I just finished tracking down a low eGFR. My creatinine number was 1.51 with an eGFR of 47. This had me concerned as I am healthy in all other regards. I learned a lot during the time it took to resolve everything. One thing learned was that if you are a taller person, as I am, the height assumption used in the calculation from the blood work will skew the number lower than it really is. Also, I am an avid weight lifter who is pretty muscular. Creatinine is a metabolic waste from muscles. If your lean body mass is higher than the assumptions in the calculation, it skews the eGFR lower than your true GFR. In my case I did a 24 hour urine creatinine clearance test. It came back at 97ml/minute, easily in the normal range. So, if your situation is similar to mine, there is better than a small chance the eGFR is not an accurate measurement of your kidney function.

Very interesting. I do weights as well and I have ( according to body scanner ) 44kg muscle against the range average 28-35kg for my height and 63 years. Is the 24 hour test best or the cystatin c test ? My eGfr is 55 in June and has been up and down with scores like 54 63 62 42 56 since 2018.

@@michaelhussey440 24 hr test is the best measure of CKD. Urine albumin(micro/macro)/Creatinine ratio is a quick test in any local lab or better yet, 24 hr urine clearance test. I can't believe that this DOC talks about GFR and no mention of anything else.

thanks for this perspective. i’m 6foot so not too tall and got a result of 57 today with high creatine. i’ve been told to stop taking the supplement. i go to the gym 5days a week so your situation has made me think more about the veracity of the results

I’m 5’7” and my primary care provider did a regular check up on me back in January and my eGFR was 79 and creatinine was 1.22. I also had Hypertension consistently so she sent me to a kidney specialist since March to have me seen with renal ultrasound to see if I have renal narrowing causing the hypertension and lowered function. He said nothing was really wrong with that. He put me on a white meat diet and my kidney function eGFR jumped to 95 by the next visit. Everything else normal. Now my eGFR has stayed 87 for the past two visits which he thinks is due to the blood pressure meds. Still nothing else in my labs indicate a disease. I am also a weight lifter and weigh between 180-190 which is heavy for my height. Creatinine is naturally higher as a lifter. BUN, AG Ratios, all that all normal always. So either there is something unknown causing it or I am being taken for a ride.

I’d like to hear more about why there can be such discrepancy in eGFRcys vs eGFRcr? … in my case 65yo 6mi/day/365 eGFRcr=70 to 90 depending on time interval between exercise and plasma draw but eGFRcy is always 120 (both adjusted for body surface area)

Hello! My egfr is 72. I am 36 years old. Does this mean I have a kidney issue? Also the reason I am writing is I want to supplement creatine monohydrate as I workout 5 times a week and I want to know if it’s safe to consume or not?
Any help in the matter will be appreciated!
    Thanks!

I'm a 47 year old healthy female or so I thought. I'm never sick, I walk 15 miles a week and probably do 18 hrs of yoga a week..some hot and I teach as well. I had a terrible kidney infection in September. My creatinine is always high but now its higher than normal(supposedly I'm muscular) my gfr is 54 as of yesterday and has been falling, ugh...my blood pressure has also been high 160/100. What are the chances I have kidney disease

your changes are high with that high 160/100 BP for a 47 yr old so called healthy. You can't be healthy if BP is >130/85. To have CKD, GFR is not enough. Go & get urine test to calculate albumin (micro/macro)/Creatine ratio. If this ratio is

Why are you not talking about cystatin C? Creatinine is NOT the gold standard. You DO NOT diagnose CKD based on creatinine. This is poor quality healthcare

Kidney creatinine or muscle creatine kinase? High kidney creatinine is 120 so you might be getting things mixed up. Iv literally just found this out myself today after have full bloods done. Creatinine kinase is a metabolic measure which can be a high number like yours which would push the estimated gfr down as normal bloods don’t separate the 2! My kidney creatinine is normal but my creatinine kinase is 563 which was forced up through muscle damage (damaged metabolism) remember gfr is estimated not always a true reading. If you had a true gfr of 12 my understanding is you would need to on dialysis or you probably wouldn’t with us today. 15 or below needs dialysis. Try getting kidney creatinine tested alone. Hope this helps

60 to 120 is normal. I'm not sure if a small amount of us have hyper filtration or another word. It's normal for me to go back and fourth. Though, this has been new since covid dx x2 times in the same year. Creatine is normal with UA so I'm not concerned at the moment. I get labs every 3 months on avg.

    • @charlescarson5318

My eGFR is 108 over several blood tests. I started lisinopril for high blood pressure and it dropped to the 70s. I stopped taking it.
I eliminated carbohydrates as much as possible from my diet and my blood pressure is high normal now (130s/90s) I’ll take that compromise.
