    Bone on bone hip arthritis (osteoarthritis) can be treated successfully with exercises that focus on the right muscles.
    Healthcare generally pushes people suffering from hip arthritis to get surgery as soon as it begins to hurt. They may try exercises, but often don’t do the right exercises and end up making the hip pain worse.
    Then people tend to quit on exercise, deciding it didn’t work for them. They often have hip replacement surgery when they really should have just changed the way they were exercising.
    Although complications from hip replacement surgery are generally low, the problem with this is that the root muscle weaknesses were never addressed.
    That means that people still have muscle imbalances after surgery. Then it’s only a matter of time before the next joint is affected and the chance of having another surgery increases.
    Often people end up having back pain, knee pain or their other hip begins to hurt as well. The replaced hip tends to do really well, but it’s at the expense of the rest of the body.
    Whether you are open to having surgery or not, you would benefit from fixing the root problem so that you have the best chance at preventing the surgery OR making it the last surgery you’ll ever have.

    You have helped my bone on bone arthritis to the point the pain is gone range of motion significantly improved unfortunately I had to cancel my June 1st surgery because I had nothing that needed fixing . My crutches are now on Craigslist with my Walker - thank you 😊

    I'm flabbergasted that every single exercise you have touched on, I have been told to do them all by a PT..your videos are the best and give the most perfect and sound advice, I was told to balance on one leg and drop as close to the floor as I could saying that it would improve my muscle strength. Your videos and advice are like gold! Thank you so much as it gives me so much hope that I can avoid a hip replacement.

      I'm the same. As a martial arts instructor I still do all of these thinking they were helping me. I've had hip pain and torn labrum for a few years now. Doctors said 3 years ago the problem wasn't bad enough to justify surgery. I've been doing the glute exercises from the other video and they are helping.

    I don't have hip arthritis but I have done every one of these exercises following a PT or personal trainer's advise. No more of that. Dr. David following your glute exercises has helped my knees, balance, walking and going up and down stairs decreasing the pain by 90%+, what a blessing. Thank you ever so much!!

    My PT exercises reduced me to tears - I never went back and turned to Dr David instead and have never looked back ! I don't think u know it Doc but ur one in a million. If I'm ever in El Paso I want to come and shake ur hand in person - it's on my bucket list 😊

    Thanks Dr. Middaugh for your quick and informative reply. You're right about "overuse." I was diagnosed with degenerative arthritis almost 20 years ago. Needless to say it's gotten worse over the years. Just walking can be somewhat painful. When I get out of my car I have to stand there for a few seconds to stabilize myself or I will l limp all the way to my destination. And after sitting for a while then standing up I'm very stiff so it seems like I have to move for mobility.
    I've lost about 35 lbs from about 185 lbs at 5'6 since January and I'm easing back into exercising. About overuse again. I have an exercise bench with connected cords and an attachment for leg extensions. I remember last year I overloaded it and was inflamed for days. I haven't done them since. Hopefully I can add them back in moderation and see what happens. Thanks again and my apologies for this long reply. I watch many of your videos and they help me immensely. Take care.

    I did most of those exercises while recovering from a partial knee replacement.
    The knee gradually got better but then my hip started to hurt and became excruciating.

      i am right with you K B Good. After listening and learning from Dr. David, I too was doing wrong exercises. I don't have hip arthritis, but had knee pain and those wrong exercises didn't help my knees or othe ares I was working on. What Dr. David explained and demostrated for knees has been a painfree life savor! Dr. David is the best!!!

    I was prescribed these worst exercises by a local physiotherapist and my hips osteoarthritis is worse, bone on bone. She recommended hip surgery saying they do good after that with recovery exercises.

    Seated marching hurts like hell. Knee extensions too. Lunges too. I’ve been doing these in my senior’s exercise class. No more.

    This has reiterated everything I just recently realized. I am over stretching. Been doing yoga for years and some pilates, but I have to be super careful with some of these moves now or I will be limping for a few days. Thanks for the great instructions on these videos. I feel more hopeful now. ☺️

    The PAC for my Osteo Doc will be sending me to a PT soon & I fear that PT will be incompetent.
    Too many comments here say their PT advised them poorly & that they actually hurt even more after their appointments.😢
    To get mixed messages on such a critical health problem is just beyond aggravating.
    At this point, all I want is surgery now! This is hell.

    Thank you! I have a torn labrum and was told to do straight leg raises. Recently I did quite a bit of this in one of my workouts, but I felt like it made the problem worse. I started searching youtube and only your channel seems to understand how it all works.

    I've been doing exercises from a functional fitness expert I was referred to online for about a year. Some of them are straight leg raises. I have had bone on bone arthritis in my right hip for 1.5 years, which makes me limp. Anyway, I cannot do those exercises with a straight leg on my right side, due to the arthritis. I do them with a bent knee. I can do them with a straight leg on the left side. But you say not to do them at all. Frankly, that is kind of a relief, since they are some of the harder exercises I have been doing for the past year.

    Beautiful video. Doctor ... why does every medical professional attempt to push me towards surgery for my bilateral BOB DJD and hip disease? I don't want it. I choose exercise. But they don't respect my decision.

      Glad you found the video informative. You can always keep yourself away from surgery if you work hard on fixing the muscle imbalance that is causing your pain and increases the compression that causes DJD and arthritis.

    Dear Doctor in El Paso. I atched your lesson on exercises to help prvent the possible hip replacement surgery. I have been walking and attempting, keeping the noxe to naval aligned with a straightened spine, and it has already helped me a lot. At first I forgot about eh trick of keeping the toes pointed outward, but when I added that touch, it was amazing. Thank you. I do ha that condition (OA), and that little bit of exercising helped me to sleep better also. One day and onenight, wow. Thanks for your saintly advice.

    I've recently had a hip arthroscopy and the first and last were very much my main exercises I was recommended. And yes I struggled with them. They said I had a weakness in my hip flexors. And doing them didn't help my burning pain.

    I'm really glad I understand this better. I know my hip flexors are dominant, but technically also weak. It's counter-intuitive but makes sense to me that I have to stretch them gradually and focus on glute strength. I'm so glad I found your channel.😊

    Dr Thank you very much for your videos. My GP told me I have legge calves disease and I should see an orthopedic surgeon and PT. I'm yet to meet all of them but ur videos are really helping me.
    My problem is the thigh muscle is reducing in size, can it become big and how should I go about it?

    I’m learning so much that makes sense. Thank you. I’ve often described pain in what you’ve just taught me is my hip flexor and will now avoid those. Thoughts please on doing exercises in the pool, they feel comfortable, I’m doing 3 sets of 12 each. I’ve been doing knee raises, kick backs, lunges, single knee squats, side to side swings, front to back swings, standing clam, walking sidewards. Thank you

    Love your Channel David! Very useful and helpful! What about air squats? I’m going crazy trying to figure out what works and what makes it worse. I did heavy leg extensions Saturday and paid for it with pain the rest of the day. I wish I had watched this first!

    Thank you for this advice because my hip is not properly aligned where I should have a hip socket. Which becomes extremely painful 😣. I will definitely look into a knowledgeable physical therapist.

    David, This is a great revelation to me when you teach us the worst exercises for Hip Arthritis. I, like others, have been doing all the wrong things and I am getting worse. Now, I know why. I am aggrevating all the wrong muscles. What do you think of an exercise bike workout instead of walking? Instead of pushing my thighs hard, I am trying to only work the hamstrings. It seems to be helping. Plus I am really working on as many glute exercises as I can because, as you have explained, they are often weak and mine are definitely in that catagory. My knees are just so weak also but I am trying to use my butt muscles more now to take the pressure off the knees. Thankyou so very much for helping us stay away from surgery.

    Thanks for a great video. I have also been given some wrong exercises by my PT and suffered from lots of pain. Your explanation really opened my eyes. I have just started with your exercises and I can feel the difference in my general movement and strength. The butt tensioning exercises seems to be the most effective in my case. Thanks for well explained videos. It really helps.

    WOW - I did all of these exercises at the Y for the past 2 years before COVID and and now I’m questioning right hip replacement that I have scheduled for 11/3, but as Mary Lou Retton
    said before she had her hip surgery, “when pain actually overrides the fear” she would go for the procedure. That’s where I am at this point my fear has caused me to seek out other options and since finding your website a few weeks ago I have been doing your suggested exercises and will be postponing my procedure for the time being. Thank you for the possibility of alternate options.

    Doctor, I understand about avoiding straight leg raises when you're raising the leg up while lying on your back. But how about the position where you are on your side and then lift the leg up? Is that still something that should be avoided? I just subscribed to your channel and can't wait to go through all the videos on hip arthritis. Thanks so much for all you do.

    im so glad ive found your channel! got labral tear and I realised that using hip flexors causes me a lot of pain. Was told to do leg raises and that worsens the pain! Thanks for this video! Also, could you perhaps make a video on alternative exercises how to strengthen lower abs without using hip flexors? or how to do bridges without using hip flexors? and how to properly strech hip flexors - i did like a lunge stretch as you showed in a video and i found that it hurts or irritates the labrum

      Bridges are good to strengthen your hip extensors. You don’t use your hip flexors with them

    I have Osteo Athritis in my right leg. Pain is mainly along the front of the thigh down to the knee and that muscle was very tight for a long time. Physio did massage and muscle now not so tense but I need to remain aware not to tension it up as it seems to do when i am distracted. I do ride a Bicycle for exercise. Should i stop this for a while? Thank you so much for your detailed explanations and now that I understand the mechanics of the Osteo Arthritis maybe I can fix it with the correct exercises. Frank

      Hey Diane, be sure to listen to your body. If you hurt when you get home, then you likely over did it with exercise. Tone the intensity down next time so you find out what you can do without hurting when you get home. That's what allows your meniscus to heal naturally.

    1 out of 4 orthopedic doctors said I needed hip replacement. Arthritis and no pain and very mobile. Thank you for your great videos. After 3.5 months, my hips are feeling stronger. I walk daily and do weights at home. I want to join the gym and use the machines. I do have osteoporosis and am careful with that. Are there any machines I should not use. I thank you so much. I am planning on leaving with all my original body parts, God willing . 😊

    Is it harmful to walk up and down fairly steep hills? It feels great for cardio but after this video i’m afraid to overstrengthen my quadriceps. Your videos are wonderful, thank you.

    Thank you for your commitment to excellence. I, too, have received so much bad advice from so-called health care professionals. I think I have hip bursitis, based on your description in the video on exercises for hip bursitis. I have been doing the butt scrunches, self massage on my quads, and sleeping with a pillow between my legs, and will add the clam exercises and the stretch.
    Question: are bicycle riding and walking ok to do? Sounds like a no-brainer, but now I really wonder. My quads are sore and my hip flexor "pops" occasionally, and based on the information I've heard from you, maybe the marching movement of bicycle riding and the quad exercise from both riding and walking will only exacerbate the hip bursitis. Help?

    What about straight leg raises while lying on your belly? There are 3 postures in my yoga class that involve those, both one leg at a time and both legs at a time? Thanks.

    Thank you for confirming my suspicion that lunges are not good; straight leg lifts also hurt me while leg extension hurt my knees and so do some of the stretches mentioned. No wonder I just cannot improve. Does that mean I cannot do any quad exercises ?

    So with my ongoing hip issues, backward lunges are not good. How about Romanian Deadlift? It involves gently hinging at the hips and strengthening the gluts. The variation called the Bulgarian Romanian Deadlift with one leg back on the chair and going a Romanian single legged Deadlift May out too much weight/balancing on one leg which I’m thinking May aggravate my hip pain. Kettlebell swings also involve tightening the gluts. I’m thinking

    I have left hip arthritis, but my left thigh is extremely sore, when i set down and get up its so sore it's yakes me a few minutes to put pressure on my left leg stepping down to walk. When i do a lean back stretch for about 10 seconds then im fine, if i bend down i have to stretch my left leg back in order not to feel the extreme soreness.

      So I have left knee pain and left hip early arthritis. So if I strengthen my hamstrings it will help my hips but aggravate my knees?!? I feel like I’m between a rock and a hard place!!!!

      @@luv2cruise100 best for both injuries is to go up on your toes now bend at your knees. Hold pose. You may want to hold onto a table or ledge.
      I have built up to 3 hrs a day
      X serious left hip and x serious rt knee injuries. It helps both
      This is a contraction exercise. After doing it rest then just stand on one leg. Hold on if you have to but build up to no holding on. Build up the time on this one too. Can do anywhere including brush teeth waiting in a line
      I ve tried everything. These two are working

    Great info. I’ve been doing yoga for decades and with this info realize many of the postures are not helping me in the long run. I need to think about how to do the classes so I have options or just throttle down.

      I'm a long time yogini also and was dismayed to find many of the poses were really bad for my hip osteoarthritis.. and I have osteopenia of the hip. This site has helped me realize the yoga has caused muscle imbalance... over developed quads from all the warriors and even doing chair improperly. I feel so much better activating the glutes and not working quads!!

      @@vtrep737 same here. I don't do yoga regularly but when I did, I was so focused on my quads and paid for it the next couple of days. I thought I was doing it wrong and went back a few days later with more intensity. All wrong, Never again. I do Dr. David's exercises, and I find I want to do them which makes me do them more without pain or injury. Am I making sense?

      I am confused. Because some physios say, tight muscles sometimes are weak muscles and that they only had a chance to give up tension when strengthened.
      I did professional horseriding for years, my hipflexors, adductors and also external rotators are stiff and shortened.

    I am 52yr old female with arthritis in bone hips (diagnosed at 48) that stems from displaysia that i was born with. I also have a spur on my left hip. I am an avid runner and the inability to do squats and lunges has left me with virtually no muscle in my upper legs. I used to have solid quads and stabilizer muscles. It is extremely frustrating that I feel like there is nothing i can do. Long distances have become a struggle for me and I live in constant pain. Haven't gone back to the doc because I am not prepared for surgery or whatever else they want to throw at me. Thanks for telling me what NOT to do.

    Excellent advice! I was wondering what you think about crossing your legs when sitting? Does it increase pressure and over time cause wear and tear, and set one up for arthritis? Thank you.

      Hey Shana, great question. We don't have any information that crossing legs makes arthritis worse. It does hurt if you have sciatica because it makes the nerves stretch. But it doesn't usually affect your joints to cross your legs (unless you already have severe knee or hip arthritis.)

      @@epmanualphysicaltherapy I'm 40 and I've been diagnosed with hip arthritis. Well, walking, seating, getting out of the car... I can do all of these let's say 90% normally - But I can't cross my legs! Even it's been diagnosed only on left hip, I can't cross them either way. External and internal rotation also limited on both hips - but I have no pain, nothing is hurting. It's more frustrating because I stopped playing soccer few months back cause of that... Because after soccer I couldn't walk normally a few days. Now all that is left is hiking :)
      Am I asymptomatic case (one PT told me that)? Should I be worried that it might get worse over time... just by doing nothing? And of course I started doing your exercise and stopped some of them after this video... ;)

    I have also been told to do every single one of these exercises & then literally spend days in excruciating pain, worn out & depleted of all energy; having to alternate soaking in the tub with Epsom salts, icing the joint’s & using a heating pad; not being able to go anywhere. The fatigue from the pain is also extremely severe & annoying. I’m also wondering about “clam-shells” exercises on my back & sides as those are 2 others I’ve been told to do as well. Are they good or bad for hips & knees? I have not had the joints replaced, but they are extremely tight & painful & overtime, I have had Steroid injections, VISCO Injections, & PRP Injections, along with PT to try & avoid surgery.
