But i just wanted to to tell you that i can't believe you posted this only 4 days ago...my father recently passed and i have listened to this song over and over again. I just wrote my own lyrics to it and i wanted to learn how to play it so i can sing it for my mom. thank you so much for posting this. its very helpful.

At bar 13? I know what you mean because it sounds like the strings might be holding a D chord there, so combined with the piano part it almost makes a Dm7 sound. You could easily play an inverted Dm chord there just by moving your second finger from the C to the D instead, but either way sounds good!
If I may suggest a minor tweak, the song goes like this: F/A - G - C/G - F (so far so good, but then there is a minor tweak) F/A - G - C/E - F/C ... try it!

I'm having a problem with the 2:57 part. Is that not supposed to be a dm chord rather than the usual inverted f chord?
