The only things I can think of are that the audio output device for VC is not set to "system default" in Windows, or that you changed some settings for the game in Nvidia control panel which causes strange effects. Leave everything there as default except you should enable ultra low latency mode.
There's a one frame window when exiting a sidemission vehicle where you can press the key to start the sidemission, have a call come in at the same time, and the automatic cancel of the side mission (from leaving the vehicle etc.) allows you to start missions again with the call available. Then once the call is taken, Jury Fury can be started again.
It's a side effect of warping out of the mall. Since I never properly "left the mall" through a door, and the check to give you 5* for entering the mall without the cop outfit seems to check that, you instantly get the wanted level

XMB buttons are mouse side buttons, 8426 are the numpad arrow keys. M is the middle mouse button and R is the right mouse button
