A very good video. Just remember Scouting doesn't end when your child ages out. I have 36 years as a registered leader and now serve as my troop's webmaster. This way I can be with the best and brightest in my community. I just heard of the Scouting Heritage badge where I can share my experiences of this wonderful program with the next generation. So, not only will your children benefit from this experience, but if you do it right you can benefit, too. I have a scout mug that says, "100 years from now it will not matter what job I had, the car I drove, or the balance in my bank account. What will matter is I made a difference in the life of a boy." In 2017, my husband died and one of his scouts called me and said, "I would not be half the man I am today if it weren't for your husband." I was in tears and so grateful for my husband's legacy.
A quick google search for "100 point cub scout form" returned the results you seek (a downloadable copy of the form). I chose the one from his Council (Sioux Council), but I see that SEVERAL other councils have this same form posted, so clearly the idea has "legs."
Talk to your District Executive, or go to your local council office. They will likely have copies there. Or a quick google search will provide several examples... ;)
