The reality of life is that you are what you think you are. If you think that sex is difficult. then it is difficult for you. At the end of the day, it is all about what you think, which in turns affects your behaviour and subsequently affects your life.

The reality of life is existence and existence sucks, sex is good but your body is ill-made or ill-replaced that by the end of the day you are left alone with the same despair.

But it is another perspective. Doesn't matter if it is right or wrong, it is fascinating how it is expressed. And it is true that there are annoying aspects to every act or behavior, but most of us find or tend to focus on what is pleasurable and that which we find wonderful, and i suppose Tsai tries to explore what he finds to be negative or troublesome maybe because it caught his eye. I like filmmakers and authors of this sort because they're showing us that they're humans as well, trying to explore their own problems and personalities through their own works.
