Those are pieces of the frame that stick out from the smokebox, they normally have lifting points in them that help lift the locomotive during maintenance/construction.
Most steam locomotives have them just less prominently and in different locations.

This is genuinely one of the coolest Truro customs I've ever seen, and the build reel was very interesting to watch as well!
I'm very impressed with the creative methods you used while putting the pieces together. The scratch-built cab looks absolutely spotless, and some ideas you came up with (even the ones you ended up not using) were very helpful and creative too.
As someone who uses Milliput, I highly recommend getting a pack. With what others are saying about it, you still have to treat it very gently after it cured!
As for the lining, if it doesn't look good for you, I highly recommend using masking tape and brushes, so you can have them as thin as possible. You can freestyle on the curved parts, it just requires alot of corrections depending on how steady you're hand is.

When I heard you used crayola model magic clay, the first thing I thought of Chris Chan, so that was a little too perfect. Aside from that, this custom is stunning without a doubt. Definitely in the top 10 best Thomas customs I've seen. Hopefully you'll do more videos like this in the future.

We should have a TH-camr that can make custom Wooden Railway Thomas characters (including OCs and characters that were introduced if T&F but never had their Wooden Railway models made since Wooden Railway's discontinuation in 2017 until its revival in 2021) without having to repaint and salvage other Wooden Railway Thomas models and make them look like that they were made in a factory. BTW, I like your custom Wooden Railway City of Truro.

It’s a great custom, but one thing Is that city of truro wasn’t known for having no dome, that’s just a Thomas thing.
