Those engine's , 3.0 V6 ( OM642) are in the Mercedes sprinter ambulance's. They do up to 500.000 miles with no issues apart from servicing. They can do over 1 million miles. The rule is regular service and look after it. Saying that, in the ambulance service they were hammered and still performed, never let us down.
50k miles, that's a good time to do the first PCV valve change. It's cheap, and it's the main killer of this engine. After 40-50k it starts leaking oil into the intake and creates all kind of deposits and issues. There's a reason sprinters reach 600-700k miles with this engine, and passenger cars don't. People are just cheaping out or postponing servicing while a company doesn't.
