How to use custom fit tooth whitening trays.

  • Dr. Rich shows you how to use custom whitening trays. Whitening trays are also known as bleaching trays and are used to whiten teeth at home. Dr. Rich uses Opalescence whitening gel by Ultradent in the video but the technique will work for other brands of tooth bleaching gel as well. He shows how to load bleaching gel into the custom bleaching trays, how to insert the trays into your mouth, and how to clean the whitening trays after you are done using them.

    I have the trays with the custom dots inside them. For cleaning, I have success with my sink hose while it's in 'shower mode' or whatever it's called. The needles spray. It gets all of the gel out quickly!

    Doc, thank you for the video. After having the trays and gel sitting in my fridge for over a year I'm finally doing the bleaching. I was honestly just so happy with the results of my braces that I feel I didn't need the extra bleaching.       LMAO. I used to use Crest Whitening Strips 2-3 times per week. I got lazy after purchasing my kit because the strips are so convenient.
      We must be twins because I paid for the tray and gel kit from my dentist last year. I have NOT used the kit in a year. I just started using it today after asking the dental tech a few questions on Friday.

    My “polyday” gel is from “2018-01”. Yes, I got it four years ago, and just procrastinated! My teeth have never been such a shade of yellow, and I’m really eager to whiten up! I don’t drink coffee, and barely any wine or tea… it’s just time.
    Just tried the first tube today. It comes out as a gel, not as a liquid. I have kept it in the fridge, I am hopeful that it will still work to some degree.
    The lower tray fits fine, the upper one, not as great. The upper one also causes me to have it get a mouthful of saliva, as well, so I may have to get a re-fitting 😞 to be able to tolerate it.
    The tubes say “9.5% hydrogen peroxide gel”. The dental assistant said to use it for *20 minutes*, which is markedly different from the amount of time you’re recommending. I’m going to call them to check if I mis-heard, or if they have another reason for that amount of time.

      You need less wear time with hydrogen peroxide gels. I use carbamide peroxide gels which need longer wear times. Thank you very much for watching!

      Hi, I haven't used Kor whitening but I have heard of it. Usually whitening is a function of the strength of the whitening gel and how long you use (both the amount of time it stays on the tooth, and the number of days you wear the trays).

    Thanks for this video! When I went to pick up my trays they handed me a bag, said to read the instructions then almost pushed me out the door saying BYE! Got home, read the instructions and the very first item on the list...load the trays as explained by your dentist. It wasn't even a dentist that handed me the bag, just an assistant. eye roll....

      You can use them if they fit properly. I would recommend a set of whitening trays just to be safe.

    Love the video! Question: I noticed that I am salivating a lot, not sure if it is safe for me to swallow, right now my solution is to keep spitting in a cup. I am using the Philips Zoom 16%.

      Anytime you put something in your mouth, your mouth thinks it's food and will start salivating more. You can swallow your saliva safely. As long as your whitening trays fit your teeth well, you won't be swallowing much of the whitening gel.

    Thank you! I got zero instruction from my dentist or the assistant who took my impressions. The front desk receptionist just handed them to me, told me to try them in right then and there to find out if they fit, but I had to ask questions about what and how to use the bleach and the desenstizing gel they provided too. It was awkward. Good thing she went back and asked how to use it, or I'd have used it wrong because she told me wrong.This doctor here gave great, plain instructions, and I'm grateful I found this video.

      Hi, you don't need to spread the gel. There is very little space under the tray and the gel will spread by itself.

      Hi, yes if the retainers are made correctly to allow for whitening. The retainer needs to cover the teeth completely but not cover the gums on the cheek side of the teeth.

      Hi, that's too long a time. Ultradent recommends 2-4 hours for 20%. I'll send you a table of recommended wear times for different strengths of whitening gel.

      Hi, the whiteness is cause by the gel getting on your gum tissue. Make sure your trays fit properly and use a little less gel.

      Hi, I use the line method personally. The trays have a tight fit so the gel will spread out by itself.

    Great video! I have a question though, my dentist told me not to brush my teeth immediately after removing the trays because it would remove the dental enamel which is softened due to the bleach. However in your video you recommend brushing your teeth after removing the trays. Have you ever experienced problems with your dental enamel?

      Hi, the gel does not soften the enamel on your teeth. I've been using it for over 20 years and haven't had any issues.

      Hi, the whitening trays should be trimmed so that they don't cover your gums. The trays will keep the gel on the teeth and your gums will be fine.

      You're welcome! The recommended wear time for the 15% concentration is 4-6 hours. If your teeth are not too sensitive, you can try overnight.

    Hey Doc, first, thanks a million for the video an clear explanation! I just started my whitening process with philips zoom 16%. I applied a drop on every teeth but I can barely see the gel after all the salivation that is going on in my mouth 😅. Is that normal or if it keeps happening every day it will reduce the effectivity of the treatment?

      Do the trays fit properly? You will be able to taste the gel while you are wearing the trays. The trays should fit onto your teeth well.

      Hi, I usually tell my patients to use the gel for 2 weeks initially. Most are very happy with the results. Some need to whiten for a week or 2 longer. After the initial whitening, using the trays one day a month is enough to maintain the effect for most people. People who drink a lot of coffee, tea, or red wine may need to do it more frequently.     Thank you for this video, i have a question i cant find a clear answer to..
    I understand that after an in office whitening we have to abstain from certain types of food and follow the white diet very strictly for the first 24 to 72 hours.
    That being said, when it comes to take-home trays using carbamide peroxide, do we have to be as strict knowing that the treatment lasts for 2 weeks and more.. ?
    if yes, is it just for the few hours after taking off the trays or on all time until we finish the treatment ?

    I did this with 20% opalescence one time for 3 hours, and my teeth did whiten but it also worsened the spots around my teeth from when I had braces. Is there a fix for that?

      Hi, it depends on the severity of the white spots (dentist called that demineralization). Sometimes the whitening will help the spots blend in, sometimes you need bonding to cover up the spots.       Hi, studies show that the blue light doesn't do anything, the whitening effect is due to the strength of the whitening gel you use, and the length of time it's on the teeth.

      Hi. With in office bleaching using a blue light, your teeth will dehydrate just from your mouth being open for the length of the appointment. The dehydration makes your teeth look whiter, but then they will appear to get darker again in a couple of days as your teeth rehydrate. Whitening from the gel will last longer since it actually whitens your teeth and doesn't dehydrate them. I hope that makes sense.

    Hi, i have the 20% gel and my doc recommended i use it for 4 hours and brush my teeth with sensodyne before and after. I got it 2 weeks ago and travelled to a new country 22 hours away… I haven’t used it yet but can I start the treatment now? The country i am in is pretty hot I must say… is the gel usable at this point?

      @@Yowzoe Carbonated beverages tend to be acidic, which can soften your teeth. Waiting a half hour before brushing gives your teeth a chance to remineralize, though I don't know if it's clinically relevant.

      Hi, as Rowan While mentioned how long you wear the trays depends on the strength of the whitening gel that you are using. The stronger the gel, the less time you wear the trays. I usually offer either 35% Opalescence which you wear for a half hour once a day, or the 10% version which you wear overnight. If people have sensitive teeth I will recommend the 10% version.

    I've been using 2 entire siringes of Opalescence 16% PF and havent seen much improvements, i saw that my trays arent really firm and saliva enters it and im afraid thats the issue, should I remake my trays?

      Hi, how long do you wear the trays every day? The recommended time for 16% gel is 4 to 6 hours. 2 syringes of gel, used for both top and bottom trays is only 4 to 6 days of wear, it takes longer than that, usually a couple of weeks. Your trays should have a nice tight fit. If saliva can get in easily then the gel can come out easily.

    Hello! Thank you for your video, this is very helpful! I am a bit late but I have two questions if you have time time.
    Firstly, I’m using 10% gel and using the treys overnight. I got the 10% because my teeth are really sensitive. I have noticed that my teeth are still in a little bit of pain in the morning. Is that just something I have to take until the treatment is over?
    Lastly, I have noticed a few white spots around my teeth but no change on the rest after my first application. Is that normal? Did I not spread the gel correctly?

      Hi, yes sensitivity is a side effect of whitening. It should go away a day or two after your finish whitening. It's always a good idea to get checked by a dentist to make sure you don't have any cavities before you start whitening. Have your dentist take a look at the white spots just to be safe, but usually they will blend in with the rest of the tooth. We're they noticeable before you started whitening?

      @@VeryNiceSmileDental I don’t have cavities but the top of my bottom teeth are a bit worn out because of grinding in my sleep (a very stressful period back in uni) that is where I feel the most pain.
      As for the white spots, they were quite noticeable before the whitening but my dentist didn’t say anything about them.

      Hi, it may take longer when you use the 10% gel since it is the weakest gel. Teeth don't have to be dry when put the trays on them. Don't eat or drink while you are wearing them.

      Hi, I usually recommend getting custom whitening trays made in order to keep the gel away from your gums. Majority of the orthodontic retainers I've seen tend to cover the gums somewhat. As the gel expires, it may not work as well. It will still work as long as it doesn't separate.

      6% hydrogen peroxide is equivalent to about 20% carbamide peroxide. I have used a 9.5% hydrogen peroxide gel in the past. It should work for you, it may just take a few more days of using it.

      No. I mentioned that after I remove the trays from my mouth I will brush my teeth without toothpaste to get the remaining gel off. My usual morning routine is to drink coffee, eat breakfast, brush my teeth with toothpaste, use the whitening trays, then brush any remaining gel off with a wet toothbrush (no toothpaste). I hope that makes sense.

    Hey doc! My dentist recommended that I store my supply in my fridge. I’m moving to another country and trying to figure out what the best way is to deal with them now that they’ve been refrigerated. Will removing them from cold storage and subsequently refrigerating them 24 hours later degrade or compromise the chemical integrity or introduce safety issues? Any other gotchas to be aware of regarding the refrigeration method and temperature changes?

I assume the refrigeration preserves, but it’s not like we’re dealing with fish whereby we can introduce potentially dangerous chemical reactions. I also know that certain medications require refrigeration, so I wasn’t sure if somehow refrigeration dictates the necessity to follow a separate protocol.       Gotcha. I don't think you will have a problem. Most whitening gels are not refrigerated during shipping. I store in the a fridge at the office, but at home I just store in my cabinet and I haven't noticed any issues over the years.

      It stores longer in the fridge. Shelf life is 1 year room temperature. 2 Years in the fridge I believe.

      Hi, I usually tell patients to brush with sensodyne toothpaste for a couple of weeks before using the whitening trays. The sensitivity will go away once you are finished with the whitening process.
