Just follow other research paper deeply and understand their researh work, writing format which is varied journal to journal. I hope, you get your answer.

This might be 2 years too late, but you generally create the abstract after you've done all your research and obtained all of your data. Its basically a very brief overveiw of what your paper is about, and what you found. Then you go onto explain in your introduction and subsequent paragraphs in more detail why, what and how you're doing your research.
Then your results, with explanations to support theory, then conclusion. You add all your raw, unedited data as recorded in appendices on later pages, then your references to support your research, bibliography too on everything you read.
As for author you would put your own name, if you're the sole author, the dept. studying. If going about it independently and not as an enrolled academic then it would be your business/home address.
If working with others, collaborating, you add their name next to yours, separating them with commas. I think you can also add an email, some papers have them.

There are some research paper templates included with the newer versions of MS word. Ones with cover pages, specific headers/footers etc. But each university and journal publisher have different remits for submissions, so it's best to double check. For example some papers get rejected if they aren't double spaced paragraph lines, or formatted to specifics. Some can be mega fussy.
