You wouldn’t use exposure therapy with Vertigo, anyway; it isn’t a fear and isn’t treated like that. It’s caused by an inner ear issue, an injury to the head, ears or neck, or an issue in the brain. It’s treated with certain exercises, getting more sleep, getting proper hydration, using stress relief techniques, and sometimes with medication. Symptoms include swaying, dizziness, nausea, vomiting, tilting to one side, headache, ringing ears, etc, and in extreme cases, fainting.
I think what they mean is in how, while exposure therapy helps some people, it doesn’t help everyone, and only works under certain controlled conditions. It usually takes many sessions, and in the case of strong phobias, should only be done with the help of a trained doctor (psychologist, etc). If done improperly, it can reinforce or worsen that phobia. I think you might be thinking of Acrophobia instead, the fear of heights, or Basiphobia, the fear of falling.

his fear of thunder has been confirmed to be because he had to live outside without a family and when thunder striked he had to endure it outside and couldnt sleep so its a type of trauma

Funny how Sonic is a lot less afraid of water than he used to be. May I present Exhibit A: Sonic Colors. There is a water level where he can run on top of the water, as well as use the Yellow Wisp's Drill Power to properly swim through the water like a submarine. He's not afraid of the water anymore.

The thing is that Sonic isn't actually afraid of water. He's just afraid of drowning. Swimming lessons have never really cut it for him, apparently, but otherwise there's no reason he doesn't swim

Exposure therapy is done in baby steps, not full immersion immediately. If the goal is to introduce Sonic to water, he needs a walk-in pool that he can choose his comfort level with, not getting pushed into a in-ground pool that comes to chest height. If the goal is getting Tails used to Thunderstorms, you DON'T tie him to the roof with an umbrella right next to the house's lightning rod. You start with just playing the sounds of a thunderstorm at low volume, like the storm is far away, and slowly increase the volume to bring the "storm" closer until it reaches a point of mild discomfort, and keeping it there for the remainder of the session so there can be a clear lack of harm and a long enough period to adjust. As he gets more comfortable with the sounds, you add in other elements, such as low light, vibrations from the thunder, or wind and rain.
Regardless of who's getting treated for what fear, you never toss them in the deep end first, because that's only going to cause panic and create further trauma. And it will always take many long treatments to get that fear to something reasonable and manageable.
