Hopefully I was able to get across the point that mainly these are general guidelines and considerations and there is going to be considerable variability between people based on life experience, extent of arthritis, pain, and symptoms. If you have significant pain and symptoms that are impacting your ability to tolerate normal daily tasks and activities, and you've tried other management options, then knee replacement surgery may be appropriate. The ortho field has blood on their hands! I wonder how many people have taken their lives out of sheer desperation because this rigid antiquated paradigm that replacements are for the elderly. No! Replacements should be determined based on function, pain and quality of life, the end!!!!
I do agree with you that the main determinants for knee replacement should be function, pain, and quality of life. However there are also risks with surgery and especially concerns with younger surgery, most notably increased need for future revision surgery. That is why it should be considered the last possible option after failed conservative treatments. I also agree that the age cutoff for surgery should not be rigid, this decision is complex with multiple factors and should be made on a case by case basis.
