    The Cost of Poor Documentation:
    If documentation is problematic then this can impact various aspects of healthcare delivery, including patient safety, revenue cycle reconciliation, and inventory control.
    * Revenue Cycle Reconciliation: A report from the HFMA reveals that U.S. hospitals lose an average of 1.5 billion dollars due to uncaptured medical products.
    * Supply Chain Inefficiencies: Another HFMA report highlights that an individual hospital incurs an average cost of $1.3 million per year due to hospital supply chain inefficiencies.
    * When we consider the bigger picture, the impact becomes even more significant, and with a $7.7 billion price tag. It affects revenue cycle, supply management, physician preference cards, item master issues, contracting, regulatory compliance, and the cost of lawsuits.
    Challenges in Surgical Data Collection:
    Four Key Factors: We’ve identified four main factors contributing to this problem.
    * There is a heavy reliance on nurses to collect data during procedures. Mostly, the nurses don’t see data collection as their job, their role is patient care. From one side, we need nurse support in this task, but on the other we need to reduce the administrative workload and enable nurses to focus on the patient.
    * The dependency on barcodes is an issue in hospitals. Typically, when nurses barcode scan an item, they struggle.
    * Point of care software. Imagine you are a nurse, standing in the operating room, using EPIC, McKesson, Cerner, all of this complex software. You need to select from a drop down here, and fill in information there, you need to enter the batch data, the expiration date. The complexity of the software is an issue. Nurses need system training, but they are always changing shift, changing location.
    * Hospital Item Masters are often not up to date and if you want to read the manufacturer’s reference number and then associate it to the hospital’s reference number it just a very complex task. When the nurse tries to report something to the patient, in many cases she gets a message on the screen that the item is not in the database.
