    I've been on semaglutide for 5 months, it's been one year total of exercise, weight lifting and dietary changes, and as of today I'm down 105lbs! I've been obese since 5th grade, this is the first time (at 35) that I've been under 300lbs in 25 years... Can't tell you how much this shit changed my life. Hard work, weight training, and protein intake make sure you lose fat but not muscle. It's working for me! Good luck everyone!

      If you have only been exercising for the last year. You added fitness, you cleaned your diet. Get off the drug. You have the rest of your life to keep exercising and maintaining a healthy lifestyle with good healthy habits. Exercise is the life time sentence, not this crappy drug. 💪💪#mindset

    At my heaviest I weighed 605; I'm down to 540 since April.
    I gained and lost the same 50 pounds for three years because I had become so insulin resistant; Ozempic has been key to my success.
    I've had side-effects: you definitely have to "behave" when you're taking it. You can't eat fatty or sugary foods, nor can you overeat, or you WILL experience intestinal discomfort.
    Before I found Ozempic, I was going to trying to get to 500 so I could have the gastric sleeve surgery. However, I've been so successful with Ozempic,I really don't see the need to surgically remove 80% of my stomach.
    But I haven't been relying solely on Ozempic: I hired a personal trainer, and I've been building up my stamina to walk longer and longer distances.
    I'll ask my doctor for that test.
    Here's what it boils down to: I'd rather be "skinny fat" than dead.
    As my personal trainer said, "You don't see too many old fat people."
    Why no, no you don't.

    You can modify the dosages in the pens by calculating "clicks". The spring mechanism works similarily in all the pens and each "click" corresponds to a certain dosage. For example, on a 1mg pen the full 1mg is 72 clicks while 36-37 is 0.5mg. Buying more needles allows you to customize how much you want to use as well as how often. I personally have had huge GI issues when going up to 1mg and will be testing splitting the dosages in two 0.5mg weekly doses to see if it works based on your recommendation.
    I was luckily smart enough to extensively research the drug and have kept up with resistance training and a proper diet, had a BMI of almost 40 and am now down 10kg in the last 4 months. I have deliberately been taking it slow and the effectiveness for the drug has not been as massive for me as for some other people.

    MD here with quite some experience prescribing GLP1s for some years now. Great video as usual but I wanted to provide 2 comments bc there are a couple of misleading messages here:
    -I am not sure what was the whole answer from Attia, but what is shown in the bit here it’s just not what good data shows. From the 2 main cohorts (sub-studies from Phase 3 trials) with DXA data, it’s clear that patients lose more fat mass than lean mass. In all studies available, body composition IMPROVES in these patients. So it’s just not true that they get fatter. It’s the opposite (I would need to see Dr. Attia’s data: baseline metabolic status, what GLP1, what dose, for how long, etc. - but the data available so far with GLP1s is pretty solid). Agree in any case that results will be even better with the diet+exercise adjustments described here, but the net benefit on body composition is already there.
    -Increase in HR: good discussion on a well-known adverse effect of this class of drugs. Agree that mechanism is still unclear (probably autonomous-mediated). But the most important thing I believe is there are a solid number of studies (and meta-analysis) with a huge number of patients showing that these drugs have a spectacular benefit on CV as well as all-cause survival. This means that despite the increase in HR, thr average patient on GLP1 should expect a 20-30% reduction in risk of CV mortality rather than be concerned by this well-known AE.
    Please do not overlook that the main benefit of GLP1s is that they are able to improve SURVIVAL in the populations tested so far, beyond the obvious and much hyped results on WL.
    And that’s probably why Elon made a good decision to get on them…
    2 minor comments:
    -The problem of the limitation of just one Wegovy 2.4 mg dose is right on point, but the company also relies on the use of lower doses of ozempic already on the market…
    -tolerability remains the biggest point of improvement of this drug class. We all feel this is just the beginning, and the upcoming new treatments will be even better wrt to these issues (nausea, sarcopenia, etc.). But I doubt they’ll be able to improve the already great benefit on survival/CV outcomes…

    I’m a pharmacist and Derek did a great job explaining all the injectable GLP1/GIP medications. I have personally managed diabetic patients on these injectables too and it’s about 50/50 for tolerability. Usually the worse side effects occur in the first 2-3 days after injection and they have a long half life so it takes about a month before reaching steady state (side effects become most tolerable at this point). Another thing is this medication has been responsible for causing pancreatitis and we will see an uptick or these cases as the years go on and more individuals are on them. Some studies are showing long-term side effects relating to metabolism, such as hair loss and an overall lower metabolism baseline.

    • @mavamQ
      @mavamQ 8 หลายเดือนก่อน

      What is about meth and losing teeth? Why does it happen? Ah, I ask ChatGPT, here's the response. "Methamphetamine abuse is associated with a phenomenon commonly referred to as "meth mouth," which involves severe dental problems, including tooth decay, gum disease, and tooth loss. There are several reasons why methamphetamine use is linked to dental issues:
      Dry Mouth (Xerostomia): Methamphetamine use often leads to a decrease in saliva production, resulting in dry mouth. Saliva plays a crucial role in neutralizing acids and bacteria in the mouth, helping to protect teeth. A lack of saliva allows harmful bacteria to thrive, leading to increased tooth decay and gum disease.
      Poor Oral Hygiene: Individuals using methamphetamine may neglect oral hygiene practices such as regular tooth brushing, flossing, and dental check-ups. Poor oral hygiene contributes to the accumulation of plaque and bacteria, leading to tooth decay and gum problems.
      Dietary Habits: Methamphetamine use can suppress appetite, leading to poor nutrition and unhealthy dietary habits. A diet lacking essential nutrients can weaken the teeth and gums, making them more susceptible to decay and infection.
      Bruxism (Teeth Grinding): Methamphetamine use is often associated with bruxism, or teeth grinding and clenching. This habit can lead to the wearing down of tooth enamel, chipping, and fractures, making teeth more prone to decay and damage.
      Vasoconstriction: Methamphetamine is known to cause vasoconstriction, narrowing blood vessels and reducing blood flow to oral tissues. This can compromise the health of the gums and surrounding structures.
      Chemical Properties: The acidic nature of methamphetamine can contribute to enamel erosion and weaken the overall structure of the teeth, making them more vulnerable to decay and other dental problems.
      It's important to note that "meth mouth" is not solely attributed to a single factor but is likely a combination of the effects mentioned above. I had gastric bypass. I come from a long line of big people, and having that surgery changed my life. But here’s the thing, you need to do so much work for the surgery to work. A lot of people think it’s cheating or a quick fix, but it’s the exact opposite. The failure rate is still insanely high. My doctor got on my an amazing program, and I worked with a great team, and I’m keeping off almost 300 lbs. I love working out. Which I never thought I’d ever say. Granted, I never hit my ‘goal’ weight or size(I’m 5’9 and stupidly wanted to be under 150). If I did, I would look so sickly. Haha but I am all for these shots and pills - if you do the work, to make sure you don’t gain everything back immediately. Like Remi Bader(?) was on it, and bitched about putting more weight on after getting off it. Well, she also has B.E.D., and she doesn’t sound like she was taking care of that or getting any type of treatment for it, while she was doing Ozempic. Which, I’m sorry - that’s stupid. Just because you fix one thing, doesn’t mean you fixed everything. The lack of education is irresponsible, and does more harm than good. IMO. Sorry if that comes off Kareny.

End of the day rebound is easy cuz people go on these crazy diets get fit but dont change the habits that made them fat, what happens after that is they get fat again. I agree. I'm on Mounjaro 5.0 mg and I'm doing resistance training, taking protein and creatine, and eating a rational diet. The result is that I've lost weight and I've gained muscles at the same time. It's a three-pronged approach. Of course, a well-informed doctor will advise their patients that these drugs should be taken with lifestyle changes (exercise and diet). Also, anyone who takes the time to read about these GLP / GIP drugs, or watches some You Tube videos about them, knows they have to do all three: injection, exercise, diet, injection, exercise, diet, and on and on. It's their own fault if they don't follow these guidelines. [BTW, from my experience, and from hearing from others, most people on these drugs do not eat fast food or rich, hight fat foods. Because we are no longer hungry, those foods no longer have any appeal. As those on the drug say, the food "noise" is gone. Also, because people know they still have to eat even though not hungry, they are more likely to eat a rational, healthy diet than a dirty diet. For that reason, I think your fears about bad eating on GLP/GIP drugs are not well founded. The real problem is when they discontinue these drugs; weight is regained. Also, the heart rate stuff is temporary and minor, and note that weekly doses are perfectly fine, not like TRT, plus, most people prefer the convenience of a weekly dose. Side effects are minor and temporary and mostly occur when starting on the drug or up-dosing to a new level.]

    Thanks for pointing out the nuances of this Derek. Guys like Peter often look at a study and run with it without critical thinking. Just because something happened to the average person in a study doesn't mean that will happen to any particular individual in the future. They need to do a study where they control for all the variables; protein intake, weight training, rate of weight loss, etc. I bet you'd see very similar results if you compared those taking Wegovy( or similar) and those who didn't when equating the variables.

    • @c6q3a24
      @c6q3a24 ปีที่แล้ว +3

      It's still important to learn what happens to "average" people. That information can be used to educate doctors, and patients, on how to safely use these drugs.

I kept increasing Skeletal muscle and lowering body fat. The biggest things for me is i never really wanted to binge eat or drink alcohol, which are my big downfalls in this regard. Gets rid of the cravings and binge eating I'd inevitably spiral into after 4 weeks. I'm pretty muscular and my maintenance cals are 3300. Eating only 2700 calories would previously make me ravenous. Now I'm stuffed with the same amount. I've even had a week or two where I tried only 2200 cals, and still experienced good satiety. I bumped the cals back up though due to gym performance and muscle retention. Only side I've experienced is some heartburn in the evenings if i have mildly spicy food.
    Started at 240, down to 222 right now after 8 weeks.
    I obtained a mixable vial and use an insulin needle to inject.
    I'm on trt too so my muscle loss isn't really an issue compared to the average joe Derek talks about in the video. My resting heart rate has also stayed the same at 66-68bpm. I have been getting 10k steps per day for the last year though.

    I am on 10 mg of tirzepatide every 10 days. I'm seventy-four years old. My battle against my eating addictions has taken its toll over the years. So as of last year, I had developed type 2 diabetes, coronary artery disease, peripheral artery disease, stage 3 chronic kidney disease, hypertension, non-alcoholic fatty liver disease, and hyperlipidemia. I was obviously losing the battle against my food addictions and was too tired to exercise.
    Mounjaro has totally turned my life and health around in 7 months. For example, my a1c is down from 6.5 to 5.6 and probably lower than that by now. EGFR went from 47 to 60, a degree of change that my doctor had never heard of. Virtually all of my critical health metrics are improved and I have even been able to cut back on my blood pressure medicine.
    The appetite suppression effects of Mounjaro have actually made it more possible for me to adopt a healthy diet, something that my food addictions would never allow me to do in the past. I'm just starting to get some energy in my life so now I am more able to exercise, both aerobics and resistance training. So for me, Mounjaro has not provided an excuse for me to continue my old bad habits, quite the opposite.
    I agree with Derrek's assessment, but don't let perfection be the enemy of the good. If you are you eating a crap diet, just eating less crap will improve your health. Then if you really care about your health, Mounjaro can free you from enslavement to your hunger hormones and help you adopt a much healthier diet. Once you lose some weight, there is a good chance that you will have the energy to start exercising.
    I have been blessed to have no gastrointestinal side effects. But the heart rate increase does concern me.
    It was not my choice to have a 10-day interval between doses. That was my doctor's directive and it was a bad idea. 10 mg increases my heart too much for about a week and then after a week I gain about 5 pounds before taking my next shot.

    Glp1 agonists make you lose more fat and less muscle than just dieting. All the studys show better body composition and higher lean mass proportion after trial. Wish you would've checked ncbi for lean mass loss like you did for rhr. The guy who said 2/3 is lean mass obviously pulled that number from his behind.

      @charlesdean640 ปีที่แล้ว

      That's in extremely obese people. Obviously their body is going to lose fat over muscle if they have 100+ extra pounds of fat. A slightly overweight person is going to lose more muscle than fat.

    • @karaardalan
      @karaardalan ปีที่แล้ว

      People lost 4 kg fat and 3 kg muscle, so overall 7 kg of bodyweight. No it wasn't morbidly obese test subject, because they were not weight loss studies, they were body composition studys, were no test subject had more fat mass than muscle msss so the change in body weight is accounted for and we investigate body composition only! I think a buch of scientists are smart enough to know that, and even I was smart enough to not tell you what a weight loss study said, but what a ton of body composition studys say.

    • @karaardalan
      @karaardalan ปีที่แล้ว

      This is why I was surprised derek didn't say that guy is wrong, because he said that those weight loss fda trials didn't account for body composition, which is not only wrong, because these are diabetes medication and literally every diabetes medication needs to have a favorable impact on body composition, otherwise it's not a diabetes medication it's a short term hyperglycemia relief! How do you approve a medication that worsens your glycemic control in long run for diabetes? What he said was also irrelevant because there are literally more than dozens of studys investigating the glp1 agonist group on body composition.

      The key to being successful with this stuff is immediately starting to lift. If you are too self conscious to go to the gym , grab a pull up bar, and a set of dumbbells. That’s all you’ll need in the beginning anyways. The difference between someone looking for the easy way and the right way is lifting.
      You can start with just 15-20 minutes a day see how you feel and work up to the hour.

      You know very well that the reason going to the gym and losing weight is such a reliable way of crawling out of a mental health hole is not because you look a little bit better. It’s because you re-teach yourself discipline, you short term and long term selves start to work in harmony, you make good decisions and look after yourself. After a while of treating yourself like you’re worth being looked after, you start to believe it, and you’re well on the path to success and happiness. Don’t rob yourself of that my man

      Not Derek but I'll give this a crack.
      When something isn't working in your training program, do the complete opposite. Flip your program upside-down. Source is Precision Nutrition (PN). The Principal of PN did an interview with Stronger by Science. More information there.
      Her advice concerned a client with exactly the same problem as you. Female exercising a lot (and under a lot of stress) was struggling to lose the last bit of weight. Her program was turned upside-down. Training went from 5 x week to 2 x or 1 x weekly. Rest days went up. Her calories increased from a deficit to maintenance. Major changes within a few weeks. Give it a go and see what happens.
      Also nothing wrong with *using* drugs like GLP Agonists instead of *abusing* them. Obviously these drugs are made to do one thing and one thing only - modulate appetite. Dr. Mike Israetel did a great video on the subject on the Renaissance Periodisation channel.
      Good luck V C.

      As long as you are looking better and are progressing in weight, the fat is and will come off. Don’t focus so much on the scale. Remember muscle weighs more then fat.
      I recently put on 20 lbs but I actually look skinnier cause the muscles. So don’t worry so much day to day or week to week the weight as long as you are progressing in weight and reps and sets and eating a balanced diet the fat will come off.
      Remember, it’s not a race; sustainable fat loss is about .5-1lb a week. Give it time and you will be rewarded.
      Personally , I agree with the other commenter; that you need your calories to do these lifts and gain muscle and raise the metabolic rate.
      It’s the muscle gained that raises basal metabolic rate that will do most for weight loss especially long term.
      You start taking a drug that makes you not eat, you won’t lift as much, you’ll lose muscle, then basal metabolic rate will go down and it will make it harder longterm.
      You are already on the right path.
      This drug, it’s for people who aren’t lifting to lose weight(ie lazy people ie people doing it wrong).
      Try upping protein to 1g/lb. That will make you lose weight and help with muscle. It’s hard to overeat when you are really getting the required protein intake as protein is very satiating and it’s actually a good bit of protein. If you can’t get it all from diet, add protein shakes.
