What was the cause of these farmed fish? Bad connection, pulled hook, bad line...?

First loss was due to the chain knot breaking, I suspect somewhere down the line the bottom loops crossed which created a weak point. Second fish was due to it rubbing on the boat, I was fishing with very tightly woven line so any kind of knicks would break it
