I strongly suspect the reason she was left unattended in hospital for hours had to do with her being African-American. Some of her pain was due to her own poor decisions: relationships with abusive men (is this song autobiographical) and wasting her money and, eventually, her life, on heroin. Then there was the sexual abuse in childhood and the lack of a solid family structure with its accompanying poverty. Very sad.
