What is subcision for acne scars?
Subcision is a minimally invasive surgical procedure performed in the office of Dr. Behnam. It takes about 20 minutes and then the patients go home. To understand why subcision would work, we have to understand the anatomy of an acne scar. Rolling acne scars and boxcar acne scars are depressed scars, which means that the scars are below the surface of the skin. The reason why they are depressed is because there are fibrous bands underneath the skin that are pulling down the surface of the skin. The goal with subcision is to insert a needle through the surface of the skin and cut the fibrous scar tissue pulling down the surface skin. This allows the skin to be lifted since it is not bound down anymore.
A recent showed that using a handheld vacuum after the subcision procedure will yield much better results. In Dr. Behnam’s clinic, after every subcision procedure a handheld vacuum is given to the patient to take home.
What is subcision acne scar treatment?
Subcision is a minimally invasive surgical procedure performed where a needle is used to puncture through the surface of the skin and cut the fibrous scar tissue pulling down the surface skin. This allows the skin to be lifted since it is not bound down anymore.
Can I bruise after a subcision procedure?
Yes, it is possible to bruise after a subcision procedure.
How many subcision sessions do I need to see results?
Typically, several sessions of subcision are required to achieve satisfactory results. The results will be better if it is combined with other acne scar treatments, such as fractional CO2 laser resurfacing.
Can subcision be combined with other procedures, like CO2 laser resurfacing?
Yes. Dr. Behnam typically performs the procedure 2 weeks prior to the CO2 laser resurfacing.
Can I have subcision for rolling and boxcar scars?
Yes. it will work for rolling and boxcar scars. The goal of subcision is to remove the fibrous bands that are pulling down the skin and causing the depressed scars.
Can I have subcision for icepick scars?
No it will not work for icepick scars. TCA Cross technique is the gold standard treatment for icepick scars. In this technique, TCA 80% is applied sparingly in the icepick scar and let it frost. This will induce collagen production in the scar and lift it up.
What is a saline injection technique?
Saline injection is form of a subcision, where bacteriostatic saline in injected within the skin. The act of injection will stretch out the scar. When this process is performed multiple times, the act of stretching the scar will ultimately loosen up the scar underneath the skin, thus allowing the skin to raise.

The lighting in the first vs the 2 after photos is SO different and it MAKES a difference. Doctors like this are so insulting to patients, as if we're unaware of how different light conditions hit our own scars every day. There are some lighting angles where it looks like I have no scars at all and others where it looks like I have the worst scarring on the planet. And every room I go into, I know what lighting will make me look good or bad on a very specific scale. This is so negligent.
Yeah, they use lighting better than Hollywood. It is exactly what you said. It's insulting to the patient. I've seen photos online where they are actually photoshopped. After they steal your money, they proceed to use the image's of you to lure in patients. I've gotten better results at home with a derma pen and roller, then I've gotten spending thousands of dollars with these frauds.

@@Faithycakess Basically just Google it. I got mine the pen from Amazon, and the 2mm titanium roller from a company in England. You can get both on Amazon.

Not sure why people are saying this isn’t a huge improvement. It is! Took a lot of work and $$$ but amazing results.
You're an idiot if you don't see the way lighting is used in the photos, at best this alters the scars into something different, in most cases not in a good way.
how to fix acne scars: take pictures with harsh light in the before and soft light in the after so it looks like it improved with no improvement. there you go.
I hate that they always have to use a different angle for the after picture. We want to see the before and the after pictures have the same angle, the same lighting so we can see the REAL RESULTS.

Are there after photos with the same angle and lighting as the before photos? It makes a hugh difference, I can do this at home and have a 50% improvement without any work. I'm sure subcison and fillers help but it's a little deceiving when the angles and lighting is different.

All of these so called acne scar treatments are complete bs. Most are insanely pricey and the results are literally non existent. It baffles me how we have advanced so much in the realm of technologically but still havent figured out how to fix acne scars of all things.

@@MS-bw8qc look up Bellafill. It’s permanent and I have 7 syringes of it. Even if it says 5 years it’s actually permanent that’s why they do a skin test to see if you’re allergic a month before the procedure. Also each subcision will improve the skin by 50-70% and it will be a long term improvement , the skin won’t go back to being the same but yes multiple sessions will be required to achieve ideal long term result

@@alenarose7790 I know what bellafill is but the idea of taking a filler like that doeasnt appeal to me. The long lasting effects and damage it could do isnt guarnteed to not happen.
I underwent subcision in June 2023 in Iran. Today, in January 2024, I can note a difference. What they say is true - it does change, but it is not a magical transformation. I can estimate that my acne scars have improved between 30 to 40 percent. I also had some fat injections. So, I believe if I undergo subcision once again, it will likely improve further. However, I want to caution everyone against opting for laser CO2 and similar treatments, as they can darken your skin and take years to recover.

The after pics were taking in a different angle and lighting , this patient acne scars most likely look pretty much the same as before pics .

The before and after photos have to be taken in the same lighting at the same angles. If you take the after photo in diffuse lighting or at a more pleasing angle of the skin, the result is not accurate and it’s misleading.

bella fill has risks. some people experience little balls on their face from it , and its hard to remove them
Thats just dumb, not everyone wants to inject a semi-permanent filler that cannot be dissolved. Hyaluronic fillers have a much higher safety profile that can be readily dissolved with hyaluronidase.
Until you have temporary fillers youre not supposed to get bellafil. Any injector who tries to use this on you the first time is trying to make $$ it can cause semi permanent issues if fillers dont work well on your scars

I think that if you do subcision and C02 laser the results would be great! With the subcision will improve the scar depression then, with the laser, it will improve even more.

The skin don't turn back exactly the way it was before the filler. It always gets a little better than before, but probably the patient will require more filler for a while

It's supposed to help heal tye scars. The filler Is there to help the skin and collagen heal. The filler is to keep the fibers from healing back together

Guys, I've undergone two sessions of subcision and I barely notice any improvement. I also got pigmentation from it. Should I continue this procedure? Will it be worth it? Are the results of subcision permanent?

I did subcision before with HA filler. But the fillers was only use as a spacer. Sadly due to covid 19. I wasn't able to schedule 2nd session.

@@SS-hd5jx I don't know much about fillers they are many kinds. I'm not a doc. But this is how my doc explain it to me. When you do subcision you are cutting those fibrotic strands. Once it been cut your scar will lift. There is a chance that those fibrotic scars reattach. So he use filler as spacer, to lessen the chances for it to reattach.

In my opinion the best would be to either use radiesse or fat transfer instead of HA fillers.
It last usually 6months max....I did and it only lasted 5 months

Well it's the confusing part for me. There was a study where HA filler last longer than a year. It was seen MRI scan that HA filler was still on the body but move in diff location.

I had subcision and fully ablative co2 laser for a hypertrophic scar on my beard/moustache area right where they connect, which is an area which is an intersectino of a lot of facial muscles. After the subcision i feel like that part of my face looks different, less defined. that side of my mouth almost looks like a homer simpson mouth whereas before there was clear definition between my cheek and chin if you see what I mean. Really hard to explain and I understand hard for you to say without seeing, but is it possible that subcision could have changed the structure of my skin, the way it falls? Could he have severed lesser facial retaining ligaments? Or is it just sweling? I had subcision 4 weeks ago tomorrow and co2 laser 2 days ago.

The dr is in LA i paid about 2600 for what he calls the mega 3 things at once its subcisiion tca cross and prp i just did in 2 days ago so i still don’t know if is gonna work or not lol

Hopefully you didn’t waste your money on it. Look around for doctors who doesn’t charge this much for a subscission. Anything more than 300$ is way too much money for subscision. This doc seems like a TH-cam Derm with inflated prices. Also I saw him putting TCA in pretty much every scars. That’s gonna make anything except icepick scars the worse.
