What does consent look like? Is it a piece of paper a patient signs before treatment or is it something more?

If my patient is 11 year old, good, responsible kid for hand fracture. And if the father calls me and ask about how's the kid doing and when can the kid go back to his hockey classes?
Do I need kid's consent to talk to his father on phone about it?
