I just wrote the same thing about him looking like he’s got cirrhosis of the liver and then I scrolled down to see your comment. I had a boss who died from cirrhosis around age 60 and he looked just like this during the last 6 months of his life. And like you said, it’s not actual fat, it’s a swollen liver.
