    YES! VERY HELPFUL!!! As someone with bad melasma, I've been waiting on this video! I'm not planning to use my hydroquinone again (only because I hear horror stories about ochronosis) I've used it off an on for a few years. So I've been doing a ton of research on po Tranexamic Acid. I would love to know your thoughts on supplements such as MSM, DIM, Grapefruit Seed Extract, Milk Thistle and some others to deal with melasma? I take a DIM called Estroblock Pro for horrible hormonal acne and it was a miracle for me (completely wiped it out), but I also noticed my melasma lightened up some! So it seems like my estrogen dominance really does make my melasma worse.

      Is ochronosis a side effect of TA? That’s great if it isn’t. I was on hydroquionone a while back and wondering if it helped or hindered my discoloration.

      Since the research seems not to support the effectiveness of TA in a gel-formula and suggests that it could even cause irritation, and the inkeylists tranexamic acid is in a gelformula, my guess is she wouldn’t recommend it. But, I’m tempted to try it still, since I don’t have access to better affordable options and people seem to like it, so would love to know for sure. What I understand from the research she’s citing, is that possibly an emulsion/cream at 2-5 % might be effective and safe, but it’s yet to be determined.

    It’s so weird I have the worlds most sensitive skin and use hydroquinone 4% and I’ve never had any side effects or dryness or redness. I hadn’t even realized It gave those side effects til you just mentioned in this video

    Cosmelan 2 cream is something you should addressing too! I just got the Tran pill. I may have lied to get it bcuz my insurance wouldn’t have paid for it.. but I’m on my 4th pill. I was already treating my melasma so I’m already seeing results.

    Happy Tuesday! The placebo treatment always surprises me when I see the results of a scientific study. Asians always seem to be ahead of other countries. Is it reality or just an impression, I don't know! But you are here to share with us everything that is being done on a scientific basis. Very reassuring to see that there are specific studies to find a product for melasma. I'm still a little skeptical about the oral treatment. But the other approaches seem very promising for the future. One day, we will surely see a positive result from these studies. Very informative this video!

      The oral treatment is the only thing that works for me. I am using aggressive sun protection and my melasma persists without the pills.

      Unfortunately/fortunately Asians have severe issues with being tanned or having pigmented skin. Fair skin = beautiful and vice versa. I’m sure many people have damaged their skin in hopes of getting it lighter. I know I have-I’m indian. Maybe due to the history of colonialism. 🤷‍♀️That’s probably rooted why they may be leading in skin “brightening” meds. Of course, the west is the opposite where a tan is seen as healthy.

    Dr. Dray, what do you think of “La Roche posay Effaclar MAT” face lotion? ( as far as ingredients/vitamins that sort of stuff) 😃 Have you tried it? Do you endorse it? Im a 22 yo Male & am trying to maintain a good cleansing ( Toleriane Foaming cream) & moisturizing ( Effaclar Mat) regimen. Any words from the wise and beloved Dr. Dray on these ? 💙☺️

    I'm using a serum that has that ingredient and with just a few days of using it i can really see a difference in how bright and even my skin looked, and that's the only thing i'm using right now after washing my face, twice a day

    What would you consider is better for melasma, tranexamic acid or alpha arbutin? Also, are there non-tinted sunscreens available as I do not find tinted sunscreens convenient especially if you have to reapply it throughout the day.

    Sunscreen helping clear up melasma by 18%. Wow. I knew it helped my dark circles a lot, but didn't realize sunscreen was this important.

      Same! I got my melasma down to 96% gone. Went 1 week indoors with no sunscreen. It was all back in 3-4 days bc it was so hot inside… after weeks of getting it down. Now that is use sunscreen 24/7 I’ve never had that issue again.

    Is the risk of stroke mentioned in these studies? Discontinued use, patient failing to take Lysteda as prescribed could result in hemorrhage? Are these patients also followed by hematology?

    Hi dr dray , can tranexamic acid oral be used for old post inflammatory hyperpigmentation which Hasn’t gone away ? Once you stop taking the tranexamic acid then will the hyperpigmentation come back worse ? Can you start back on tyrosinase inhibitors when you stop the oral TA ??

    i take Tranexamic Acid for heavy periods. It works well but I can only take it for 5 days maximum, however I take 1 gram 3 times a day which is significantly more than the dose you mentioned for Melasma.

    My doctor put me on twice daily oral tranexamic acid for melasma (that persists despite aggressive sun protection) and it really helps. I also notice my rosacea is better controlled with twice daily Vs once daily dosing.

    Triluma didn't do much for me but oral TA helped dramatically. It's the only thing that worked. Unfortunately, the melasma starting returning almost immediately after stopping the pills. I use two layers of sunscreen on my face and have for years, so there are definitely other factors (hormones and genetics) involved in my melasma.

      Marcy W hi when you stopped the TA then how come you didn’t start topical tyrosinase inhibitors cream to maintain the results ?

      @@dynamitedynamite2737 I was using Triluma as well and it helped a little, but it didn't work well enough to maintain the results of the oral TA. The best melasma "treatment"? Menopause! But of course that comes with its own miseries.

      Marcy W oh really menopause ? I was told sometimes it will go away when you menopause and sometimes it gets worse ! How much of a difference did you see when on oral TA? There is topical TA too and can oral TA be used for post inflammatory hyperpigmentation?

    Just discovered your videos today and have spent the last couple of hours catching up on the wonderful information you have to offer us. I was hoping to see a video on Keratosis but have not found it yet. I am 65 years old and a few years ago I starting see these brown spots on my back. As this condition became prominent, I saw a Dermatologist who told me it was quite common with age and that there was nothing that could be done about it. Now there is hardly an area that is not effected, even on my scalp. This has effected my life greatly. I will no longer wear a bathing suit, clothes that do not cover most of my body and, I have completely stopped dating. I find it very embarrassing and God forbid I become involved with a man who would want to have a sexual relationship with me. I live alone. No children or family. I had hoped to meet a gentleman, fall in love and grow old with him. I now feel destined to grow old alone and become a recluse. Can you offer any advise? Is there treatment?

      Tranexamic acid. Superdrug tranexamic booster by me+ less than a tenner or facetheory exaglow which is vit c and tran acid. Much better than things with arbutin like the ordinary bit c n alpha meh!!!!!! If you have melasma eucerin dark spot corrector is a game changer. Along with a few other things mines gone

    I have a platelet disorder and have been taking tranexamic (lystedia) for a few years. It's been a miracle drug for me! I don't have melasma but would be interested to know if it would work on sun/age spots.

      there is no safe amount of sun that will "raise your vitamin D" without risking skin cancer...you can get vitamin D from diet and supplements if indicated....the majority of the UV that hits the skin is not even the right wavelength to activate vitamin D synthesis

    Dr. Dray can you make a video about mosquito/bug repellents especially since summer is here. I know that there’s chemical and all natural versions of them. I would love to hear on what you have to say about those. Also, I’ve heard you say that essential oils are not the best to have on our skin. You’re not a veterinarian but would you happen to know if that same fact applies to the dogs skin as well. It would be a life savor if you had a video going through pet products (shampoo, bug sprays etc.) ingredients.

    Dr. Dray, Please do a video on poison ivy rash. I live in Michigan and I wait all winter to get outside and enjoy. Unfortunately, the last few years, I have struggled with a poison ivy rash. This year I got the rash putting the pants my husband wore to mow the lawn in the washer. I am amused and bothered by the crazy remedies random people offer. I have gone to the doctor, but can you offer some advice. Is there something at the store I can buy for small areas that works? What ingredients should I look for?

      It’s not about the makeup. It’s about how she’s selling expired lipstick with holes due to bacterial growth, imbedded hairs and various other foreign contaminants. There’s all sorts of controversy over this and hopefully the FDA will force a recall. I was just hoping you would shed some light on the cautions on wearing expired product such as this. Also, you have such an amazing channel with a highly educated and honest view point. Your unbiased would definitely be beneficial, and it’ll certainly help expose you to new viewers.

    Y’ALL IN THE COMMENTS ARE SO FRUSTRATING. The evidence for efficacy of TXA is in oral and intradermal delivery systems not topical. Skincare brands know this and that is why TXA products are typically paired with brightening ingredients like niacinamide. If a TXA skincare product works for you (it’s probably another ingredient) then good for you. She’s not forcing you to throw your products away 🙄

    Dr Dray, if you have sunspots and sun damage is there anything that can be done to prevent cancer on top of sunscreen? I am trying not to obsess about the aesthetics because it can be costly and quite temporary but I wonder if laser treatments and so on reduce cancer risk. I can always wear concealer. I was joking with my husband that my sun spots act like an exposure monitor like what radiologists wear to detect radiation exposure. I just learned that there is something called DNA protecting enzymes and that is interesting but it seems to be on the down low. Have a great day and thankyou for all if this info.

      there are many treatments that can reduce the burden of pre skin cancers, but are only indicated in high risk individuals/those with multiple skin cancers
