This is sad to see. I remember watching your videos last year about your house. This is a common problem that happens in Ghana. There should have been a damp proof membrane laid on the foundation. How did you solve it? Did you introduce damp proof chemical before plastering?
Hi Naa, yes the damp proof chemical was used before plastering. I've since learned that even if the DPM was used at foundation it doesn't guarantee no damp issues.

You need to fix the problem at the source. Install a foundation drain or French drain. After that you do the replastering.
