    Your my fav , I am a RN MSN and I have a lump on my thoracic spine and I am seeing my PCP tomorrow, , it showed up suddenly and I have a lot of night pain..its killing me,, it's near my T12,, could be anything , I didn't even notice the lump. I just had increased pain from my arthritis and my rheumy put me on prednisone which did not help,, that's unusual..i was scratching my back a few weeks ago and felt the lump..i pressed on it and it hurt..i am 58 years old..with psoriatic arthritis..the night pain is the worst..probably because I am laying on it,, haven't slept well for a month,, it's probably a cyst or lipoma..i look forward to a MRI..and I have a MRI reader...

    1:39 I find with MRI the grayscales can mean many different things. I’m pretty sure the bright spots don’t mean you have more active nerves there than in the darker spots along the cord, if you impede the spinal cord anywhere the same problem should show from that point down, it’s one cord after all. Also dr isn’t saying what type of MRI image he’s using here.
    Even without imaging tech used, it could also simply be that the cord doesn’t form a perfect line with the saggital cut, so where it goes dark it simply is running a bit distal to it.
    Also interesting:

    Can I work with an Anular Tear?

      Depends on "kind of work", your pain and disability.

      It's not really appropriate to "comment on" or "recommend a" work status without a complete history and physical exam.

    After annular tear surgery how many years person can live normal life without tear pain??

      No pain - 40%. At least 50% improvement - 40%

    Hello,tnx for the info. I just got my MRI and it says that I have a 3 mm l5-s1 circumstantial disc bulge with mild lower neural foraminal narrowing, central canal is normal. I am having pretty severe discomfort on my central lower back when sitting down or driving. Can anybody help to explain my situation?any help would be appreciated, since I have stopped working because of it. Thank you.

    Is it normal for a lumbar MRI to jump over the vertebral axial views and just show the disc slices?

    Hey Doc, how can you tell in the images the difference between a protrusion and an extrusion with an annular tear? I know sequestration is pretty obvious in the sagittal view but the others aren't so obvious. I'm worried mine is just a buldge/protrusion without a tear. I know that sounds better but they don't usually heal because the body doesn't recognize anything being wrong so it doesn't send the macrophages right?

      You have a lot of questions there … yes you can tell the difference between extrusion and protrusion all disc herniations have to have an annular tear.
      Remember, imaging studies image, they don’t tell you where you hurt.

      @@DrTonyMork I thought protrusions don't make it through the entire annulus? Just a portion of it. That's what all the publications show.

      @@DrTonyMork And apparently they have a much lower chance to heal because the nucleaus pulposas doesn't make it through to the canal so the body's response system doesn't see it.

    Dear Dr Tony how can I send you MY M.R.I SCANS DISC FOR YOU TO READ FOR ME ... I Have HAD 3 spinal ops in 6MONTHS ... IS IT LIKELY TO HAPPEN AGAIN.. WHY Didn't THEY FUSE MY SPINE?
