A category B SPAD occurs where a train passes a signal at danger with authority because a stop aspect or indication was not displayed with sufficient time for the Driver to stop safely at the signal due to:
I. failure of signalling or level crossing equipment, or
II. It being returned to danger in error
In this programme we look at some typical examples of where Category B SPAD incidents have occured, We also look at some of the consequences of these incidents that might not be immediately apparent to you.
Understanding the causes and consequences of signal reversions and category B SPADs will assist signallers, signalling technicians, track maintenance staff and other key railway personel to appreciate the actions that can be taken to avoid inadvertantly contributing towards these incidents.

Very much reminds me of a crash at Clifton Hill in Melbourne which was caused by a trainee panicking and pulling the points under an up train. The front bogie went toward the platform but the rear bogie and the rest of the train were switched to the NS road. The leading motor car was on an angle that resulted in it hitting the end of the platform.

1:02 they should put the green light below the first yellow so it resembles a typical BR 4 aspect signal, that would be such a neat little reference...
Wasnt the 2nd example with the signal box, a Category C Spad?
No, a category C SPAD (now they like to call it a 'signaller SPAR') was when the signal was replaced to danger by the signaller in accordance with the rules, e.g. reported bridge strike, signal back to danger in the driver's face, driver doesn't have time to stop = cat C SPAD (signaller SPAR). A signal replaced to danger in error was always category B (now a 'technical SPAR')
