How they ensure 360° observation and manipulation during procedure? Human anatomy might differ drastically, not only by size, but also blood vessel, muscle etc structures. There are also about half a dozen medical problems which if resolved currently have a chance to get Nobel Prize.

To answer your question, it works exactly the same, i.e. if you transfuse whole blood you can get all sorts of fun side effects like arrhythmia, hypothermia and up to metabolic alkolosis and heart attack. And regarding transplants, even if it's from a twin you would still have to take immunosuppressors for like ever, and your overall lifespan would be slashed in about half, so yeah, that's not the way chief.

Substances can still interact with the neurons and all other cells WITHOUT the need of blood. Blood is just a means of transportation and delivery of necessary substances. Plus, the operation is held at 5°C, which is close to the freezing point, maintaining integrity and avoiding degradation of the cells
