Neurological exam and structural treatment following lumbar disc injury, flexion-distraction ASMR.

  • Our patient Jeneen had a herniated disc a couple of years ago, and after a great deal of thought and advice, opted for a surgical microdiscectomy. As happens in most of these cases, the healing is not rapid or linear, and she still feels some stiffness in the lower back and tingling and cramping in the right leg as the result of the injury.
    We discussed her progress and I determined that there was no present tension on the nerve root with a straight-leg raise, Braggard's test, and Valsalva maneuver. She also had full strength of the toe flexors and extensors, the tibialis anterior, peroneus tertius, and tibialis posterior, and peroneals.
    She does have some sensory deficit in the right leg consistent with the dermatome of S1, which correlates to the L5 disc, which is the disc she injured. The conclusion was that there was slight compression of the S1 nerve root on the right.
    Following the exam and determination that treatment would likely help, we worked prone through the lumbar and sacral regions using myofascial releases and flexion/distraction, as I try to talk through my thought process and what I'm seeing as I work with Jeneen.

      but... in the uk he wouldn't be a doctor. He'd not even be a health care professional. In the uk we recognise he would me a massage therapist. at best.

      ​@@emsservices6090 doctor of chiropractic is different to doctor of medicine and this is recognised in the UK. I've been to a Chiropractic Dr in the UK.

    Another lovely video ❤ I had a pretty bad S1 injury in 2013, had no surgery just stayed active and I would say it has pretty much healed. Still no achilles reflex but that's it. I hope the patient had a wonderful response 🙏

    Ohhhhh Dr. Russ, wish us veterans had access to this conscientious style of adjustment! I’ve had same symptoms of Janene(also my sisters name:-)for about 15yrs. I have tried everything and I mean everything(outside of surgery)but I am at a crossroads and need injections 2-3 times a year. Just nothing stops the pain. But being only 45, can’t really stop adulting.

    Wow! I wish you were in my area in Maine. I keep telling MTs and Chiro my ribs are stuck on right and can't get any of them to listen to me. Chiros I've been to just mash on my ribs and lock them up worse!

    I was diagnosed with MS about two months ago after dealing with neuropathy from my knees down for 4 years. I would love to either get rid of the neuropathy all together or lessen it. I have lesions on my brain and three places on my spinal cord which is what's causing the neuropathy from what has been explained to me. I have been on medication for the two months. I don't have the tremor or the extreme compression sensation in my back. I use CBD oil on my spine which also helps with the MS Hug.

    Hi Dr. Russ! Can you tell the difference between nerve damage and nerve compression?

      The symptoms will be the same, nerve pain such as tingling, sharp/shooting pain, hot/spreading pain, numbness or cramping. The differentiation is in the history--how long has it been going on? And what is the cause? For example, nerve pain from a severe disc injury 5 years ago is probably "damage" and is probably long-term, where nerve pain from a mild-moderate lumbar sprain is probably "compression and is likely to resolve.

      I had a laminectomy with spinal cord tumor resection (t10 t11) in April of 2022. Most of my symptoms are in my left leg (decreased sensation, decreased proprioception, and pain) and are worst at my toes. From what you said I think it’s most likely damage. But, is there anything you would recommend I try? I can still walk but running and balance are still things I am working on.

      That must have been quite a surgery to recover from. I am sorry that you have continuing symptoms, but I am glad at least you are able to walk. I can't give you any specific advice, but I can say this: keep working on your balance and strength. You are still just 20 months from major spinal surgery, and you may yet regain more function.
