I'm getting flashbacks a little bit hearing this again...
I hate how scary this sounds cos it's like Pyramid Head's scraping blade on a metal floor and you stumble into a dark room full of dripping dead bodies. Some hanging in all sorts of macabre positions with their bellies sliced open and their blood and intestines spilling out. Hell those might even be bodies that Pyramid Head himself has slaughtered. Some bodies on metal hospital beds and operating tables. The walls and floor painted crimson with blood and gore like something out of Outlast or Silent Hill. The stench of excrement mixed into the smell. Like All the bodies on the tables had bombs inside them like in 999 and the blood was from the detonations going off. Truly a theme with gore-filled room themes. That and Chill and Rigor.

I always like this theme its just 2 good reminds me of tht ine clock tower theme i think it was the 3rd one
