naw you ever seen the dude who had a sword through his stomach? if you do it enough times with proper precautions the skin and muscles will heal and turn into scar tissue and stay like that. so he basically has stuck it through his arm enough so that when he does it himself he can find the proper path the needle needs to go and makes it through.

I doubt the scar tissue thing is real. Not through his arm and both hands.
A clean and well made/maintained needle won't leave much tissue damage for the body to repair. No air is introduced into the puncture, it's relatively thin, so the pressure exerted normally by the body's tissues is enough to keep the wound sealed together. Because the needle only has a point, and not an edge, he didn't have to really worry about it severing any blood vessels. There'd actually be less scarring from this sort of damage.

FYI, how the "trick" is done: Beforehand, Blaine purposely punctured his arm to develop a fistula (a "scar tunnel") in his arm. That's why when Blaine punctures it again, it doesn't bleed.

Absolutely real. Zamora taught him how to do it. I saw Zamora do it three times literally 2 feet away from me, I was front and center stage at a local venue I could have reached out and grabbed the needles. He did it not only through his arm but also his neck coming out his mouth. Absolutely real!

You can tell David was actually in a lot of pain. He's just a mastermind of body language, so he hides it well unless you're looking for it. He was clearly very adamant about getting the needle out, and actually looked at the camera visibly frustrated when Ricky was stalling at around 2:50. His grins are disguised winces of pain. And while the needle was in, he swallowed several times, which he typically never does because he keeps so cool when the tension is high. I'm not sure how he did the act itself, but it entailed a whole lot of physical pain. What a trooper.

He's not actually piercing the skin, that would bleed a lot plus a normal pointy needle couldn't possibly penetrate the tissue without a LOT of force.
Legend has it Ricky is still sitting at that table trying to figure this thing out.
He did stick it through his arm. There are ways of slowly feeling the needle through your arm in which you can avoid all the vessels and arteries (which is why there was no blood) and to minimize pain. It takes a lot of practice and very steady hands. There are even videos of gurus and such who stick needles through their torsos in the same fashion. It is in fact real.

A normal pointy needle would need a tremendous amount of force to go inside the skin at that diameter. I think he's searching a kind of channel with scar tissue (from actually piercing his arm before and letting it heal). That's why It's not bleeding and why he's searching a spot on his skin to insert the needle.
