Understand it in simple words: Doctor patient study
First patient have give all the details like name, address so it's case profile.
Next patient will explain problem, causes etc.
After that doctor will guess various symptoms etc... hypothesis.
Then doctor will write all the causes, hypothesis and collect it,
Next analyse it and based on analysis, will reach to final decision.
Then verifies again all the details.
Then write medicine etc...
At last will say him/ her to follow up some steps.. that's it.
