There seem to be some confusions from different video on TH-cam presenter's regarding form filling. Question 30 on the application form says: "Intended date of departure from the Republic of Poland". For example, a program of say 2 years. Some believes the answer to the above question should be should be 2 years from the program commencement date. In your case, it's 1 year going by the 365 days mentioned in Question 25. Which is actually correct ??

I don't see the need for a 2 years travel insurance going by the mail below from the Poland Embassy when I reached out to them. I advise 1 year + 2 months which is okay and save cost.. Furthermore, going by the 365 days stated on the form, the insurance shouldn't be 2 years but 1 year + at least 2 days as stated on the embassy website. Remember, these Insurance will not be applicable in Poland as you are required to purchase their national insurance. Travel medical insurance valid for the entire period of intended stay plus at least 2 days; valid for all Schengen countries; minimum coverage 30,000 EUR. mandator to "reserve" reserve means you make the ticket for those dates but you don't pay for it. when you get visa then you can make adjustments and pay for one way or return if you want

I made a mistake while filling by application form, now if you fill it u can’t come back and correct anything once you press save, my mistake was I put my travel return for Less than 365 days and the preliminary form were I first filled while booking for an appointment the travel return is more than 365 days. The whole thing just confused me and now I’m trying to see if I should reapply because I’m not sure if they will check the data when I come for the appointment and they now notice the dates are different. Is re registration advisable cuz I’m so scared I don’t know what to do. The worst part is when I tried printing I couldn’t print out because there was an error in the system but they gave me a number and said my application was successfully saved

Yes fill in student if you are a student. Your previous school should be what the address is.
Travel insurance valid for 2years would also work
