Very good question. I think the answer is simply that context helps A LOT. Let's look at this example: "We'd talked a lot before we met in person." After "would" we can't have a past tense verb, so "would" doesn't make sense here. The sentence is "we had talked a lot before we met in person" and the latter part of the sentence helps us know that the 'd is "had" because it makes sense that we are describing two past actions, one which happened before another, which is the Past Perfect tense in English (had+past participle) and another example: "He'd go on vacation, but he doesn't have enough money." Again, "would" requires the infinitive verb without "to" so our word here "go" makes sense, and this can't be past perfect because we aren't talking about two past actions, rather we are using our conditional sentence "what would someone do if the situation were different." So, our sentence here is "he would go on vacation, but he doesn't have enough money." It's something that takes patience and experience and learn and become better at and to make it second nature for you. Trust me, with more time and practice you will understand and feel it just like natives do. When studying, just asking yourself about what your options are and what each option means. Are we using a conditional? Are we speaking about past actions? What does it mean if I use "would" instead of "had?" And just make sure you go slow and really look at the sentence structure. Over time, you'll get faster and faster and these things will become automatic. I wasn't born with all the answers, I was just a baby and listened a lot and naturally gained the feeling for the difference, so you will too, of course! You just need more time. I hope this helps :)

True and English is changing a lot all the time, like now even the president speaks using contractions. So, I wouldn't be surprised if someday soon contractions become fully accepted formally too

If you're asking about finding vocabulary, there are great lists of vocab by topics or by levels all over Google, you can just search for them, find ones you like, then try to just study them, use the good ones you need or like with your friends, keep practicing, read, write, say, and listen to those words each day and eventually they'll be ingrained into your head :)

But it's good to get uncomfortable because you'll grow from that experience and be better and better, you need to always push yourself out of your comfort zone. Just keep moving forward and reviewing words, practicing every day and you'll see the results ✨
Maybe you are thinking of a British accent? With my American accent, we pronounce the R very clearly and hard, some British accents may kind of remove the R sound, but I’m not an expert on British accent 😄
