This is so helpful. As a very active middle aged adult, being diagnosed with PTTD seemed like a death sentence. This video helped me realize that one can live with it with some reasonable modifications.
I just want to say that this injury started when I was a teenager and I'd been compensating for it for over 20 years while doing extreme physical activity in my personal life and in the military. It wasn't until I started practicing martial arts, meditation/mindfulness and breathwork did I have the awareness to realize that this was even an issue for me! Turns out, it's HUGE issue and this discovery has been a game changer. Thanks for this video!

@@kentompkins7957 I did the exercises for a while and it improved, but it would still hurt in the morning. I saw an osteopath (in Spain, they are different from the ones I see in the US) and he recommended an anti-inflammatory diet. It had been 1.5 years, and I was still in pain in the morning. I did it for two months, together with a strong dose of vitamin D (5000IBU daily) and omega 3. It worked! I eat very healthy, but he additionally asked me to stop gluten and cow dairy (sheep, goat or A2 cow dairy was allowed and made it tolerable for me). His reason was that my body was not restoring/healing properly, and inflammation could be the culprit. Good luck!
superb video. I have been trying to treat this since november and still suffering. I was referred to NHS (UK) physio and was told to immediately stop running and was given some very aggressive stretching exercises. I felt like it was recovering but seemed to keep 'flaring' again. My ankle feels very stiff and I struggle to walk after being in bed. Interestingly I spend a lot of time standing with my job and this is ringing a bell in my head from what you've said. Thank you so much for this, it's so good to hear from a runners perspective.
I currently have the exact same problem. so I tried stretching it helped a bit but the flaring up keeps coming back. this video helped in a sense that now I literally know my mistakes and can get on to fixing it.

This is the best physio channel, bar none. I've just returned to this video after about three years because of a recurrence of tib post issues. This time it's pretty clear to me that downhill running caused it. I recently started running my home loop the other way around, which makes it predominantly downhill, and have been running it more often than I used to because it's so pleasant this way. Apparently my tendons disagree and are demanding more time to adjust to the unaccustomed load. Thanks Maryke for putting this and other fantastic resources online!
@@joel553009 I have completely stopped running and am cycling a ton, it's quite fun. I figure it's not tendinitis and actually stress fractures, in both legs. I came back to running, when not fully healed, did a total of maybe 150 miles over 2 months and the pain returned. It's been really depressing and stressful dealing with it but I've finally accepted that it's gonna take as long as it takes. I won't run at all until there has been no tenderness. I'm making the most of cycling and the gym for now. Maybe yours is fractures too, otherwise it might have healed by now.

What a great video on this, thanks so much for this.Your mention of different terrain, camber and hills is spot on. I find running XC and up a slight incline is much better for me and less harsh than the road. I have also stuck in a superfeet insole and what a difference. At 1st when walking I hated the feeling of it pushing on the arch, but once i started running i felt so much more secure and i ran 6 miles without any pain. By the end of the run i did not even notice the insole. So anyone watching and suffering with this get an insole in your shoe as it def helps. Thanks again.
She gave more thorough explanation of my injury then my podiatrist did. I had to ask him to show me what was going on in my foot and he draws a little picture of it. He also charged me $259! 😡 Grateful for this video. Thank You!!!!

You’ve just ticked off 3 things that no one really picked up. Previously broken tib/fib, your point number 6 I think is a major one. Just did a week of walking, followed by riding. That right leg has never been as strong as the left and it’s all coming together quickly now! Thankyou for your info!
Thank God a found this video. I have this problem for over 8 months. I got 3 cortisone shot one month apart from each one. I’m so depressed because I work on my feet over 8 hours at day. I start thinking on quit my job, because is real painful, to hold that pain all day long. I going to follow all your advice. Thank you ❤

I was diagnosed with severe Posterior Tibialis Tenosynovitis in my right ankle (through an ultrasound). I think it developed as an overuse injury from using my sewing machine pedal. I was recently retired and doing a lot of quilting. It was excruciatingly painful when I got up in the morning. It was also painful in bed when lying on my left side and my inner right foot wasn’t supported and was subjected to gravity. Eight months later I’d say my symptoms and pain are 50% reduced through rest, ie abstained from sewing for a couple of months and now limit time sewing, lying on my right side in bed and wearing good shoes from the moment I get up in the morning. Just mentioning as it can be caused by non exercise activities. Thanks for a very informative video.
I find this video very helpful and for me, it's the gold standard. You have a very good way of explaining things which makes things easy to understand. As I write, I have ice on my posterior tibial tendon which has flared up after being on my feet a lot yesterday. Thanks for your help.
So pleased I found this video. Spent over 300e plus more on pain killers. Finally after over a year with pain on the left foot, and now last few weeks on my right foot. more information in this video than any osteopath, podiatrist, MRI scan, x rays, GP in the past. I am a Concrete Mixer Truck Driver and using the clutch as much as I have to to deliver the product at a rate that the contractors can use and be happy with is causing a lot of PTTD in my left foot. I will try some of what you suggest and I have an appointment with my Podiatrist. I have very flat feet. Thanks again. At least now I understand what is happening!!!

@sportsinjuryphysio Thankyou so much for this video. I injured my ankle badly while running, a few weeks ago. I am not able to walk with it and the pain is just like in this video. The causes too. Ground shapes, shoes, faulty form.. The nurse just called and said it is not guaranteed will my foot ever be painless or better again. The doctor was very unprofessional and would have sent me home without any treatment or help. I basically made him sent me to MRI. So there's a tear, small cartilage damage and tenosynovitis. I have been left with only these pieces of knowledge. As I said, the pain in my ankle is just like you describe in this video and I am so grateful of everything you have just told me! I feel kind of hopeless, but try to start some kind of self learned rehab for it.
Very informative. Nurse that walks 12 hours! Side of foot hurts exactly where you pointed out. Have had an ankle injury where the right ankle turned inward. I have wide feet and I tend to take the insoles out because it makes my foot feel like I have a little more space in width. May have to do with the fact that I also have a high instep. Anyway..wonderful video! You obviously understand and like what you do! Wish there were more like you!

I was wondering would this also be the right protocol when it has been a chronic problems for years? It is so difficult finding a professional sports physical therapist who deals with all this. Excellent video.
The treatment for this when it has been present for years usually consists of: Getting the correct shoes + insoles and doing strength training exercises to restore the tendon's strength.
I have had an injury exactly 3 months ago. Went from not being able to walk to now being able to walk with a limp. I will give these exercises a go and see if they flare up in the morning. Thank you for the informative video! :)

@@juliuspope6271 Very well! I have been able to walk with little to no limp and even went of a very short jog! I made modifications to these exercises because over time they flare up my ankle, but after these modifications I do these exercises about 3 ish times per week. My recovery is going very well thank you for asking! :)
What you are referring to is for petallar tendinopathy pain management - so far, nobody has been able to get the same results for other tendons, so it is not something that is necessarily applicable to all tendons and also not something that has super strong evidence behind it. Also, that dose was recommended pain management - what I use it for here is to improve load tolerance. In my experience, if you jump in with 45 seconds when you have Tib Post tendinopathy, you are likely to flare it up since the tib post seems to be a lot more irritable than other tendons. I find my patients do better with very short holds to start with and I build it up to different levels depending on the patient and their tolerance. But again, none of this has been definitively proven by the research - it is just what I find works for me.
@@SportsInjuryPhysio this could partially explain why my tibialis posterior is so irritated at the moment. I got to be more careful about information I find on the internet it looks like

Indeed I really enjoyed your presentation but my concern you have solved half of the issues I have the cause or genesis of my problem results from back and pelvic girdle pain
Honestly after seeing Dr and prescribed PT I felt worse than ever. I put PT on pause and glad I did. All the aggressive things that she warned against is what they were doing. Stretching excessively I think was the main culprit. Consequently my physical therapy would actually cause more inflammation and then I would recuperate over a couple of days and go back for another session and then inflame It again. I will now take it slow. Grateful for this video.
Very informative. After 3 months of running after Easter I stopped completely. Can’t even stand. Podiatrist said your X-rays are best I have seen for the day and ball of foot and all looks fine. But I kept telling him pain is radiating from bottom of foot under the arch towards my talus and tibia. Said arch support insoles should help and gave me one. But even with that after 4 weeks of rest I tried to go for a brisk walk. After 2 miles I started getting pain and next 3 days I can’t stand. Then again same way rested 4 days and tried again. Same pain. Will it be the same condition as explained in this video. Please advise and help. I have already spent 300$ for podiatrist and the arch support insoles.
I appreciate that you mentioned some concerns about calf stretching because I tried to tell my doctor that stretching is not really working for me, in addition I always feel the stretch in my left calf and NEVER in my right calf which is the one that is worse off. I have had my issue for a while, went to PT, etc, it got better and now returned out of no where. I’m so frustrated and dont want to keep going to back these doctors because they are all telling me the same thing and prescribing the same treatment which is the definition of insanity! Sorry for so much complaining I am just frustrated and tired of wasting my time and money only to still have the issue.
Brilliant video. I was captivated from the start. Mine is an accidental injury. Had to run after my dog after a thunder clap scared him. I am not a runner so running in full water proofs and mountain boots caused me major problems with my feet. Thank you for this help and guidance. The rehab information has been particularly helpful as I got a diagnosis from a friend who is a podiatrist who helped me greatly but this video has been helpful from beginning to end. Thank you. 😊

Excellent video! One question I have that was not addressed is if ice and ibuprofen is good for initial reduction of inflammation and symptoms. Thanks!
If you only have the tendon being painful and flared up, then ibuprofen is usually not advised because the inflammatory process is useful for healing. However, if you also have inflammation of the sheath (tenosynovitis) then anti-inflammatories can be very useful and help prevent it getting adhesions. You can usually suspect that you have tenosynovitis if your also have quite a lot of swelling in the area. If it is just the tendon, then you usually don't really see swelling. Same goes for ice but if your tendon is super painful, then I would consider doing ice as it is not as long-acting as ibuprofen - but only for short periods e.g. 7 to 10 min at a time.

@@joel553009 my advice would be, don't run until your legs are 100% healthy, then ease into it in a similar way to 'couch to 5k'

Voltaren twice daily and KT taping has really helped my pain levels with PTT.

@sportsinjuryphysio Thankyou so much for this video. I injured my ankle badly while running, a few weeks ago. I am not able to walk with it and the pain is just like in this video. The causes too. Ground shapes, shoes, faulty form.. The nurse just called and said it is not guaranteed will my foot ever be painless or better again. The doctor was very unprofessional and would have sent me home without any treatment or help. I basically made him sent me to MRI. So there's a tear, small cartilage damage and tenosynovitis. I have been left with only these pieces of knowledge. As I said, the pain in my ankle is just like you describe in this video and I am so grateful of everything you have just told me! I feel kind of hopeless, but try to start some kind of self learned rehab for it.

Very informative. Nurse that walks 12 hours! Side of foot hurts exactly where you pointed out. Have had an ankle injury where the right ankle turned inward. I have wide feet and I tend to take the insoles out because it makes my foot feel like I have a little more space in width. May have to do with the fact that I also have a high instep. Anyway..wonderful video! You obviously understand and like what you do! Wish there were more like you!

I was wondering would this also be the right protocol when it has been a chronic problems for years? It is so difficult finding a professional sports physical therapist who deals with all this.

The treatment for this when it has been present for years usually consists of: Getting the correct shoes + insoles and doing strength training exercises to restore the tendon's strength.
I have had an injury exactly 3 months ago. Went from not being able to walk to now being able to walk with a limp. I will give these exercises a go and see if they flare up in the morning. Thank you for the informative video! :)

@@juliuspope6271 Very well! I have been able to walk with little to no limp and even went of a very short jog! I made modifications to these exercises because over time they flare up my ankle, but after these modifications I do these exercises about 3 ish times per week. My recovery is going very well thank you for asking! :)

What you are referring to is for petallar tendinopathy pain management - so far, nobody has been able to get the same results for other tendons, so it is not something that is necessarily applicable to all tendons and also not something that has super strong evidence behind it. Also, that dose was recommended pain management - what I use it for here is to improve load tolerance. In my experience, if you jump in with 45 seconds when you have Tib Post tendinopathy, you are likely to flare it up since the tib post seems to be a lot more irritable than other tendons. I find my patients do better with very short holds to start with and I build it up to different levels depending on the patient and their tolerance. But again, none of this has been definitively proven by the research - it is just what I find works for me.
@@SportsInjuryPhysio this could partially explain why my tibialis posterior is so irritated at the moment. I got to be more careful about information I find on the internet it looks like

Indeed I really enjoyed your presentation but my concern you have solved half of the issues I have the cause or genesis of my problem results from back and pelvic girdle pain
Honestly after seeing Dr and prescribed PT I felt worse than ever. I put PT on pause and glad I did. All the aggressive things that she warned against is what they were doing. Stretching excessively I think was the main culprit. Consequently my physical therapy would actually cause more inflammation and then I would recuperate over a couple of days and go back for another session and then inflame It again. I will now take it slow. Grateful for this video.

Very informative. After 3 months of running after Easter I stopped completely. Can’t even stand. Podiatrist said your X-rays are best I have seen for the day and ball of foot and all looks fine. But I kept telling him pain is radiating from bottom of foot under the arch towards my talus and tibia. Said arch support insoles should help and gave me one.
But even with that after 4 weeks of rest I tried to go for a brisk walk. After 2 miles I started getting pain and next 3 days I can’t stand. Then again same way rested 4 days and tried again. Same pain. Will it be the same condition as explained in this video. Please advise and help. I have already spent 300$ for podiatrist and the arch support insoles.

I appreciate that you mentioned some concerns about calf stretching because I tried to tell my doctor that stretching is not really working for me, in addition I always feel the stretch in my left calf and NEVER in my right calf which is the one that is worse off. I have had my issue for a while, went to PT, etc, it got better and now returned out of no where. I’m so frustrated and dont want to keep going to back these doctors because they are all telling me the same thing and prescribing the same treatment which is the definition of insanity! Sorry for so much complaining I am just frustrated and tired of wasting my time and money only to still have the issue.

Brilliant video. I was captivated from the start. Mine is an accidental injury. Had to run after my dog after a thunder clap scared him. I am not a runner so running in full water proofs and mountain boots caused me major problems with my feet. Thank you for this help and guidance. The rehab information has been particularly helpful as I got a diagnosis from a friend who is a podiatrist who helped me greatly but this video has been helpful from beginning to end. Thank you. 😊

Excellent video! One question I have that was not addressed is if ice and ibuprofen is good for initial reduction of inflammation and symptoms. Thanks!
If you only have the tendon being painful and flared up, then ibuprofen is usually not advised because the inflammatory process is useful for healing. However, if you also have inflammation of the sheath (tenosynovitis) then anti-inflammatories can be very useful and help prevent it getting adhesions. You can usually suspect that you have tenosynovitis if your also have quite a lot of swelling in the area. If it is just the tendon, then you usually don't really see swelling. Same goes for ice but if your tendon is super painful, then I would consider doing ice as it is not as long-acting as ibuprofen - but only for short periods e.g. 7 to 10 min at a time.
