went to the emergency room with chest pains,ecg came back clear,wasnt happy with diagnosis when i left the hospital,the nurse placed 2 of the pads by my groin,3 in a straight line under my left chest area,chest pains haven't gone,been a week since i been to the hospital..

When you counted the 4th intercostal space, you counted the rib at the manubriosternal joint as the first rib, when the rib at this joint should be the second. I think you're just missing rib 1 in the diagram.
4th intercostal space is correct. The space between rib 1 and the sternum is not an intercostal space and is referred to as the interscalene space. It is NOT counted for purposes of ECG lead placement. Hope this helps anyone reading :)

I have always found it extremely difficult to feel intercostal spaces. Especially if there is more tissue/fat. Its just one of those types of skills.

Sorry but in the diagram you are referring to 5th space as 4th cux second rib is at sternum and after that is 2nd intercostal space
