  • Cataract surgery and the preparation for it require many decisions and can be overwhelming. Having options is great, but it can make this a confusing and difficult time! In this video, I'll break down the available options for intraocular lens implants (IOLs) that you may have as you prep for cataract surgery.

    I had cataract surgery 4 years ago. Had a very high correction 10 diopter with astigmatism of about 1.25 and reading . I have a multifocal toric lens in both eyes . I see 20/20 with no glasses and none for reading as well. Make sure you pick a highly skilled surgeon. There was about a 6 month adjustment period for me to see optimally. Night vision very clear with minimal halo around lights . I am very happy to be glass free.

    I choose multi focal lens and was very near sighted. Picking a doctor with many years of experience is crucial. My doctor did a perfect surgery and now I have 20/20 vision. Night vision is great also. Don’t need glasses anymore for near and far. Reading food labels and road side signs are 20/20

      I am 58 years old and I've never wall glasses until I got to 50 mainly for reading but when I went to get my licence I did not pass the eye test it mainly is my left eye they say it is because I was a boxer and I have some star in my lens they haven't seen before they are doing my left I first but to be honest I do not want to be blinded and I'm completely terrified I know I have another eye but still do not feel like having my eye cut umy lens anyways I know from what you've said that you had a good experience I'm just wondering what lens they used as 2020 vision would be great that to me maybe worth the risk PS very happy for you💯👍

    My wife and I replaced our lenses with multifocal lenses. I also had correction for astigmatism. No glasses for the last three years. For both of us the lenses were less expensive than a vehicle but more important than a vehicle.

    your lecture was the best i have seen on youtube. Have gone with monvision approach and it worked great. I have worn contact lens for 50 years (monovision lens) and developed cataracts with pseudoexfoliation and the cataract surgery made everything better. Thanks again

    Thank you so much for a very clear and easy to understand explanation of all the optios. I wish I had seen this before my first cataract surgery, I probably would have still got the Vivity lens, but I wouldn't have been as stressed about the decision and trying to find so much information and learning to understand it too.     I chose the expensive toric multifocal lenses that give me great distance, middle and close up vision up to about 20” before slight blurring. To fix that I got progressive readers which are pretty much no correction up top and 1.75 on the bottom, but for driving I just wear regular sunglasses.

    I chose to see close, rather than far because that was my original childhood experience before my vision worsened with age, they also gave me a toric lens, here in Milan... on the health care system. That was super nice of them, now i have thesame vision that i originally had as a six year old.

    I had the dropless cataract surgery done recently. I chose a monofocal lens set for distance implanted in both eyes.
    No regrets and my sight is just fine. Just need glasses to read small/fIne print. In my opinion a healthy dialogue with your eye doctor is paramount and educate yourself before you choose a proper lens for your eyes.
    Keep in mind that a relatively healthy eye is required for best results, regardless of the lens you choose. If you question what your eye doctor says. Get a second opinion.

      @@rhett7716 Me? None, I wanted the best QUALITY vision available with little to no side effects. Monofocal gives me that, not to mention that 84% cataract surgeries result with a monofocal lenses being implanted, NOT necessarily a premium lens. It may have to do with insurance and not being able to afford the out of pocket expense for a premium lens. But I don't like the side effects and diminished contrast a premium lens can give a patient.

      I had great vision until I was 58 then after a head injury I can no longer pass my driving vision test they want to replace the lenses in my left and right eyes they didn't give me any information at all only they were going to chop up the old lenses and putting some man-made lenses yes I'm terrified of them blinding me or at least one eye I know I have another one but I would like to keep the two do you have any advice to what lens I should get as I am terrified it is the 29th of the 8th 23. Surgery around about 3 months from now and I am really not sure as I have really I've been told nothing much at all only that I will need glasses for reading what should be able to see for a long distance 😢

      @@Bok2022st Ask to speak with someone who can explain things in plain English. Ask questions, in particular what are they doing and why is it necessary.
      If you are not happy with the response. Get a second opinion.

    I had my cataract surgery in March and to say I had rather low expectations and am absolutely blown away with what I got. I was not a candidate for multi focal lens and I accepted the fact that glasses were going to be needed after the operations. I had extreme far sightedness with +6.00 in the right eye and +6.25 in the left with a +3.00 for the bifocal. What I got with the single vision IOL for distance no correction required in the right eye and +0.25 in the left and for the bifocal +2.25. I never seen this well without the glasses. I have a slight astigmatism but that is corrected with the glasses. It never bothered me to wear glasses that much but I finally after wearing glasses since I was 3 years old (I am 66 years old) can ditch them and when I am outdoors doing yard work and mowing the yard and wear just a cheap pair of safety sunglasses. This was a life changer for me to see this clearly.

    Great information! I am extremely nearsighted and have worn glasses since high school. I don'twant to give up my up close superpower, so having the choice to keep my current nearsighted prescription is good news for me.

      I was also glad to hear the near sighted situation explained this way. I will have surgery soon, and will be getting the near sighted prescription, but have been unsure about it. Seems like most people get correction for farsightedness, but I have also worn glasses since high school and do not mind wearing glasses.

    Just had cataract surgery also went with a laser corrective surgery for my astigmatism w/upgraded lenes.Told to expect 20/20 vision & to use readers Since I was set for distance.
    Well NO 20/20,now told will need glasses for both near and far sight ! Dr has over 20 yew experience, paid $ 4.700.00 just to STILL wear glasses !! PLEASE Double check your doctor Before ever doing the surgery It is a regret I will live with for the rest of my life

    Excellent video but as a candidate for cataract surgery I am convinced making the right choice is a crap shoot. No free lunch and you can never fully anticipate the final outcome.
    What for example would be a sensible option for a sixty year old myopic with (currently) 6/9 vision in both eyes (index myopia) and PVD who reads a lot, watches movies on television, is used to wearing glasses 24/7, has faultless up-close vision, rarely drives at night and is a perfectionist to a fault when it comes to vision (colour, depth, contrast/acuity, halo and glare-free vision)?

    There will be multiple reasons for explantation of a hydrophobic acrylic IOL. Out of 100 cases that need IOL explantation how many on an average will be due to reason of glistening type of opacification?
    My age is 45 years, cataract in both eyes, weak retina in both eyes and had to undergo barrage laser in both eyes. I do not have any from astigmatism, glauvoma, uveitis, diabetes. I am convinced about using monofocal with UV light filter for my weak retina. Should i go for Alcon AcrySoft or go for glistening free model like Hoya Vivinex?
    I will prefer to avoid situation of undergoing post-cataract surgery IOL explantation for reason of implanted IOL getting severely opacified due to glistening.

    Why do I still have the ability to change focus over a small range with lens implants? What am I doing that allows that - reshaping the whole eyeball or shifting the lens capsule? I really can't tell - it's just instinctual. It does take more effort than normal focusing. Is it potentially harmful to do that, whatever it is?

    I need a single vision lens because I also have glaucoma. After surgery can I wear multi focal contact lenses eventually, after the eye heals? (For computers and reading?)

    Pilots have to use single vision lenses, unless you really enjoy dealing with slow motion bureaucracy. I already use two prescriptions. We spend most of our time indoors; so I'm thinking of IOLs for arm's length tasks and glasses for readers and distance. But then I'm quite used to having glasses off for fine work.

    Great video! Thanks! I've been myopic pretty much my entire life. Currently I wear contacts. The box says PWR -4.75 MED (MAX ADD +2.00). After seeing your vid, I'm thinking the monovision set up might be most appropriate for me, because every once in a blue moon I'll get an infection in one eye which requires me to remove that contact for a couple days to heal. While doing this I hardly notice the difference. As a matter of fact, I'm an aircraft inspector and I find that having one of my contacts out, that eye is almost a microscope, with a focal point about four inches from my eye, while the eye with the contact in can somewhat see distance, although street signs are getting harder to read. My optician says he see some cataracts, so I've set up an appointment with an ophthalmologist. Your video will definitely help me with conversing with him. Thanks again!

      This makes you a great candidate for monovision. But be sure to have one eye done, and then wait for a good bit of time before the other eye is done.Untill you are very happy with the first eye. Don't go for the story that both eyes need to work together...If this happened after the first surgery, you still have an option to fix it ..
      I would choose the non-dominant eye for the first surgery. I am saying this because, not always the results are great. Some people might not be that happy.
      I was one of them.

      I was at -11.5 before surgery - now that's a microscope. When I was younger (and only at -9.5) I could read the 20x microfilm unaided.

    Thank you for the video! It's very helpful. I'm 44 and considering a toroc IOL, but what concerns me is the fact that I have accomodative esotropia due to a high farsightedness (plus 7 plus astigmatism). I also have amblyopia. My question to you is will my eye crossing be fixed once I have the cataract surgery? Since is accomodative, it is mostly corrected with my glasses and my contacts (I can't see well with my toric contacts). Please answer my question.

      It is hard to know for sure, because it depends on a lot of factors having to do with the degree of esotropia, amblyopia, and presence or not of prism in the glasses. Discussing your concerns and also your expectations or desires post-surgery will really help to guide you and your provider! I hope all goes well for you!

    I have super elongated eyeballs. Extreme nearsightedness prior surgery. 1st dr....slam bam.."you only have one choice & you'll still need -6 glasses afterward" Ended up going elsewhere & because I've been wearing monovision contacts for about 10 yrs...opted for "mini" monovision correction with the IOLs
    . Results? 1st of all that yellow filter was gone. My distant vision is pretty darn good..EXCEPT...in low light situations (church sanctuary, movie/play theatres) Which I just can't get over because I end up with double vision in that scenario. Could you or someone else help me understand & possibly fix that? Also, having issues with backlighting (as computer monitor). And, I'm approx. 4 months out from surgeries & believe my vision is getting worse already if that's possible. My reading is pretty good especially if I hold book at a bit outwards. You could hold a gun to my head & I wouldn't be able to thread a needle though. So, my cataract experience both good & bad. It is SO freeing not to have to deal with contacts & solutions. Now, however looks like I'm needing some help in the areas I mentioned.

    I went with monofocal lens. I wanted the sharpest and best contrast distance vision and dim light and night driving. (age 73) As you age rhe amount of light reaching the retina naturally decreases and multifocal lenses make it worse. Using readers is not a big deal to me.

    EDOF toric lenses with blended vision can give you perfect vision at all distances with very little effects around lights

    I love to play volleyball, and so need to be able to track a ball over long to short distances quickly, but I also do a lot of reading for work. Would a multi focal lens mess up my depth perception because of the changing focal points? Am I better off with an EDOF lens or perhaps a monofocal with reading glasses for work? One doctor recommended the Clearview Lenstec 3 for me. Any thoughts on that? Thank you.

      What do YOU want. That's the question you should be asking and express that to your eye doctor.
      Do not let someone choose for you. They may not have your best interest in mind. Ask questions! Why can't I have a particular lens implanted and such.

    What is a multi distance lens ? Could you explain how they work ... they appear to solve all focusing problems

      They do provide vision at all distances, but do have some downsides. They work by having concentric circles of varying powers. The eyes and brain use a focus on different parts of the lens depending on where you are looking (there are variations of this, but that is the simplest way I can think to explain it). The downsides are often halos or glare around lights that can affect distance vision (most often noticed when driving at night) or difficulty with near vision in some circumstances (reading in dim lighting or reading long term). I am making an entire video about Multifocal IOLs coming out in a couple of weeks. :)

    Would it not be possible to make an IOL that the eye can flex to focus on near objects (reading) in the same way it does with the natural lens?

      There are lenses that do this, called accommodating intraocular lens implants, but they are limited in how well they can focus at near compared to a natural lens.

      Yes, the halos are similar comparing multifocal IOL and toric multifocal IOL.

    When it comes to cataract surgery why should both eyes be done?
    I had to have cataract surgery on my right/non-dominate eye due to retina detachment. I got my astigmatism & distance vision corrected.
    But, I have no problems with my left eye up close & the distance vision is great with glasses. So, I would prefer to leave it alone. Why risk possible problems? ...especially, since I will have to continue to wear glasses. I've never been one who could put glasses on & take them off & put them on & take them off. If I put them on in the morning, I wear them until bedtime.

      Good question! Both eyes don’t have to be done. If one eye does not have a significant cataract yet, then certainly it would not be operated on. If the second eye does have a cataract, but the patient is not bothered by it, it is fine to wait. Just good not to wait until it gets too advanced which can increase risk during the procedure in the future.

    What is your opinion of monofocal lens with one eye going for distance while the other is done for close up?

      The results really depend on the individual and their expectations. My grandparents both love theirs, but it’s not the best decision for everyone. It’s best to talk with your provider who knows all of the details of your eyes and their history.

      If your cataracts aren't that bad yet, you could try it out for a month using a contact lens or lenses with appropriate correction (or just one lens out if you already have one eye at -2 to -3), and cheap glasses over them, like you would probably use for computer distance after surgery. You can do this cheaply and easily if you buy online.
      I'm really happy at +.25 and -2.50. They were aiming for zero and -2, but -2.50 is better for phone distance and fine print anyway. I have computer glasses corrected to -1.25 and full-correction distance glasses that I only use when driving in unfamiliar areas where I want to be able to read road signs from great distance (my best eye is 20/10 fully corrected).
      You could also try out multifocal contacts for comparison.

    Hi Dr. Elisabeth,
    My astigmatism is moderate (right eye: -2.50, left eye: -2.00). Can I select regular IOLs instead of Toric lenses when having cataract surgery, then use eye glasses to correct the astigmatism after the surgery, will that be ok?

    Hi Dr. I keep reading that most people have their monofocal lenses set for distance. Why is that?? It seems like it would make sense to have them set for near. Otherwise you are not even going to be able to find your glasses, or put your contacts in.

      That is an interesting thought! Over-the-counter readers are very accessible. Since they correct near vision, it is easy to get vision corrected for distance during cataract surgery and just use the readers for near work. It's also more convenient for driving. If you forget your reading glasses, you can find a way to manage, but if you forget your driving glasses, you could be in trouble! Fortunately, people are able to adapt and learn to put in contacts, even when near vision is blurry. And if vision is corrected for distance, navigating to find your reading glasses will still be manageable. It will be blurry, but not bad enough that you couldn't spot them on the table. All that being said, some people who have worn glasses for their near-sightedness their whole lives love to be able to take their glasses off to read and want to maintain that function. In that case, monofocal lenses to correct near are a valid choice!

I have been far sighted my whole life, and another one my fear is that, if I choose distance, I will get the implants and still be far-sighted, and my eyes will fatigue and cross. Because, I know that right now, if I choose beautiful sharp vision, my eyes fatigue faster, and start to cross. So, I was thinking that choosing near vision implants would help that situation as well.

    I am myopic (have been since age 12; I'm now 68) and my surgeon never gave me an option for IOL that would keep me myopic! He felt the monofocal IOL with laser surgery was my best option. Thank you for pointing out that there's an IOL that will retain my myopia. I will call the surgeon's office tomorrow. If not available, I am going to find an eye center that DOES offer it.

    I was just told by my eye doctor that my cataracts have finally gotten to the point where I am now qualify for the surgery, so this is very timely.

I spent years with mild slowly-developing cataracts, having to use huge screen fonts, not seeing very well at night, not being able to read fine print (eventually, even with strong reading glasses), not to mention the hassle of contact lenses. After cataract surgery I have better vision than I've had in a decade - 20/20 distance and 20/15 close uncorrected, 20/15 and 20/10 distance with full correction.

      @@ramanjyani9384 I have basic lenses with no astigmatism correction (my astigmatism is quite low anyway).
      No halos, itching, or dryness, but I was only 53 years old when I had the surgeries so dry eyes were probably less likely. Eyes were brutally sensitive to dust, smoke, cutting onions, etc. after surgery. That has improved very gradually, and is almost back to normal after 18 months. One eye stayed about 30% more dilatated than normal for eight months or so (not fixed, just more dilated at all light levels).
      One eye had perfect vision the day after surgery. The other had a "grainy" spot in the middle for weeks after, but it cleared up completely.
      I already had huge "cellophane" floaters, but did get a couple new ones. Usually they are not blocking my central vision, and if so I can "flick" them to the side.

    If i choose a distance monofocal at what distance does it get blurry where i need to use glasses

      It will tend to start getting blurry around arms length.

    why do people get lasik instead of just getting intraocular lens implant?

      That’s a great question! LASIK is less invasive and less expensive. LASIK only interferes with the outer layer of the cornea, while the incisions made during cataract surgery (though very small) allow entrance into the eye. LASIK is about half the cost of Clear Lens Extraction on average.

      Remembering that a standard lens should be covered by insurance, the other advanced technology lenses could range from $900-$2,500 per lens.

      If you dont have cataracts and use a premium lens, your looking at around 12k.

      @@dr.elisabeth7778 $1550 if you want LenSx per eye this is a basic lens. Treatment for Astigmatism is $2700 per eye 3rd choice is $3700 per eye with the LenSx. I just wonder how much better the LenSx surgery is than the blade surgery

      I'm an optometrist. We are trained in things ranging from prescribing glasses and contacts to treating ocular diseases (pink eye/infections/glaucoma), to managing ocular complications of systemic diseases (diabetes/autoimmune diseases), to doing minor procedures.
