    The biggest thing about this case was the fact that she had reported her error the moment she found out and NEVER tried to hide it. That is why the family forgave her, she knew she did wrong.

      It would have come out at the autopsy tho. They would’ve found the lethal dose of medicine and it would be easy to identify who gave it to the patient being that this woman was her nurse. She would’ve been in more trouble if she lied so of course she told the truth.

      @@lovejunkie6078 I remember reading that the original death certificate said she died of natural causes. Vanderbilt failed to notify the state of the drug error related death, even though RaDonda Vault reported her error immediately. It was determined that Vanderbilt did not have safety precautions in place that would have prevented Mrs Murphy's death and was only required to submit written safety precautions to maintain their Medicare and Medicaid reimbursement status. There should have been no way for an override to make vecuronium available in the first place. Vanderbilt faced no penalty for their role in Mrs Murphy's death.

      @@lilkittyanime Ms. Vault immediately reported the error. She didn't try to hide it. Vanderbilt failed to report it to state. They tried to cover it up and a death certificate was issued that said Mrs. Murphy died of natural causes.

    This right here is why they shouldn't force nurses, or even doctors, to work these grueling, insane shifts without any sleep, or give them up to 60 patients with no attending doctor. The entire medical industry needs to change.

    I cannot fathom the guilt she has to shoulder for the rest of her life knowing that a mistake of her's took someone else's life away. Even if you say she has to face consequences, that's a punishment in itself.

      Well it's kind of hard to hide a murder at a hospital of course she's gonna report it right away, people act as if this was on the street or in a bar. If she didn't report right away and try to cover up they would trace back to her anyway... She did nothing admirable by reporting "right away" she killed somebody on accident, we call that manslaughter in america. If that was your brother father sister you'd be upset but I'm sure a big lawsuit settlement would wipe all your tears away. This nurse did nothing special, she effed up

    Thank you for forgiving her, we live in a society today where there is no grace for any mistakes. I pray she forgives herself as well. I was a nurse for 17 years and the patient load and expectations for nurses were bad back then. I can't believe what they expect out of you today. My last job they gave me half the floor to take care of, they all were alot of fresh surgery's needing medications. I was in tears by the end of my shift, so I went to the head nurse that morning when I got off and said if this is the way you do nursing I don't do it this way and left that job.

    I once accidentally gave the wrong dose of an iron supplement to a young child. It was the one and only mistake I've made in my entire career. Nobody was hurt, and the issue was corrected, but I still think about it. I cant imagine how much worse it would be if my mistake had killed someone. I hope she's okay.

      We all make mistakes, some can be reversed yet some cannot. Its also human to forgive because mistakes will always be there. Its not easy to end someone else's life when you didn't mean to and to live with the guilt for the rest of your life.... I hope she is ok too

      . It actually cost you your entire career. Presume you had studied for approximately 15 years in the field, and due to a mistake, your career as a physician would be abruptly ended.(It's really tough to make it through probation, and fear can start to consume you just like plague.) That is truly a devastating outcome.

      That’s why many kill themselves. Honestly, my hospital is so strict on everything and I’ve had a couple of times accidentally forgotten to waste medications and I’ve had to do urine tests and whatnot and it was grueling. It made me feel so low. Now I’m just hyper vigilant which can be unhealthy but hopefully I don’t make any more mistakes or I get fired.

      Right! I wouldnt want to do it. Think of medical, military, daycares (being distracted and dropping a baby), mechanics, you name it. Accidents happen. We have to accept that things can and do go wrong. Let's stop trying to end peoples lives because of it and hope it doesn't happen to us...

    The victim’s family is very honorable and peaceful and I’m glad they chose to forgive than to try to destroy someone else’s life. Horrible tragedy. I wish the victim’s family nothing but the best from here.

      There was a doc's order for a correct med. She overrode the correct med in the computerized medication dispensing system & gave a paralytic med instead. Normally it takes a second nurse or pharmacy staff to allow override. Not sure how she managed to do this....so sorry for all concerned. Glad no prison term is involved.

Our loved ones leave such sadness when they pass. Kudos to the nurse who reported what she had done immediately. And the family who forgave her leave us all humbled.

    The most frightening part about this case was that the hospital tried to cover it up, even when the nurse came forward with the mistake immediately. She was the one blamed for everything. The hospital faced no consequences (except a severe staffing shortage because they showed just how little they value their nurses)

      @Mom you really shouldn't just openly talk about suicide like this. It's really not something you should just throw around as a comment when something horrible happens in your life. I've lost someone incredibly dear to me through a suicide that I witnessed, 10 years ago. Hearing people say they'd " just kill themselves" if so and so ever happened. Just truly breaks my heart. It's honestly the worst pain you could possibly put your loved ones through. Noone of us will ever be the same. And the most f-ed up part is before he collapsed I seen the regret in his eyes. But it was already too late. Please reconsider what you say. The suicide rate is on the rise everywhere, among all ages. It's only going to get better if we do better together. I understand what happened to this lady was completely tragic. But that doesn't mean she should curse her family with the pain of her suicide on top of it. She owned up to it. The family forgave her. I'm sure her God understands. If she's a believer. And the judge understood too. If she would have killed herself what good would it have done? Just stopped her from feeling guilty? But meanwhile do what kind of harm to her loved ones? And sending what kind of a message to all those nurses? It would just feed the cycle of suicide. And make others feel like it's a justifiable thing to do.

      I find it hard to believe that 17 back to back errors is a one time thing. I think that hospital needs to be investigated. It should be clear to everyone that profit is more important than lives. Not just in hospitals, but in America as a whole.

      Just be extra careful when you override medications. There’s a lot of power in overriding orders. But nurses do it all the time to get around med dispensary machine safe guards since they’re over worked. Just be careful.

    This is exactly why I quit nursing school. My professor told me a story of a nurse who gave a patient the wrong dosage due to the doctor’s orders. She went to prison and ruined her life all for following orders. No thanks. Being a nurse is way too high risk with no reward

      My point is that Nurses could either trust the doctor enough for them not to question them or just simply be in a hurry
      Because I can't tell you how tired my mom always is when she gets back home late at night or sometimes even in the morning because she's too busy running around, getting stuff, checking on patients, and delivering what the doctor's need etc especially since the pandemic
      She's to busy working instead of checking the medication she brings to clients because she can't just waste time on checking each of the bottle/medications she brings.
      Hope you understand what I'm getting at here, I'm only in 6th grade, can't type well
      So I hope the nurse in the video really was just following the doctor's orders

      So im trying my very best to not catch it along with keeping my hands Clean and wearing a mask
      At school they said that we don't have to wear masks as much anymore but I'm not taking any chances!

    Being on strong pain meds for almost 20 years, the only time I overdosed was in the hospital because of a nurse. Also a pharmacist gave my pills to some lady and gave me her pills. I told her and she freaked out and called the lady. Luckily she didn't take my pills because it would have killed her. With certain jobs, you have to be very cautious and pay attention.

      hey I knew a situation at Vanderbilt that involved a chronically I'll 17 year old. she only had about a year to live anyway. a nurse gave her phenergran but did not mix it properly. it burned up her arm and they had to amputate. she lived her last year of life with one arm and multiple issues and surgeries.

    I’m a pharmacy technician and been working as one for over 10 years. There will forever be a human error factor so making sure you follow policy and procedures with no shortcuts is the only way to prevent human errors. I cannot stress how important that is as a health care worker.

Luckily the hospital I work for has a lot of policy in procedures in place to protect members and staff. Also the yearly training we do. I don’t take shortcuts even if it means the member waiting a bit longer.

      Give me a break. She was egregiously negligent. She ignored procedure and close to 20 warning signs that what she was doing could kill someone. The victim died a terrible death. She was fully cognizant, unable to speak or yell for help, while she slowly suffocated to death. This nurse deserves to be in prison.

She immediately recognized her error, and reported it, never even considered trying to hide it. So it was truly an accident, as tragic as it was. People make mistakes.

And this reporter is just choosing to ignore it. because the sentence the first time she apologized in 4 years will get a bigger reaction from viewers.
      There's also all the legal stuff, and it's a very common for the two sides to be banned from talking to each other. And only communicate through lawyers.

    A hospital dropped my 97 y.o. dad on his head and it killed him. My mother said to forgive and asked the hospital to have meetings to find a way to ensure that this never happens again. They did, and were very serious about it, giving my mother a copy of the minutes of each meeting. Accidents and mistakes happen even in the best of circumstances. Learn to forgive. It's hard sometimes but try.

    I would encourage everyone right now to read more into this case, this is WAY more than meets the eye. This nurse was essentially thrown under the bus by the hospital she was working at where this occurred. The hospital was already having flawed procedures and electronic communication issues where nurses were constantly having to override the system to get any medication for patients which played a big part in this mistake happening. Right after she gave the fatal dose, she realized it and from the beginning told the truth about what happened, but the hospital is the one that didn't tell. Seriously, look into all of the full story. I'm not a nurse, so nurses feel free to correct anything I might have said wrong.

    This is one of the reasons why I didn’t renew my license. Nurses have way to many heavy responsibilities. I cried when a seasoned nurse rushed me to administer my patients meds. It’s overwhelming

    This is terrifying. You trust your doctors and nurses, with your life. I think it’s a good idea for family to double check medications given to a loved one. No, we shouldn’t have to, but after this… I will be checking. I don’t think jail time was appropriate. This was a horrendous mistake, but she will have to live with it for the rest of her life. It’s sad all the way around.

This wasnt a simple mistake, this was negligence and it killed someone. There needs to be serious consequences for negligent actions that lead to death. Not a slap on the wrist.

    Analyzing her body language, it’s clear she’s not a bad person. She was honest to own up to her mistake as soon as she realized. I have so much respect for the family for giving her another chance. It’s painful to lose a family member, but they were also very compassionate and wise to not let anger and hatred get to them, knowing that this was all just a mistake. I hope the nurse and the family can move on with their lives happily!

      @@lala_k05 just because she reported it doesn't mean it was a mistake. The two medications were totally different. One was solid and had a warning label on it. The one she was supposed to use was liquid. There's no mistaking the difference between the 2 drugs. There were other things she did wrong afterwards too. She wouldn't let anyone in the room for a while after administering the fatal drug. I feel bad for the deceased patient. She was murdered.

    This is why I could never work in the medical field. I make those “tiny” mistakes more than I’d like to admit with small things, wrong spice, wrong label etc. Never on purpose, but still annoying when I do it obviously because it sets me back. The thought of accidentally grabbing the wrong medication to give to a person or animal terrifies me. I don’t think I’d be able to handle the guilt.

      She grabbed the wrong drug. She want told to give the patient this lethal drug, she gave it to the patient in error. Problem is, the lethal medicine came with a big red label located directly on top of bottle lid that stated how lethal the drug was. So this nurse not only grabbed the wrong drug, she somehow missed the huge warning label.

    This again shows that nursing isn‘t just cleaning up behind patients and putting on a few band-aids. Every healthcare professional including myself knows how just one error can impact others as drastically as this and it’s terrifying. It should also act as a reminder to all the CEOs, managers, Directors and so on how dangerous it is to have overworked and understaffed personnel. Glad the family was able to forgive her

    I’m in nursing school and this scenario terrifies me more than anything. I double and triple check everything even if it takes me longer than my classmates because hurting someone because of not doing so would devastate me. I wouldn’t be able to live with it. Like I said, it terrifies me. A human life is precious and needs to be safeguarded at all times.

    Props to the family of the deceased for showing forgiveness. I've been on the receiving end of a medical mistake made by a healthcare worker that could have ended in me bleeding out to death. I didn't make a scene because I know mistakes happen and it wasnt due to gross negligence. We all make mistakes. I make mistakes at my job but I also work with computers and files so my mistakes have far less dangerous consequences. I hope she comes to the point where she can forgive herself.

      @@thaloblue Sorta. It's a long story involving cancer treatment and a misplaced suparpubic catheter(namely the bulb that gets inflated). Basically, I didn't realize my bladder was filling up with blood until I felt a weird sensation by my pelvis. When I got up off the chair in my room, I saw a huge pool of blood on the floor. My shorts and pc seat were soaked. Went back to A&E and bled even more there. I felt mostly okay during the entire incident - just dealt with some dizziness and feeling light-headed. I didn't realize how dangerous the situation was until the two attending doctors told me.

    We need a nursing policy overhaul. All 50 states need to have a mandated PATIENT TO NURSE RATIO, so that 1 nurse isn’t taking care of 30 patients in a unit. Or an ED or ICU nurse aren’t overloaded with too many nurses. There needs to be MAJOR policy implementation with that.

      A nurse that isn't being overworked shouldn't feel the impact of a law like this, but a overworked nurse definitely will.

    The fact that she immediately admitted her mistake speaks volume for her character. Both my grandparents use visits from nurses for thier medications - one day we got a call that they had switched up their medicines. They had told thier agencies hours after the fact, but hadn't said anything to us while we were in the room with them just after they had done it. And we knew they realised the issue immediately because they were being weird and discussing something while still in the house. Thankfully everything was OK. But it could have been really bad as they had completely different medications and we were left with the decision of whether we needed to take them both to A&E to get thier stomachs pumped as the agency offered no advice

      I am an RN for 35 yrs and retired now. The general public doesn't realize the constant mental, physical and emotional demands placed on nurses. At some point, you get mentally and physically fatigued and make poor decisions. The results can be devastating, as in this case. This nurse did not intend to harm her patient. I really feel for her and for the patient and her family members. I'm glad I'm out now. It's a system failure.

    I was an elementary teacher for 30 years. If I made a mistake on a child’s test, I could easily correct it and maybe give the child a sticker for pointing it out. I was forever grateful that a teacher’s error wasn’t lethal. My heart goes out to the doctors and nurses who make a mistake and it has devasting consequences.

      Yeah that's true, but people tend to give leniency when one is remorseful. I am surprised at the amount of people who just assume law is enforced in a binary nature, that's not how the system works, nor should it be.
      If we wanted to enforce things like that, we'd get rid of the idea of "manslaughter" and just charge everyone who causes another to die with murder. But no, that doesn't make sense. The goal is not to punish people, it's to keep dangerous people away from society, which means locking them up or issuing them a death sentence. This kills two birds with one stone, as it deters people from committing crimes as well.
      Should we really lock a nurse up for making an honest mistake? She should have her license revoked and kept out of medical practice, but she shouldn't be treated as a danger to society.

    She knows she is responsible for the death of someone's family member. This will most likely follow her for her entire life. I feel so bad for her, she never wanted any of this to happen, yet her life has been forever changed. I'm so sorry for the family and for the nurse. No one should have to go through any of this.

      @@RougeIncognito She wasn't paying attention, unfortunately, as she was training someone (not an excuse). Vanderbilt has a terrible system that requires a 2 letter override, (as many hospitals unfortunately do), used on a regular basis. The meds have the same first 2 letters for the override, as do many other meds. The meds in the computerized cabinet all have warning labels. Vanderbilt tried to cover it up when she reported the error. Was it a colossal mistake? Absolutely. But, the hospital trying to cover it up after she reported it?! Then placing all the blame on her when they got caught, that's criminal.

    Working in the medical field for almost 20 years, I can say for a fact that reporting one’s error is not something you see frequently. Cover up is almost always the 1st line of action. Hats off to her for coming clean and respect to the families involved for showing forgiveness.

      I've heard about doctors and nurses covering up things like this but I don't understand the pat on the backs for her doing what she supposed to do. If someone stays on the scene of a accident, we don't give kudos to them. It's what your supposed to do.

      I worked labour and delivery for 10 years in the UK. I’m glad to report ‘cover up was never the first line of defence’ I saw some pretty major life threatening ‘mistakes’ from midwives and Dr’s. There was no hesitation in reporting it and a whole team would be on it. They have a culture that encourages honesty in reporting mistakes, and it works well in saving lives.

      @@Venus20000 that's because in the US everyone is litigation-happy and VERY quick to point the finger and place blame and want "justice" for literally everything. And not just on the individual but as a healthcare worker in the UK I'm sure you have a very good understanding of how screwed up our Healthcare system is in the US. All of the huge fees and insurnace investigations and criminal and board of health investigations that would happen to the medical facility it occurred in. Everyone would be ruined so I think they avoid the consequences. Not on a moral basis. I got my BSN and became and RN and then I switched to medical administration with a concentration in medical ethics and for a while I did sensitivity training for medical personnel. It hardened my soul to the core and I HAD to leave the medical field completely. I had to start my education and career completely over at 32 years old with kids because I just couldn't be a part of the horrific system I saw play put every day. I'm a preschool teacher now and make a quarter of what I did before but I'm much happier. I am envious of a happy labor and delivery nursing team, I am grateful for them but I am only envious because I wish that's what we had more of here. I'm very grateful for medical personnel like yourself who hold yourself and others around you accountable. It shouldn't be a lot to ask but it isn't the status quo in a lot of places. 💜

    As far as I've read. She reported her accident immediately when it happened and she didn't try to run or hide from the problem. The family forgave her. And most importantly while covid is still running rampant I'm sure this poor nurse is working 70+ hours a week and is dead ass tired with minimal help. Save our medical division America.     She made more then 10 procedural errors before she finally got her hands on the lethal medicine that clearly stated "warning paralizing agent" I'm no nurse but I think after seeing that warning I'd step back rethink what I'm doing ... no matter how busy or tired I am.

      Well in all fairness it was supposed to be a sedative for surgery so being labeled paralyzing agent is not outside the realm of possibility in my opinion.

    My sister’s doctor gave her penicillin back in 1968. It was an emergency, and he did not know that she was allergic. He became my family’s doctor after the older doctor (who made house calls) died. My sister had a bad reaction that sent her into coma. I was angry of course, but I eventually forgave him, as did our family. He apologized and I guess he expected us to sue, but he had did many charitable things for our community and was a good person. To take his license would have meant bad healthcare outcomes for poorer patients that he served. This was a mistake, a tragic one.

      Extremely different circumstances here. The medication was ordered and had a specific name, and this nurse negligently just didn’t read the label OR the warnings. Your sister could’ve died without intervention regardless (if it were truly an emergency as you’ve stated) rush decisions happen in emergency situations. Nothing about this was rushed other than that nurse not doing her job right 🤷🏼‍♀️ doesn’t really compare AT ALL.

    I wish there would have been a sentence or two on the liability of the hospital. Over working nurses makes these mistakes inevitable. When we take care of our nurses and pay them well enough to take days off when needed and staff their units well enough, these mistakes drop drastically.

      Sadly, covid 19 causes alot of hospital to overwork everyone and not just nurses. Sometime you just don't have a choice. My friend is a nurse and when covid 19 newly started she had to work almost everyday for the first 5 months nonstop since her colleagues were getting infected left and right.

I understand forgiveness, but what if this patient was 25 years old? I think saying she did nothing wrong is a bit scary. You have to hold healthcare providers to a high standard.

      @@F34RGLS You realize her attorneys would most likely advise against an early apology as it is turned into an admission of guilt. And these matters, obviously, can drag out for years which sucks for everybody.

    Same happened with me. My doctor prescribed me what I was allergic to and a higher dose. When to get the prescription filled and the lady behind the counter asked what I was allergic to. I told her and she done a slow turn of her head looking at me, then pointed to the drug and told me it was what I was allergic to and the strongest does they had.
    He would have sent me to the ER that week. The lady saved my life.

      A similar thing happened to me just before cancer surgery. The pharmacist pointed it out. I questioned my surgeon and he just dismissed my concerns. I fired that surgeon from the remaining surgeries. Such an AH. I have obviously taken a much more proactive role in my own healthcare and will no longer be that deer in the headlights that takes everything at face value.

    My daughter is a very attentive, loving nurse. And this would be my worst nightmare for her. Sincere condolences to this patient's family .

      I was in a clinic once. Never once considered the nurses could potentially inject the wrong thing on me.
      Now I'm afraid.

    New procedures including what I would suggest triple check policy should be considered for administering any type of medication to a patient. Even having a second nurse double-check before giving it to a patient
