Also just subbed for more NF! All of NF's albums tie together in a progression to tell his story,. He uses a lot of symbolism in the videos (which he co-directs) , and a lot of callback references in his lyrics. There is a whole NF Journey/Cinematic Universe list - I've been putting together mine since 2016 - but for now just to understand this song you want to at the very least listen to/watch these:
Mansion (title track from the first album)

Intro III (first track on the third album, which is the same album that this song is on)
He's very intentional with everything he does, and in addition to co-directing the videos he also co-produces the music with his best friend Tommee Profitt. As he says, "I've got my hands in everything"

Nate had his first album in 2010 called Moments. It's okay, but not really part of the journey. It was his Mansion album where all the symbolism and personification of emotions starts. Hi there - great reaction! I see what you did there in your outro with the "black smile"... nice! ;-) As for the origins of the "fake smile"... it first shows up in the track/video for the song "Therapy Session" (off of his 2nd studio album, "Therapy Session"). So many of his songs have lyrics and symbolism that intertwine, that if you are really digging his music and want to get the whole NF experience, I'd recommend you take the "NF Journey", which walks you through the key tracks/videos from the beginning. (As an example, in the song "Outcast" you just listened to, he has a line, "...and put the 'caution tape' around me like I did in 'Intro 1', yeah, they got me reminiscing now..." -- because in the video for "Intro" off of his "Mansion" album, he has 'caution tape' wrapped around himself.)
Also, you commented on the word "REAL". NF's brand/logo is "NF Real Music". He has a tattoo on his right arm that says "REAL" -- which is why he says (again, in the lyrics to "Outcast") "...and I stare at the locks and the tat on my skin..."
If you want to dive deep into NF's music, I'd recommend you start the "NF Journey", which begins as follows: From the "Mansion" album (2015): 1. Intro (music video), 2. Mansion (lyric video), 3. Do clouds mix tape first cause that will cement you as a fan. You will love NF then you’ll wanna go on the journey. Do the whole mixtape. It has less of the symbolism in it. It’s hard to understand the hidden meanings cause a lot of them are references to things he’s spoken to in his other albums. Dude is a genius, I think he entered mansions with the next three albums basically planned out. No he revealed the fake smile in therapy session the song. He’s referencing Breaking Bad btw.

So NF has OCD which if you don't know what it's like to have OCD, it can feel like the voices in your head aren't your own because of their taboo and compulsive nature. There's a condition called OCPD which is a personality disorder and it's very dangerous because someone with OCPD doesn't distinguish between the compulsive thoughts and their own. So very often when NF references being trapped in his mind and alter egos, what he's referring to is these thoughts and the feeling of not being in control of yourself with OCD. As if someone else is controlling you. (I have OCD which is how I'm able to explain it in such depth)
