Thank you so much for this ! I’ve been an assistant for 6 months now, but I’m just learning everything about perfecting my temps. This helped a lot thanks again
Carla your so HUMBLE and very polite ,even in your other video when explaining the composite filling talking about having that open communication with your DR when not knowing or wanting to make sure whats next !!! SOOOO HELPFUL :D
Omg going to rewatch this video at a later time. Honestly , thank you to her for making this video because I always wanted tips and tricks in how to make a temp crown!!
I keep a colored pencil in my room to mark the margins of the temporary with before I trim it. Lightly rubbing the pencil on the temp really shows the margins well. Then I just trim until the color is gone and have defined margins.
I hold small anterior crn with hemostadt when trimming. Also...when looting out temp material, I put some on my glove as body temp can affect setting time.
You can also just very lightly line the prelim with Vaseline (instead of placing topical or Vaseline directly on the tooth), which can be removed from the temp before cementing, with alcohol.
Hey ladies, I have a few questions. 1. How do you make an anterior temp crown if the patient has a gap? Is it better to use a triple tray to make an anterior preliminary or is the one shown in the video best ? I tried making a crown for a patient and when I removed the crown they were splinted. I tried to adjust the temp so that the material was not so noticeable but then the temps broke. 2. How would you make a crown out of scratch if you have no preliminary? I know some places have some stainless steel crowns but what if those aren’t provided either ? 3. If the Dr is doing a B/U and peeping the tooth but you didn’t get the chance to take a preliminary would you still recommend building up the tooth ? Would the build up you do bond to the build up the DR did to the tooth?
When you have a fractured tooth you said add flowable in the beginning. AM I adding it to the real tooth and will it come off? Because if I am taking an impression will it stay on the tooth or come off the tooth when I take the impression?
For a case like this we recommend adding something like liquid dam (from Zoom) on the tooth to build it up so that you can take your pre-op impression. You can use flowable because it won’t really bond to the tooth without etch and bond, so it’s fairly easy to remove BUT it will be a waste of product. Other than liquid dam you can also use wax (not ideal cause it will distort easily) or any “expired” composite/flowable you may have in the office…And yes you will add it to the tooth; it will be easy to remove from the tooth 👍🏼
I noticed you’re using Structur 2 temp material. I’m trying to get used to this material right now. I’m used to Perfecttemp/Luxatemp type material. Any tips specific to this material? Currently I’m using blue bite for my stent, I use vasoline on the prep and contacts and try not to overfill but so frequently I end up breaking a piece off as I’m removing it from my docs usually thin, tall, straight, conservative preps. It never comes out in the impression with Structur where it always does with Luxatemp. There has to be some technique I’m missing here that would help me get through the 6 cartridges I have left! 🙄🔫
mindymm2206 Mindy Morgan I used perfecttemp before it’s pretty similar. Honestly I don’t think the material is the issue. Maybe you’re getting it locked on undercuts and when u pull out it breaks. I would say have your dr double check the prep and clearance. Try using topical on the prep and contacts of adjacent teeth instead of Vaseline. Also, make sure you’re not leaving in the mouth for too long
No, vinyl polysiloxane is used for final impressions. For pre-op impression we usually use Alginot or a generic version and for the temporaries material we use a few different ones like Protemp or Structr.
Hi Gabby! I was wondering who does the models? Another thing - how long it usually takes for you to prepare temp crown? Is patient on the dental chair at the time of preparation? Who place temporary crown in the patient mouth - assistant or dentist? What if crown still need a little bit adjustment?
Karolina Rogowska great questions! So as I mention we were just using a model because we didn’t have a patient at the time to film. The impression is taken from the patients mouth prior to prep. If it’s a single tooth it can be from 5-8 min if I had to take a guess lol I never time myself. Patient is in the chair after the tooth has been prepped. The assistant should be able to fabricate the temporary crown and cement it. After cementing it, if I noticed still needs a little adj I will do so with a slow speed polisher. If that doesn’t do it then I’ll grab the dentist to adjust it with the high speed. I hope I answered all your questions 🦷🙂
@@DentalVibes thank you so much :) Qualified debtal nurse in Scotland can take impressions ( she needs extra impression course to do so) and in theory she could do a temp crown but its all done by the dentist. What we CAN NOT do for sure - cement crowns in the patient mouth and use any handpieces in the patient mouth . I was wondering what is the dentist doing when you are preparing the crown? Does the dentist check on patient when it's cemented?
Karolina Rogowska so in the US what a dental assistant can legally do varies by state. In Florida, expanded functions dental assistants (which you go to a program for that certificate) can do everything that’s temporary. So we cement temporaries with TEMPORARY cement. We can use the slow speed only in the patient’s mouth. The high value of an assistant that can do these duties is that the dr can multitask. While the assistant is making the temp the dr can go do an exam or see another patient. Every dentist is different when it comes to checking after it’s done. If you worked for a dr for a while and they know they can trust you will do a great job. Then they won’t come back to check it
Karolina Rogowska 2 weeks maximum! They come back the assistant removes the temps (it comes off very easily) tries the crown in and the dr comes in to check/adjust/cement
One more question, you said never remove the sides, but a lot of times I have excess material what should I do b in this case? Meaning the Distal or Mesial?
You should remove the excess on the Side of the contact not right where the contact is. In other words think of what a permanent crown would look like. It would be smooth everywhere, so you smooth your temp 🙂
Do you find issues ever using alginate? I have never used it to make a temp. I worry it may dry out by time we make the temps. Plus we keep the temp impressions until seat appt just incase we need to make new temp due to broken or lost temp. I have used the bite or putty material so it keeps longer.
There is usually a margin line you can see, don’t cut back past that and barely polish the interproximals. Try it in and slowly adjusted as needed for your contacts.
There should be a thing (extreme dental term) in the high vac suction that is there to filter or catch bigger items. This is not a screen more of a wire and it is where the metal is attached to the hose. This should ALWAYS be there anyway to catch other larger items that could block up the suction.
That’s because on a real patient there are more obstacles you have to learn how to trouble shoot. Usually there’s always a lot of contouring to make it look perfect.
@ Tracy Blackburn: Seriously you’re getting free technique instruction here, and you’re bagging at her mask. Sigh. Somebody gave me a really great tip of my masks last month. I keep a colored pencil in my room to mark the margins of the temporary with before I trim it. Lightly rubbing the pencil on the temp really shows the margins well. Then I just trim until the color is gone and have defined margins.
That’s a great way to make sure you’re following the margin 🙂 that would be helpful for someone struggling
I think I’m going to do that , I’m having a hard time trimming the temporary crowns, I either trim so much that I have to keep adding Flowable
Do you guys have a video on how to pack cord? Especially on patients with healthy gum tissue. I struggle with it.
