But often they help out with various things even with a hospital birth. Many coach their clients to lie to hospitals claiming poverty (even though many have the means to pay) in order to get discounted medical services. I'm sure he's aware of how this all works, but he's just trying to simplify it for the understanding of the woman, through a translator. The other thing that commonly believed in Chinese culture is that the birth mother must rest for at least a month and be fed a special diet to supposedly aid in recuperation. Part of the fee they're paying is for this sort of service. And there are permanent residents and even US citizens who have tried to hire people who supposedly specialized in this sort of thing - taking care of mothers for that month.
