Maybe lock the pallet into place? Looks like you didn't get the forks in far enough into the pallet.
Also, how about keeping your eyes looking at the direction of travel; if you need to look off to the side, stop the order picker!If this was a certification test; I don't know if I would pass him. He would certainly gotten a few warnings.
