    Hello sir. I wanted to thank your for your videos. I have landed an MTI in general medicine. A lot of information i got was from you. So thanks a lot
    I wanted to ask one thing though. I want to apply for IMT after signing after working for a few months. Would it be possible to leave MTI midway for IMT.
    And second . If i get alternate competency forms signed after MtI of 2 years,can i apply for group 1 specialities

      Congratulations on your posting.
      You can leave MTI midway by giving an appropriate notice period in your job. Usually it's 3 months.
      Yes, you can apply for ST4 group 1 specialty. But IMT will make the process smoother and hassle free.

      Roughly 3-6 months to complete the application, interview and other formalities for non training jobs. Being a fellow of Royal college is fine but they must have worked in the NHS in the last 5 years. Make sure your supervisor satisfies all the criteria that are mentioned in the alternative form.

    Sir.. how long will it usually take for a UK consultant to sign a core competency form being a non trainee in uk after completing MD RESIDENCY in india for three years and clearing the MRCP exam… i know its minimum three months sir.. but on an average how long it takes to complete the core competency form signed

    Hi doctor , thank so much. I have a query. I am a Board-certified General Medicine Specialist in Iraq. I have full MRCP and I managed to secure a specialist post in acute medicine in the UK. Do I need the CREST form or the Alternative form or both to be signed off in order to enter higher speciality training ST4 level? and is it possible that the alternative form signed in my country like the CREST form?

      You need only the alternative form, not CREST form. It can be signed in Iraq by someone who satisfies all the criteria.

    I want to do my MRCP after my internship, if I get it and get GMC registration, can I skip IMT if I work for 6 month in service job uk to fill the Alternative core competencies certificate? I thought if I finish MRCP I can immediately work at ST3 level

    Hello Sir, is it possible to achieve all the competencies as a non training IMG SHO?
    Secondly, once I have this form signed do I have to be in a registrar role to apply for ST4?
    Thirdly, may you please advise on which trusts in the UK support jr doctors using this route to enter higher speciality?

      This is how every IMG is getting into training posts so certainly it's possible. You have to be in a registrar post for getting the competency signed. There are hundreds of Trusts in the UK and I only know a few of them. It depends on your personal circumstances if it will work for you or not.

    I had completed 2 years medical training and then neurology board, can I sign these in my home country? Should I create E-profilo in my home country or once I'm in uk?

      You can get it signed in your home country provided there is someone who can fulfill the criteria to sign the form. You won't need a portfolio for neurology. You can sign up to NHS e portfolio once you are in the UK

      MRCP+ alternative core competency form will make you eligible for application for ST4 Renal medicine.

    Sir, how long would it take to get the competency certificate if I get a non training job at the UK after completing both steps of MRCP?
    Would it take more than a year?
    And is it possible to get alternative certificate signed by a non GMC registered consultant in India?
    Which of the above routes is better, according to you?

      3 months at least provided your supervisor is willing to sign the form.If you find a consultant In India who satisfies all the criteria as per JRCPTB, then you can get it signed in India.

    1. Hi sir. I am about to start my DNB Medicine in India. I want to do my HST in UK. Is it possible to maintain eportfolio during dnb general medicine in India.? Will that help to get the alternative core competency form signed off? Or the DNB logbook which we use in India will be of any help?
    2. In case if I can't get the core competencies signed off in India, how long will it take to get it signed off in UK via a Non training job?
    3. Is there anything I can do during my dnb course in India that will be helpful for HST application? Like doing presentations, posters, getting MRCP cleared etc.,,

    I have it signed from outside of UK by a consultant.
    I have 5+ yes of experience
    Is that enough? Or need an E-portofolio??

    Thank you for the valuable information. Could you please help me with the following please-
    Please Do you have to get this signed off while working at a registrar/act level or will JCF/st1-2 also qualify??
    Please do you know which areas qualify as critical care settings apart from icu, can I work in mhdu to meet the criteria
    Also 200 patients in acute settings- does this mean AKATS, or can we ask the consultant to fill simple CBD forms.
    I have done my MRCP and working towards getting this signed

      JCF/ST1-2 level
      Medical HDU is fine
      We don't need CBD for each of 200 patients. We keep an excel sheet calculator for these numbers.

    Amazing video Suman ! tq. just wanted to ask, I'm a new non training reg in gastro in UK. I have a 2 month rotation to acute medicine. Have no specific rotation to the ICU for critical care or geriatric ward specifically. How should I go about the 4 months of geri criteria ? Thanks ! Appreciate it

      Thank you for your kind words! It's great to hear that you found the video helpful. When it comes to meeting the geriatric and critical care criteria, you need to arrange 3 months posting after negotiating with your employer. This is the tricky part.

    Hello Dr Suman, If I've completed MRCP and have MSc in Cardiology in my home country, would this be the best route to take, or should is it easier get CREST form signed and go for IMT first.
    I would also like to know what kind of jobs should I apply for to get the form signed if I don't have any NHS experience.

      doing CREST+ IMT is more assured way of career progression.Any acute medicine, A and E or medical specialty jobs are good for CREST form.

      Hi, I don't know much about MRCP ireland.its not recognised in UK and also not by MCI as post graduate qualification.

      Sorry just a correction, MRCP I is an acceptable qualification to work in UK and recognised by GMC.

      Portfolio is mandatory for these competencies. Not possible without it.

    If some one is going to UK through MTI , can he get his crest and alternative competencies siged during that time only to directly go into speciality training. . Is crest even needed. Can we directly do alternate competencies to start super speciality training

    For surgical specialities is there a 2022 form for alternative core competency? Can we get it signed from India? What all difference will be there from medical specialities?

      I can only find 2021 alternative form for surgery. New forms might be available as soon as the application window opens. The latest form wants declaration from the signatory consultant that they are aware of UK core surgical curriculum. If you can find an Indian consultant who can declare this in writing, then it can be signed from India. Surgical form is quite different from the medical form. Please download the form and study the requirements.

      One of the option is to bring them as your dependent on dependent visa. But this is a very difficult option for your parents as they have to relocate to a new country away from their friends.also it involves the hefty visa fees. More practical option is they can visit you through tourist visa.

      For surgical specialties , you can purchase the portfolio.for medical specialties, NHS e portfolio will not be available.best is to keep paper copy of evidence.

    Hello sir , firstly I would like to appreciate for your videos . Really you are doing great job .
    Since March 2023 I am working as a specialty doctor in AMU & SDEC . I’m planning to apply for higher specialty training group 1(cardio) & 2 ( Medical oncology).
    I have done MD INTERNAL MEDICINE from India with 4.5 years experience after my post graduation, completed MRCP (Uk ), pg diploma in diabetes and renal medicine (RCP) and fellowship in critical care medicine ( India).
    My consultant is happy to sign in the alternate core competency form .
    Could you please guide me further regarding JPRCTB approved multiple consultant report ,how to show proof of 500 acute medical consultations?,geriatrics and ITU - which I have done in India ?

      MCR form is available in the NHS eportfolio. please subscribe to it. you dont need proof for each medical consultation. Your Educational supervisor needs to certify this in your portfolio based on your rota.
