florida. If they dumped the fuel she would be alive but a paraplegic

    I've been in the music business for over 40 years. Much of the every day decision making, logistics, etc is insane. There is a sense of invincibility when artists (and their people) reach a certain level. This was a certified dumpster fire from the start. Hoover, you gave a great analysis of the incident. By the way, I've also been a pilot for over 40 years - always striving to maintain good judgement.

      I wish I could be a pilot. But I would never be responsible enough. I smoke weed too. It’s unfortunate I’ve heard cases of other pilots on this channel and elsewhere also being like me, or worse and taking drugs like cocaine.. idk how you can know that about yourself, and still decide to fly a huge ass plane being responsible for yourself, others, and possibly people you could crash into.

    I shared this video with my students. I did fly charter for about three months from South Florida to the Bahamas, including Marsh Harbour. This accident has resonated with me since it happened back in 2001. Such a tragedy, but at least we can learn from it. Thank you for sharing.

      2001? Damn! I was in a restaurant in Niagara Falls Canada when it happened, people in the restaurant started telling each other what happened, a buzz went through the whole place. It was a younger crowd and some young girls started crying. Man time goes so fast.

      And ed mccafrey Christian mccafreys dad snapped his leg on MNF the day before and was never the same. None of us were after these events. I know that sounds trivial. But I'll never forget two people I really cared about as a fan were fucked up badly.

    I remember this crash well. I was a pilot down in Ft Lauderdale back then. This incident spread throughout the pilot community. Even the rampers in Marsh Harbor questioned the pilot regarding the weight.
    Sad, very sad.

    I’ll never forget the day she died.. I cried at the mere circumstances surrounding the flight, for the initial survivors, for Aaliyah as it was known she hated flying, for her family, and for the thousands of people who mourned her death. It was such an avoidable accident.

    Oh my - this adds a whole other level of tragedy to this! Aaliyah had all the right instincts about this flight but was abysmally failed by those around her who were supposed to be looking out for her.

      Just imagine, on the news and in magazines, I NEVER recall hearing much about the pilot! It comes out years later. I heard Aaliyah did not want to film in the Bahamas and that the pilot was concerened about the weight on the plane. Nothing more.

    One of my instructors used this as an example of why you need to have he strength to walk away from a bad situation. I had not heard that Aaliyah had been drugged by her "friends" and carried aboard. That is truly wrong, but the Music Industry does not care about artists just the money.

    What a tragedy! Thanks for the detailed debrief Hoover. Beyond all of the other issues you covered, the fact that Aaliyah was placed on this flight against her will while unconscious was something that really stuck out to me.

      Agreed. And the worry that this dead "bodyguard " had for her post crash was just as tragic. He allowed her to be drugged into unconsciousness. And probably was the one to carry her on to the plane. He knew she didn't want to fly on that plane. His response to everyone there should have been, "That's it. No flight today!" But it wasn't. He failed at his primary job to protect her.

      Aaliyah failed at taking care of herself, but no one,is mentioning that. No one, and I mean no is giving me drugs without my consent and also knowing what the drug is that I might / she did take. Maybe not so adult after all.

    I did some CP work for a very famous musician. He was absolutely thorough in ensuring that any aircraft he flew on was top notch as was the pilot. If he ever got a bad ride he would never use that pilot again. He knew that many musicians met their death by ignoring aircraft safety.

    I remembered when this happened and have seen a lot of individuals discussing the tragedy. But I don’t recall ever hearing about the fact that she was drugged and carried onto the flight. Had to have been one of the two men. Bodyguard absolutely failed in his duties, even if he wasn’t the one to carry her aboard. The fact that she hadn’t willingly gotten on the plane makes this tragedy even worse.

      That was my first thought, too. Based on this version of the story, the bodyguard was negligent. I understand the pressure he was under from the others who were eager to get moving, but still... On the other hand, it's hard to believe that they were all willing to carry their employer, unconscious, onto an airplane she didn't want to get on. Perhaps, there's more to the story...

      @@Pretty_karma - well there is only 1 lady aside from Aaliyah, so the math is at least one or more of the people escorting Aliyah on the flight was a man, since it is literally IMPOSSIBLE for a woman to carry 120 lb-ish Aaliyah from the car, 10+ meters or more across the tarmac (the vehicle has to be parked away from the plane) up the steps of the aircraft and through the narrow doors... but i'm sure you already know this.

      @@augustdaye2746 This sounds really sketchy. This whole thing looks like a setup designed to blame the pilot. Speculation at the time is she ran afoul of someone powerful and/or satanic. The owner may have been paid off, told the pilot that he was flying the other plane with more capacity, and messed up the engine mixture to guarantee that plane couldn’t get airborne.. Of all the pilots mistakes, his worst mistake was he was bad at math and it got them all killed. He was probably skilled and sober enough but failing to balance the plane meant he didn’t have the nose authority to level the plane so after crossing V2 he was doomed. Plus the fact she was terrified to get on the plane shows she sensed they were all in danger.

    What I can’t get over is the way Aaliyah’s entourage DEFIED her will on this and carried her onto a plane she specifically refused to board. I HATE people who not only think they know what’s best for a grown mentally competent adult but who physically force them to do something they’ve decided is not safe or good for them. This is disgusting narcissistic behavior. She died because others thought they knew better. And they didn’t. She was so young.

      @@bobbymoss6160 Do your research. Aaliyah was unconscious after her entourage gave her a sedative, then carried her against her prior will, onto the plane in an unconscious state. According to the coroner and it matches FAA investigative reports, she likely did not regain consciousness before the plane crash. Her crew killed her by opting to place on her on the flight after she refused. She had choice - right up until the minute they chose for her and she could not argue against it.

    One of the first things they taught me in flight school, maybe THE first thing, was DON'T OVERLOAD THE TAIL! The nose won't drop in a stall like it's supposed to. The plane falls tail-down, passengers and cargo fall to the back, making things even worse, you go into a tailspin, and you can't pull out of it. Everybody dies. That's basic stuff.

      Even HGVs have to be loaded properly to be safe. When I used to work loading/unloading HGVs, the driver always supervised, and decided what went where, to keep the weight low and even.

    Thanks for this debrief. I’m old enough to remember this happening. It’s not even an example of the Swiss cheese model, it’s an example of surrounding yourself with dishonest people. The blame is 100% on Aaliyah’s management team, which cut corners rather than looking out for her safety. RIP Aaliyah.

      Aaliyah did not take care of herself! The “responsibility” (not blame, which is how a child deals with reality and how the world operates today) ultimately falls upon her. She hired all involved and if she gave them the power to make these decisions, the responsibility lies with her. If they did ignore her wishes, responsibility still falls on her……she kept these people on her team and I’m sure this was not the first time they ignored her wishes. Very sad scenario and need to include pilot who also allowed himself to be pushed around into attempting that flight.

      @@asya9493 As I recall, the pilot argued with them about all the heavy equipment they were bringing along with them. Maybe the pilot thought he was transporting 8 people, and one had to sit in the co pilots seat. Not ideal, but doable. Not making excuses for the pilot, but maybe he didn't anticipate (2) 300 pounders and 600 lbs of speakers and technical gear that I understand was included. ( Pretty sure the pilot was not trying to expire that day as well, and probably didn't expect what confronted him. He did as reported fight with the crew about being overweight before relenting.

    I will always remember being home and seeing the breaking news on MTV it was so sad. She was such a bright musical star at the time. This was just one tragic mistake after the other. This also happened right before the 9/11 tragedy. So her story was swept away for the bigger news at the time. Glad you are covering this story with just facts. Her light should never be dimmed.

    I remember this all too well because a close friend of mine, who used to be a singer (at the time of Aaliyah's death) was crying her eyes out when she found out.
    After learning about what happened, I was incredibly angry because her entourage demanded to fly on that day. Apparently some other pilots heard the pilot arguing with the entourage and had he not had alcohol and drugs in his system, he could have said "no". But, then the entourage would have filed a complained and the pilot would have been found out. That must have been on his mind. It doesn't make any sense why he'd agree to fly in a smaller aircraft knowing that the aircraft was too small.
    I've met a few 'A' listers and they're always down-to-Earth and polite. However, their friends and companions seem to think they're the best thing since sliced bread and act liked spoiled brats. Due to that, a beautiful singer was killed. It really angers me.

      Whoever did that to her is really a murderer. The pilot is 100% responsible for all deaths, Blackhawk's owner shares like 95% of that responsibility for all deaths. Whoever drugged and carried Aaliyah 100% effectively murdered her in the name of "we gotta be there tonight".

    What’s shocking to me as a manager of multiple chartered fixed wing machines is how many times these completely illegal operators get found out in tragedies and how few we hear about getting caught on the ground before they kill their pax - what’s amazing to me is how difficult it is to cut even a single corner but operators like this leave no corner uncut and get away with it until people die. The idea of squeezing a flight for a Metro into a 402 and thinking it will be ok is astounding to me. There is almost not a single thing as it should be in this flight, how do you just decide to switch from a 404 to a 402???😮

      If flew single pilot charters for 36 years with a very reputable company. In that time the feds would audit us relentlessly and watch over us with a magnifying glass so to speak! We had impeccable training and maintenance yet they chose to look the other way at other companies, some with very shady reputations. When asked, off the record why we were under such scrutiny the reply was always “ because you guys are the big guys and go by the book so if we make you do everything correctly the others will follow”. They knew that aside from some very minor audit findings their job was easy but if they dug into the shady companies they might have a difficult time ahead of them. So much for regulatory control. My guess is the owner of this charter company was well known to the FAA but was too small and too much paper work for the feds to bother with!

      He was qualified and he didn’t “cheat” , that was clearly explained in this TH-cam upload.. He was pressured into flying an overloaded plane which was correctly reported in the press at the time of the incident… He hadn’t gone over the check list that he was trained to do when he became qualified…Fraudulently falsifying his flight hours had nothing to do with the unfortunate out come of this incident..

    Thanks for this one! I was at my grandmas funeral when I heard of this happening that day. I had just bought her new album for my sister weeks before. And just a couple weeks later we had 9/11! Was such a memorable period of my life.

    Been with this channel since wheels up. It just keeps getting better and better. I mentioned in an initial post how well-versed and articulate you are-- and enjoyable to listen to... literally the sound and cadence of your voice.
    These slightly longer form explanations, imho, are your sweet spot. I'm not a pilot but many here no doubt are similarly situated as fascinated with flying [incident] lessons. Great Channel!

    I recall that in the initial days after this accident the story going around was that the pilot was pressured by Aaliyah and her team to fly the plane even when it was clearly past it's weight limitations. This initially put the most of the blame on Aaliyah and her team for "coercing" the pilot into flying in such a condition. Also I recall seeing some commentaries regarding what happened which again suggested that Aaliyah and her team should have listened to the pilot instead of being in a rush to get to where they where going. Now some decades after the crash -- the truth actually comes out that Aaliyah had nothing to do with the pressure being put on the pilot to fly. And in actuality Aaliyah had sense enough to refuse to fly -- but was forced to fly because she was drugged. In every way possible poor decisions made by everyone else aside from Aaliyah resulted in the tragedy. SMH.

People being treated poorly in the industry for the sake of money is nothing new.

      The pilot's job was to refuse to fly that much overweight. But he didn't refuse.

      @@MalcolmRuthven As Hoover mentioned, the guy was on drug, and he wasn't in the best position to refuse the flight, get a complaint and lose his job.
      Everyone can act high an mighty criticizing others and say "What I would have done". But when pressure is coming from all front, we are only human.
      I am not saying the Pilot is not at fault, but this scenario is "Everything that could go wrong went wrong".

    How many very talented artists have been killed in exactly this type of accident, with inexperienced pilots. Don't people who make the travel arrangements bother to check to see if crews are qualified? It's insane. Buddy Holly and Ritchie Valens, along with legendary radio host the Big Bopper, Patsy Cline, Jim Reeves, Otis Redding (1967), Jim Croce (1973), Lynyrd Skynyrd (1977), Stevie Ray Vaughan (1990).

      Jim Reeves crashed his own plane He did also kill Juan Manuel, a country music artist. And how about Kobi Bryant ? Was his pilot qualified to make your list? I think so, although he was highly experienced.

    I had no idea about any of this… thought it was just weight and that someone from her team argued to ignore it. I didn’t know she was carried on to the plane passed out, either. That’s even scarier; but at least she died in her sleep? She was so pretty and had such an amazing voice. I also didn’t know some people survived the crash originally, awful way to go…

    I remember when this happened, it didn't get a lot of news coverage beyond "Aaliyah was killed in a crash" I also remember the media blamed her saying she acted like a diva and insisted that the flight be overloaded when the Pilot told her that there were too many passengers and too much luggage for the flight to take place.

    thats horrible the lack of respect for her, give her a pill then put her on a flight she already said she didnt want to be on, a beautiful and extremely talented woman gone because of others choices not her own, makes this all the more tragic.

    Thank you for naming the other passengers. I was a 7th grader when this happened and I don't remember ever hearing names, nor did I think about it. I hope that, with time, these families have been able to heal.

    It’s so weird after seeing one of your videos for the first time the other day I was wondering if you had ever done a video about this. I feel like I manifested it lol. I remember watching the news break on MTV I was a Jr in high school. She was such a bright light and everyone felt like she was only getting stared. Such a devastating loss for everyone involved. I remember hearing years later about her being drugged and carried on the plane. The only peace I can take from that is she was a likely a sleep when it happened and hopefully didn’t feel any fear or pain.

      The TV series Mayday covered that. The airline was growing too fast and hired any pilot they could find. Captain Hans Lutz was a real idiot, once did a sight seeing trip over Italy until one of the passengers noticed they were in Austria. The first officer called for a go-around when landing, Lutz ignored him and flew the plane straight into the ground.
