Although I focus more on the Captain in this video, the First Officer is also responsible and had a duty to speak up and take control of the aircraft. Unfortunately perhaps the culture contributed to his unwillingness to do so. Also, note: I had to remove every image (and video footage) of the crash and wreckage in order to comply with TH-cam's monetization policy. Thankfully the lessons we can learn from this are still just as valid.
Culture could certainly be a factor but it’s not possible to really generalize. Interesting observation though that one’s culture may not be well adapted for dealing with every circumstance, and that is a lesson we should all probably keep in mind lest we become complacent about how our own cultural attitudes and tendencies can cause problems in certain situations.

1.Landing gear is lowered without voicing the action 2. FO retracts the landing gear before landing again without saying anything 3. They belly land on the runway 4. Captain takes over 5. They do not discuss the belly landing with each other 6. They receive a call from tower warning them about the still retracted gear 7. Upon hearing that, the FO immediately realised, they belly landed because he forgot to voice he had retracted the landing gear to prepare for go-around. 8. not wanting to commit the same mistake he lowers the landing gear too early, while both engines are down and they're barely being kept in the air by the tail engine.... meanwhile CPT pushing down, while FO pulling up cancels each other's input
@mtm4a sorry, my mistake. What I was visualizing was the pilots probably manipulating horizontal stabilizer and elevators to adjust the aircraft’s pitch and extend the glide at the end. The APU at the tail only provides power to the instruments, compressed air to attempt restarting the engines and not enough trust to keep the airplane in the air.

They were also fasting. Hunger and dehydration from Ramadan made them extra incompetent. PIA banned their pilots from fasting if they're working after this avoidable accident.
Amazing video!! Another factor to this crash which was not mentioned in the video may have been the fact that the pilot and co-pilot were observing Ramadan by fasting while flying the plane. While fasting/hypoglycemia may not have caused the crash, it may definitely have contributed to fatigue and pilot error leading to some very bad decisions being made. After the crash PIA apparently put out a circular stating that flying pilots were no longer allowed to fast while flying.
This was a morning flight.. and these bastards probably had gulped tons of food when they break their fast early morning ..just a few hours back to this incident.. can it still cause what you're saying might've caused them to fuck it all up.

Hoover, love the direct, concise information you provide. Brings back bad memories of PIA flight 268 that crashed with no survivors on approach to Kathmandu in September 1992. I was flying with two work mates Jim and John on that flight from Heathrow. 20 minutes before boarding I heard my name over the PA. It was the office cancelling my flight due to some work issue. The two lads and 165 others left on that flight... The very next day I flew with PIA to Kathmandu to help sort out the aftermath. To say I was apprehensive is probably an understatement...! I remember seeing the wreckage from above. Often wonder why the pilots of 268 made such a mistake. Perhaps your insights explain why. Keep up the great work. All the best.
as Pakistan's average IQ is 81 and this pilot was classified below average by the airline... he had a fake license that no on ever scrutinised closely. 'may face significant learning challenges and may require specialized educational services' is how we would describe him.
You had me at not doing a check list and the continuous unrelated cockpit chatter. And just when you thought it couldn't get worse- it does. Thank you Hoover, great debrief as usual.
"below average intelligence" should be an automatic no-go to become an airline pilot. "Hello, this is your Captain speaking. I don't understand all these instruments and procedures, but I'll do my best to bring you to our destination which I already forgot."
I remember stories my dad told me as a kid about the state of PIA aircraft, which used to land at Manchester Airport in the UK. My dad used to drive the catering trucks to unload food, drinks, and carts from the inbound flight and load fresh food, drinks etc for the outbound flight, in the mid 90s to early 2000s. My dad would routinely find insects on food and in food carts, which would require fumigation. And strong smells of urine and faeces that awaited him as the cabin door opened! Then came the partial ban of PIA flying in EU airspace due to safety and maintenance standards, followed by issues with the authenticity of pilots licenses (as mentioned in the video). And now, causing the death of passengers due to bad flight management, poor communication, stubbornness, or of the above? Personally, I'd rather walk than risk my health and life on a PIA flight.

As a student, I’ve learned to focus and think about what I’m doing. Verbalizing each step and confirming what I’m doing and what the planes doing. I can’t imagine flying a big plane without focusing, knowing you’re simply can’t screw up.
Do you really enjoy flying? It doesn’t sound like it. Verbalising everything you do does nothing to prevent cock-ups. Nor will the average checklist. Want really counts is putting your brain well ahead of where the aircraft will be pointed. So instead of using brain power to speak, use it to think about where you want your aircraft to be well ahead of your arrival and constantly consider if you really want to be there. I’ll not go into checklists.
Verbalizing each step means you're eventually going to take longer than needed once you're proficient. It's great to start training with, get the routine down to an artform.. but you can have discipline and safety without re-enacting a Thunderbirds take-off EVERY single time :P

Have mentioned this on other aviation channels, but I met a former 757 pilot for AA who was also a former Navy pilot. One Thanksgiving he told us some amazing stories, but also an admonition: DO NOT FLY on commercial Carriers based South of the US Border, or from 3rd World Countries. Primary Reasons: Inadequate funding for parts, maintenance and pilot training. He gave specific examples. For instance, entering a Mexicana DC-10 to meet up with a colleague pilot. He sat in right seat and reviewed the Log Book while waiting for his friend to exit the Lav. He said he saw that the DC-10 was flying with several critical systems, including hydraulics, that were non-op and running on stand-by source. Several instruments were also down on one side. He said that pilots from some countries look at every successful flight as a case of ‘cheating death’. If you knew your flight crews thought like that, would you step foot in their aircraft?

So then he appealed to the Pakistani Ombudsman equivalent and they then eventually cleared him to start airline pilot training. Naturally he had his problems during training and his career but somehow managed to make it to captain and keep his job despite the glaring flaws. I honestly don’t know how he managed it. I’d guess maybe he had wealth or family connections in combination with the fact that generally Airbus airliners are heavily automated with a lot of computer assistance that’s the friend of any low skill or inexperienced pilot. 3. They belly land on the runway
4. Captain takes over
5. They do not discuss the belly landing with each other
6. They receive a call from tower warning them about the still retracted gear
7. Upon hearing that, the FO immediately realised, they belly landed because he forgot to voice he had retracted the landing gear to prepare for go-around.

Keep that focus and never stop learning.
Always use the checklists.

My ex was a career Naval Intelligence officer. His rule was to fly on American and British flag carriers ONLY.

this one falls into the category "You can't make this stuff up." Great analysis, Hoover. Love your videos.
Great, alongside Grreen dot aviation, Mentour Pilot
"He likes bossing others around and having his way!"
"Oh dear. Is he at least clever when he has his way?"
"Haha! No."
sir, your content is both fascinating and shocking, extremely well presented - I can’t stop watching. pilots in training and experienced alike should watch your entire library of debriefs, I reckon
So many opportunities to take a step back, recover and reassess. Shocking how many bad decisions were made by the captain given his experience.
He wasn’t a captain, he was a capn, as in Cap’n Crunch
So much beta energy.
A great amount of his experience was doing whatever he felt like doing and getting away with it.
I'm not a pilot, but I applaud all the pilots out there for not letting gravity get them down.
HA
Survivors only received $3k, victims $6k. That is truly insane.
Pakistan is a poor country. 3 000 USD might be a lot of money there for the poorest folks. Not of course for the super rich

@@RetroscoopThat’s about a year of minimum wage in Pakistan. Still absurdly low for how much money the airline is probably making when they’re 100% at fault.
As far as what happened, I have no words. As far as “Hoover”, GREAT job as always. Love your format, and keep up the good work!
What?!? Up to 34% of PIA pilots had fake pilots license and/or paid OTHERS to take their exams? WTH? Their own butts are on the aircraft! I now know what the assessment of captain's "below average intelligence" refers to, if he was one of those with questionable credentials.
Believe it or not that means he was one of the "GOOD" ones. He took the tests and passed and despite being below average intelligence-wise. The ones who didn't probably couldn't...
a lot of people are in their fields just for the social status
For your info there was a complete inquiry and all was found in order. It was an attempt by the Air Force assholes to get civilian pilots fired so they could get in.At that time there were twelve a/f basttards deputed in Pia during Imran Khans Govt. The aviation minister had a fake welders degree ! A messed up list with wrong service numbers mixed up timings between UTC and PST and mixed up names was given by CEO PIA to another air force Air Commodore working in CAA Pak (whos since died due to artery rupture) who passed it onto the so called "aviation minister" (a drunkard and fake degree holder himself) who like an idiot blurted it out in the national assembly........
reminds me of a friend who told me she had a passenger in the car from china, who asked on a trip if she could have a turn at driving...Jo asked her if she had a license "yes" the woman replied , "I bought one" turns out she could not even drive, had never driven, but had bought a license which was valid as a tourist in NZ ...
@@graantmnz I'd love a theory as to why that is. Is it magical thinking, where you think a piece of paper imparts the necessary competence simply by its possession, like learning pills in sci-fi. Or is it that when you're from somewhere where you need permission to so much as take a -, and licenses are a bureaucratic obstacle to be overcome, that you come to believe any and all licenses are that way. Or is it something else entirely (and is the stereotype real, for that matter.)
Would that there was real anthropology that studied these things instead of the ideologized nonsense field that tries to prove that apes speak and stuff.
The intro about the captain is the nicest way I have ever heard anyone describe someone when they actually mean "stupid a**hole"
Insha-Allah ?
Any approach that’s reliant on your deity should sound alarm bells in your mind😨😢
Hard to say if it's unusual for them to say in the cockpit. It's certainly not unusual to hear that from a Muslim because we don't know the future; you can say see you tomorrow and it will be followed with Insha-Allah.
Funny how being present in fact based reality with adherence to scientific protocol would have landed them perfectly, removing gods will all together, just sayin
Just to be clear; not a dig at any religion. Just I wished that first officer would’ve been woken up by the need to say that phrase!!
@@E4RLIES It's most likely just a common phrase, like "God willing" in English. People often use "Si Dios quiere..." ("God willing") or "Dios mediante" (more or less the same) in Spanish for even trivial everyday things ("I'll be there around 5:00"), and it doesn't even mean they believe in God. According to Wikipedia, the Quran requires using this expression when discussing future events... so for someone religious or living in a religious environment is probably automatic when talking in the future tense.

Mentour pilot did this too. When the Captain first applied to PIA, he failed due to his mental state. He then got other psychiatrists to say he was sound and he was then taken on. He also became a check pilot. This tells me he ‘knew people’. Those people have those deaths on their shoulders but they won’t care because ‘inshalla’.

I bet the captain was one of the PIA pilots with bogus credentials. Disregarding ATC commands (in an effort to save him from disaster) and then bragging how amazed ATC will be with his piloting skills is insane arrogance. It’s so sad for the surviving family members of the 90+ passengers that their loved ones died in such a preventable tragedy. R.I.P.
Mentour’s video about this incident explicitly says these two pilots were not among the ones with fake credentials.
@@tookitogoyou know, that makes it even worse.
@@sarahalbers5555Indeed. The first psychological assessment deemed him unfit to be an airline pilot. His reaction was to try and get a second opinion-of five “second opinions.” He managed to get a “yes” from some of them but the rules changed to disallow second opinions.
I honestly wonder how he would have fared if he’d tried to fly Boeing planes given they have less automation.
But these are only guesses. I have no idea how he managed to last so long and get promoted given his deficiencies.
This is the best channel on TH-cam. I don't understand why pilots rely on the auto-pilot so much. Harder to get distracted when you are hand flying.
Just looking how PiA is painted on the Plane should tell you not to fly with it.
It only looks bad because that's an AI upscaled photo. But still there is no way i would fly on PIA flight.

You had me on the edge of my seat the entire time.
I'm at 12:33 and I'm a little confused as to the level of authority an air traffic controller has. My question is then is this, if the atc is instructing the pilots to fly around and join a holding pattern, is this an actual instruction to do so or is it purely advisory? If someone could answer this it would help me understand a little more of the blind panic that atc operator would no doubt be going through.
Classic case from a state-run firm (Pakistan International Airlines) stuffed full of cronies and incompetents. Very frightening end for the passengers.
